A third of the Medicare budget, she says, goes to the last year of life, and a third of this goes to the care in the last month. At the same time public opinion polls show Americans see the problem being too much medical intervention rather than too little. A Pew Research Center poll in 2006, shows 22 percent say a doctor should not always make efforts to save a sick person's life, and 70 percent believe that patients should be allowed to die in some situations, when it only prolongs life for a few months for instance and patients live in pain. The problem only gets worse in future years as an estimated 8.5 million Americans are expected to be over 85 years in 2030 with these same choices facing patients, their families, Medicare and the country....

Kristof of the NYT says the narrative about Hillary Clinton being dishonest is clearly overdone and inaccurate. He says the gaffes about the use of personal email server have unfairly created a distorted narrative. On changing positions on trade and minimum wage- this happens frequently with all the candidates. Some are glib enough for not being noticed, Clinton not a natural politician drawing attention. On Libya, and on Syria, Clinton is blamed for the President's errors and not given credit for pushback in league with Panetta, Gen. Kean and Gen. Jones, that has influenced changes made in 2016, and the president accepting blame for errors. In this instance Clinton has been far from shifty by publicly allying herself with Leon Panetta's position in "Worthy Fights," and Gen. Jones's remarks. Far from having erred on Libya and Syria policy, Hillary Clinton, like Chuck Hagel and others in the administration including Joe Biden, showed exceptional patience in dealing with the president, national security advisor, and McDonough in policy matters- when they were right but the country was weary of what were seen as futile global engagements in remote areas setting too high a bar for any action. Clinton rightly described this as a pendulum swinging too far in the opposite direction after the Bush/Cheney years. On young women and being "progressive" Hillary Clinton is from a older generation that experienced the kind of discrimination that young women fail to grasp, according to a recent analysis of University of Massachusetts polling survey results cited in the Washingon Post. A PolitiFact Pulitzer winning fact checking site shows 50% of the Clinton statments are either true or mostly true, compared to 49% for Bernie Sanders, 9 percent for Trump, 22% for Ted Cruz, and 52% for John Kasich. ...

Leslie Gelb says US should focus on its strengths, on the areas that it can build on to come up with something of enduring value and not focus all one's energies on situations and troublespots that siphon off a lot of American energies. This is a good idea and should be something that policymakers at State and the National Security Council and advisers to the President could keep in mind. There is alot to be said about the Truman example of the Marshall Plan in building western Europe after the war. At the same time its not as clear cutthat troublespots and the difficult work can be avoided. Was the Berlin airlift a troublespot? And Truman had little time to respond to the invasion of South Korea from the north, acrisis he would have chosen to avoid, if he had the choice. Instead he was drawn into along drawn out war on the Korean peninsula. Still the idea that you want to save your energies and not dissipate them in conflicts which can't clearly be won is a useful one. The Korean war was fought on conventional terms so the US could point to success years later in South Korea. But for conflicts fought in the jungles of Vietnam or in the remote mountainous terrain of Afghanistan, limited objectives and willingness to consider history, customs, and local tribal cultures is very constructive. ...

The Census Bureau reports that 46.2 million Americans were living in poverty in 2010. This is an increase of 2.6 million over 2009. This is the highest number of Americans living in poverty since 1958, when this statistic was first measured. Poverty is defined for 2010 as income at or below $22,314 for family of four. Also relevant is the median household income which went down to $49,445 in 2010, a decline of 2.3% from 2009. The typical household earned less in 2010 than in 1997, in inflation adjusted terms. The Census Bureau reports 16.3% of Americans had no health insurance coverage in 2010, the same as 2009.

This op-ed essay by the President of Toyota Motor Company, Akio Toyoda, talks frankly about the situation that led to the quality failures at Toyota and promises that Toyota will try to live up to the ideals that shaped its beginnings some 70 years ago. Toyota had humble beginnings in the thirties when the original founder of Toyoda was in the textile loom manufacturing business. Following a new venture in automobiles in the 1930's, for three decades Toyota was only trying to catchup with the U.S. in auto manufacturing. It started with the current chairman Shoichiro Toyoda's father -and Akio Toyoda's grandfather Kiichiro- visiting a Professor of Metallurgical Engineerig at a university in Tokyo to collect ideas and information for entering the automobile manufacturing business. Akio Toyoda seems quite cognizant of these beginnings in this essay. Action steps he mentions are a top down review of global operations, establishing an Automotive Center of Quality Excellence in the United States, and asking a blue-ribbon safety advisory group of outside experts in quality management to independently review Toyota's operations, with the findings made public. Akio Toyoda points to the lack of effective communication in quality matters in its global operations that led to the problem festering for so long. He says that in regard to sticking accelerator pedals, Toyota "failed to connect the dots between problems in Europe and problems in the United States because the European situation related primarily to right-hand-drive vehicles." Toyota also moved to address problems in its Prius and Lexus HS250h models for anti-lock braking systems....

