The British public is very conservative when it comes to reopening. 73% support prioritizing the health of citizens only 17% say prioritize the economy. This is the highest of industrialized nations, Japan being the next highest with 60% supporting prioritizing health only 16% in Japan saying the economy.  For Boris Johnson as he makes the speech on Sunday May 10 on reopening the political margin for error in decision is nonexistent. Britain's tabloid press and other media simply took the idea that to heck with it lets reopen reflecting a lack of caution in the headlines after Mr. Johnson expressed his intention to reopen. After seeing this Johnson and his closest advisers met without his hawkish ministers to reflect on what was happening in the country. The British government's scientific advisers say whether there are 100,000 deaths by the end of the year depends on many factors including testing, contact tracing, the way the lockdown is eased, the situation at nursing homes, and other government action  to prevent a resurgence in infections. At the meeting with Gove, Sunak Raab and Hancock, Mr Johnson stepped back and reversed any plans except for mild reopening- giving people more time outside for exercize, opening limited locations such as garden centres and advising strongly to wear masks on public transport. Both Johnson and Dominic Cummings his adviser had coronavirus, and Johnson spent some time in ICU. They know the impact of the coronavirus from their own personal experience.  For Johnson there is only one chance, Tory senior advisers say the public will forgive mistakes going into coronavirus, but will never forgive mistakes getting out of cotronavirus. He told Keir Starmer of Labour in parliament that he bitterly regrets what has happened in nursing homes. The scientists have warned him that the staff at nursing homes could seed communities once again. And that the coronavurus R ratio (1 being the level it starts growing again) could go up back to 1. This is the situation on May 10 as Johnson prepares to speak to the nation on Sunday at 7 pm, as he shifts to "maximum caution." ...

Dr. Nancy Krieger of the Harvard School of Public Health says data on health risk -including which counties have high rates of health conditions such as obesity and other diseases- should inform targeted interventions and public messaging. This is important for administrators and health authorites in each county and state as they deal with a surge in coronavirus in parts of the U.S. which have so far been only slightly affected such as rural southern and western states. More recently places such as North Dakoa which the virus missed in the early stages are now seeing a jump in cases. This report shows counties in states such as Tennessee, Georgia, Alabama, Mississippi, North Carolina, particularly susceptible because of high rates of obesity and other health conditions. In all almost half of the U.S. population has one of these health conditions- obesity, diabetes, high blood pressure, lung disease and heart disease.  The high prevalence of obesity which worsens all other health conditions is a result of the lack of healthy food and nutrition, exercize and healthy living habits, that were neglected in the closing decades of the twentieth and early twenty first century. The shift to processed foods is only one of many bad practices that happened in these years. The lack of attention to healthy eating and nutrition, and outdoor exercize has marked this period and created conditions in which the virus could damage health. ...

US life expectancy dropped by 1.5 years to 77.3 years, the largest drop since 1943. This takes life expectancy back to the level in 2003. During the pandemic of 1918 the drop was 11.5 years, showing how much difference vaccines and modern medicine can make. The drop is a result of coronavirus, and added to this are the complications for people with other diseases including chronic liver disease, which is connected to use of alcohol. There was a decline in asthma and cancer related diseases. Delayed treatment for health conditions because of lockdowns and homicide increase were other causes of the drop.

Isolation, stress and disruption of normal diet and exercise will have effects still to be seen, say experts.

As part of the effort to get Britain to cut all that unnecessary fat which is ruinous for healthy living, Britain will now ban all buy one get one free deals on unhealthy food products. All food in restaurants and pubs will soon be required to post the calories count on menus, starting with the bigger chains. Boris Johnson says he is supporting the NHS and the national campaign for fitness and cutting out all the unnecessary calories that have crept into people's food habits and intake. He never intended this but Johnson like others have learned from one experience that has compressed several lifetimes of experience trying to recover from the virus without the fitness of body that is needed.      ...

People have to take charge of their own lives by eating healthy food and lots of fruits and vegetables, to reduce obesity in this pandemic. Studies show that people with obesity were twice as likely to end up in hospital, and 74% more likely to end up in intensive care. Efforts to rid our diets of sugary drinks and junk or processed foods need to be escalated, and exercize, walking, cycling, other activity need to be made part of our daily activity. French president Macron announced a new lockdown starting October 30 that last till December 1.  Under this second lockdown people can leave home only to go to work, to go to school, to give assistance to loved ones, for essential shopping and for 1 hour of physical exercize. People will have to show documentation when leaving home. Travel between regions is banned. Bars and restaurants and nonessential businesses will be closed. Universities and higher education will be done online. Schools will remain open, essential businesses will remain open. Most public services will be open. Small businesses will have access to 10,000 euros per month of assistance, employees get short term work assistance, and people having trouble with rent receive assistance. It also means woking hard to be able to change one's life in many positive ways and believing that you frame your own destiny. It is said that a wandering mind is often more of an enemy than a friend, in ancient wisdom. Developing a new calmness, being able to meditate, requires putting a distance from negative thoughts and not believing the thoughts of a wandering mind. This is called cognitive defusion. It is also part of what is needed for yoga and meditation.

WSJ looks at the changes in the way medicine should be practiced in the light of what we have learned from the pandemic.  Medicine practiced before the pandemic and still today relies mainly on a visit to the doctor or specialist who is short of time. There is a shortage of doctors. Patients have many illnesses as a result of decades of neglect of proper nutrition, and exercize habits. Obesity is at about 40% in the U.S. about 30% in the UK and 17% in France, and high also in other parts of the world. These high rates were unknown throughout history and result in many illnesses and increase by four times the vulnerability to the coronavirus. One authority in medicine calls obesity pouring gasoline on a fire for effects of the virus.  A doctor's appointment with doctors short of time with no coordination around a whole range of factors related to obesity, illnesses, health checkups, mental health, is now seen as a heavily handicapped way to practice medicine or for patient healthcare and wellbeing. The alternative is discussed here as the way forward. A  team will be responsible for a patient's care not just an individual doctor. The team would care for general health after a patient's checkup, cover individual illnesses, weight issues, mental health, exercize nutritional needs and other good healthcare habits. Instead of relying on doctors at a time of shortages of doctors the team would be led by nurse practitioners.  A nurse practitioner is someone with a bachelors degree and a masters degree or doctoral degree in nursing with 1000 hours of clinical training. Studies have shown that they are effective and even more effective than individual doctors. Today particularly with the problem of doctors with limited time compounded by the built up problems of decades of bad habits in nutrition and exercize and poor "cultural" habits getting entrenched, there has never been a greater need for a better way to practice real healthcare for a person's wellbeing. Particularly in rural areas with an even larger shortage of doctors the health practitioner led team will play a big role. Patients will under this setting receive more care virtually and get more followup care by phone and video messaging. The numbers tell the story- there are shortages of doctors in USA, Europe, Africa, Latin America and Asia. In the U.S. shortage of doctors is 55,000 projected to 2033 by Association of American Medical Colleges. There are 290,000 nurse practitioners licensed in the U.S. and 131,0000 physician assistants. The goal will be to get an adequate number of nurse practitioners licensed in this decade to take care of these teams. The pandemic has made virtual visits to doctors and nurse practitioners popular. Medicine reimbursement should and would be practiced on the basis of how well a patient is doing not on a fee for each micro service that is delivered. For this to happen the teams led by the nurse practitioner have to commit to patient education of the benefits from good practices and good habits for nutrition, exercize, caring for oneself. A doctor short of time is hardly the person to carry on this patient education which is where the major opportunities for a new system arise. The virtual care also provides a new medium for patient education and awareness of the risks of getting illnesses, preventive actions to be taken in advance. One approach being tested in California and Texas is for a monthly fee for patients more payments by health plans to doctors or healthcare teams if the patient is healthier. Additional health professionals are added to the team including health coaches, dietitians and medical assistants to increase its effectiveness in counseling and education and monitoring.  The nurse practitioner team approach is already being practiced in parts of the U.S. including the example of New Hampshire shown here, and is predicted to be the approach for primary care in the next decade. ...

During the first days of vaccination drive in India the uptake is lower than expected, below 48% in many states of India. Healthcare workers are coming forward in smaller numbers. The glitches in the app that was used to transmit information is one reason, the other being that not enough information was disclosed about the two vaccines.  On the first day Jan. 16 India vaccinated 200,000 healthcare workers. The target was 300,000. The uptake is lowest in states such as Tamilnadu and Punjab.  One of the reasons for the low uptake is also that India has a lower number of cases, and cases are in decline in India for coronavirus. The first wave peaked on September 13 at over 90,000 and has declined ever since to 13,000 on January 19, 2021. The Indian diet includes vegetables and a large part of the country is vegetarian. Indian diet also includes herbs such as turmeric, ginger and cinnamon, and many others that help build immunity. The lack of cars for large numbers of the population means a lot more exercize walking long distances is essential in daily living. Another factor is that the mandate for masks is strictly followed everywhere. Also helpful is that after independence in 1947 the pharmaceutical industry was built up to serve the large population with prices regulated carefully by the government, enabling steady supply of medicine during the pandemic. ...