The Daiichi Fukushima nuclear reactors are of a older 1970's design. A Wall Street Journal analysis of the safety record at the plant shows a safety record that is worse than other nuclear plants in Japan. Experts say the maintenance practices at the plant contributed to the nuclear meltdown. In the US spent fuel is retained in the nuclear reactor vessel during maintenance. At TEPCO's Fukushima plant the spent fuel is removed from the plant during maintenance and depends mainly on cooling water to keep the heat from building up. The additional protection of the reactor vessel is not present if something were to go wrong, as happened with the earthquake and tsunami.

President Obama's proposal on Dec. 17, 2012, in the fiscal cliff negotiations sets the figure at which Bush era tax cuts are permandently extended at $400,000 instead of the $250,000 in earlier proposals. Speaker Boehner's Republican proposal was for a figure of $1 million. The $400,000 proposal would mean that the top tax bracket of 35% would increase to 39.6%. Currently the tax rate increases to 35% from 33% at the cutoff point of $388,500. The White House plan now cuts spending by $1.22 trillion over 10 years. $800 billion comes from cuts to programs, with half of these cuts in federal health care programs, $200 billion in programs like farm price supports, $100 billion in military spending, and $100 billion in other domestic programs over which Congress has control. The White House proposal also supports additional spending on infrastructure, extension of expiring unemployment benefits, protection of "vulnerable populations" such as the disabled and wounded veterans on Supplemental Social Security benefits in inflation calculations, and permanently stop expansion of the alternative minimum tax affecting the middle class. On business investment the president's proposal would make permanent the credit for corporate research and development....

Ajami makes the point that opinions and attitudes -after the Obama efforts to improve America's standing in the Muslim world - havent changed much since the Bush days. He cites the Pew Global Attitudes Survey for 2009- In Turkey after the Obama Anakra visit favorable rating is only 14%, 69% are unreconciled. In Egypt 27% have favorable view 70% do not, in Pakistan unfavorables actally went up from 63% to 68%. He also points to the situation in Iran where the protester for the fraud in the election of Ahmadinejad did not receive much supprt from Obama, as the Obama administration decided to engage with Ahmadinejad to achieve nuclear settlement. In effect the rhetoric from Obama has not been matched with courage of convicitions , and lacks the courage to turn a new chapter by breaking from the past not just with talk but in real policy changes. And says Ajami the Arabs havve stopped listening to the rhetoric as little has been accomplished by way of change. At the same time false expectations may have been aroused because the Cairo speech was made at the University with the aging Mubarak at Obama's side, and beyond addressing these students the feeling clearly must be that the US would simply continue its policies of supporting old regimes that tolerate no dissent of any kind such as Mubarak's. ...

Now there is asplit in the Islamist parties, between the factions working within the democratic process and other factions including younger militants who are against Musharraf and elections. IBM's researchers predict five developments in new technology in the next five years to 2016- precise language translation, precise voice recognition, storing of biometric information to replace passwords, conversion of energy from walking or water moving through pipes to power small devices, search engines and software that gives people the information they want. A novel application is conversion of the approximately 65 watts of energy generated from walking to power devices such as a phone. Even though E-Bay executive Meg Whitman has outspent Jerry Brown by $130 million, much of it her own money, she trails Brown by 8 percentage points in an Oct 20 survey by nonpartisan Public Policy Institute of California. That survey showed Brown leading 44% to 36%. She had a hard line stance on illegal immigration in her ads for the primaries, which changed later in the campaign to attract Latino voters. Her campaign crafted by consultants focussed on three themes- cut government spending, create jobs, and fix education. Brown is a former Governor of the state and current attorney general. Experts attribute the lagging Whitman campaign to voters leery of 2 kinds of politicians- the first is the career politician and the second is longtime corporate executives. Because of last minute surges in the polls, a lead of 8 points or more is considered necessary for the governor's campaign, for a candidate to win. Another factor is that Governor Schwarznegger came in with no political experience, and was beset by the state's fiscal troubles, with his poll ratings now at 28%....

And the Economist emphasizes as General Chrystal's chief conclusion: "an insurgency cannot be defeated by aforeign army alone." It points to General Chrystal's view that success means winning the support of the people, and the loss of faith in the government of Karzai.