"Feierabend" is the German word for when you stop working for the rest of the day. It is also the period after stopping work and beginning a period of leisure and rest. Germans working from home for government, in software industry and in other business places, are using this time when work stops at say 4 or 5 pm to shift to exercize. Here Nils Backhaus, of Germany's Federal Institute for Health, marks the end of a day's work by taking his racing bike and cruising along the the tranquil landscapes of the Ruhr river, just outside Dortmund. Stress and recovery go hand in hand, it is the bodily rhythm for Nils Backhaus and many Germans. You cannot do double time the next day. You have to first regenerate and get renewed after a day's work. This improves both health and the productiveness of work. It also creates enthusiasm the next day to begin work. A clean disconnection is needed says this report and "Feierabend" helps one do it. Workers working from home can end up working too much with no demarcation that ends the day. During the coronavirus with many workers working from home this demarcation has been lost for many people resulting in overwork and fatigue of body and mind. Microsoft 365 team has seen this surge to the point where managers have the software do this demarcation to stop work, and ask people using the software to say how they feel. What better way than something like Feierabend where one makes a clean break from work and goes out and does something completely different. It gives the mind and body a chance to rest and to regenerate. Prof. Rothauge of the Catholic University of Eichstatt who has studied the history of the evolution of work says this comes from an historical context. The industrial revolution introduced new work habits and days structured around work routines. This also provided a period of breaking away from work to rest. It was important to see the rest period as a way to regenerate not simply engage in some other equally taxing work. It was all part of the same coin, resting and renewal of mind and spirit after a day's work ended enabled one to make a fresh start the next day. It was what made productive work possible and an integral part of it.  During the coronavirus it is very important to do this regeneration and renewal, and to start this by having a clear disconnection from work after several hours of intensive work or a day of normal pace work. Intensive work of 3-4 hours or 4-5 hours can make up a days work at home because of the uninterrupted nature of working remotely from home, say workers with extensive experience of working from home. At that point break away and make the clean break or disconnection to regenerate and renew for the next day. ...

About half of UK adults reported experiencing high levels of anxiety says the Office of National Statistics for the the first lockdown. During the second lockdown the fatigue coming from that period in March and the higher level of cases is likely to have increased the number of adults experiencing anxiety. Anxiety was experienced from the loneliness and the effect of the coronavirus on work. A quarter of all adults reported increased sleeplessness. Stockpiling, overworking, irritability, micro-managing, and alcohol consumption, are some of the erratic responses to this level of added anxiety. Experts suggest different responses. leadership and incremental change to put the problem into context. Such as in the case of coronavirus the important behaviours that one can control such as masking properly, social distancing properly, ventilating and cleaning the air with aircleaners, using necessary caution in outside exposure by limiting to the essential, and taking nutrients for defence against virus, other actions. After putting these in place the risks can be minimized.  At that point focusing on the present is seen by experts as the right way to respond. Get through this period or this week first, leave the next week or the next period for next time. To do this  baking, reading, hobbies, running, walking, yoga, gardening, and outdoors, a whole range of other activities including watching sports, listening to radio and music, all fall into this. Spending time doing things that make us happy. A good exercise is asking how does this make me feel, am I more relaxed? am I happier? Put things in perspective, is this catastrophising? Is this making it bigger than it is? Can I put away the illusion of control when control requires some higher power such as God. Can I leave that part to God, to the divine. Cognitive behavioural thinking modification is a way of tackling stress, loneliness, and the depression and anixety that feed on each other. Being aware that we may have wrong behavioural responses, asking questions about how accurate our thinking about things that pose threatening situations is, are helpful in tackling the anxiety.   Just breaking the pattern of behavioural responses of repetitive thinking is helpful by engaging in other activities. Meditation is helpful. Yoga is helpful. In this pandemic learning about nutrition and increasing one's knowledge of food, eating and exercizing right, of cooking, is a useful way of turning a negative into a positive.     ...

France was exceptionally well prepared says France 24, citing a report in Le Monde, for the SARS crisis in 2002 and the H1N1 influenza in 2009. A billion masks were stockpiled by 2009. Following the H1N1 influenza not appearing in any significant way the media, political parties and the public shifted their attention away from public health crises preparation. For H1N1 the government spent 1 billion dollars some of it going to pharmaceutical labs. The eurozone financial crisis that followed the global financial crisis shifted policy to austerity measures. The entire preparation effort for influenza type health crises was abandoned as too costly.  The same pattern repeated in Britain which was also well prepared before 2010. Austerity budgets after 2010 had little room for public health investment.  One could say a similar pattern was seen in the U.S. Today the worst hit countries are U.S., Britain, France and other European countries. France which had 1 billion masks in 2009 to tackle a possible H1N1 epidemic finds itself with 150 million masks in March 2020 and scrambling to find masks. Some masks which were usable were even destroyed as expired, ministers and experts who had built up the prevention effort in 2009 were even demoted and forgotten, as was much of the preparation in these years. It wasn't just medical supplies pubic awareness had practically disappeared. In the U.S., in Europe, the same situation of a lack of public awareness so that experts, government, and the public could work together quickly, was clear to see. In countries such as Taiwan the preparation led to speedy response at all levels, making contact tracing, isolation of clusters effective. In the U.S. and Europe this early, early, period was lost leading to makeup mitigation measures and the growing sense of a loss of control over the virus. ...
