U.S. health insurance company WellPoint Inc. will offer higher payments to primary care physicians. This is part of new strategy to reduce emergency room visits and costs after deterioration in a patients condition by relying on primary care physicians for better care at the outset. Payments to primary care physicians will be increased by 10%, with higher payments when the results show better quality and preventive care. WellPoint has a network of 100,000 primary care doctors. Physicians who meet certain goals such as lowering the overall cost of a patient's health care costs will be given an incentive of 20-30% of the savings realized. The new effort will add 1-2 percentage points to the 6-8% of the $100 billion that Wellpoint pays for claims processed each year. WellPoint's management sees a reduction in medical costs of about 20% by 2015 as a result of such efforts.

In the case of Vytorin study of Merck and Schering Plough researcher Kastelein says he regretted going along with an attempt by the drug companies to change the original plan. The companies decide eventually to stay with the original plan.

A trial by Merck and Schering Plough that lasted 2 years with 720 patients in the Netherlands found that while Vytorin helped reduce cholesterol more than Zeta taken alone, the plaques of patients on Vytorin grew twice as fast as the one on Zeta alone. Vytorin is a combination of Zetia and Zocor in a single pill. About 60% of patients take Zetia in the Vytorin form. Zetia by itself reduces cholesterol by 15-20% but its not clear if it reduces the plaque formation in arteries at all. The trial was completed in April 2006 but released now only after articles were written about it in the media. Zetia and Vytorin account for 20% of the overall cholesterol lowering drug market. Its about 70% of Schering's earnings and about 100 million prescriptionhave been filled in US so far. The drugs cost $3 a day. For Merck its reputation after the Vioxx issues would be affected by these results leaving some doubts in the public mind.

It predicts a "disorderly exit from the euro" without further help. WSJ reporter Bob Davis writes this report on the end of the China economic miracle in 2014 as he completes a 4 year assignment covering China. He says China's economy is slowing rapidly and he is pessimistic abou the future. Construction cranes visible across China's skyline says Davis, can no longer be interpreted as growth inducing. With rows upon rows of empty flats in third and fourth tier cities which account for the bulk of the increase in housing construction, the consequences of a debt fueled construction boom are easy to see. Davis cites the IMF on the dangers of credit fueled growth in China- only 4 countries have experienced as rapid an increase in credit to GDP ratio in 5 years. Each of the 4 countries Brazil, Ireland, Spain and Sweden experienced a sharp decline in GDP growth and banking crises following the credit bubble. Estimates of debt to GDP are as high as 250% for China. Krugman, Roubini and other economists have warned about the credit bubble, saying China is no exception to the rule for the risks posed by such a bubble. ...

A Pew Research Center poll before April elections shows 70% of Indians dissatisfied with conditions in the country. 63% of those polled say they prefer the opposition BJP party to lead the next government, compared to 19% for the ruling Congress party. More significant was the favorable view of Narendra Modi by 78% of those polled, with only 16% holding an unfavorable view of Modi, the chief minister of the state of Gujarat which has seen fast growth rates. The Congress party has been hurt by a series of corruption scandals, weak leadership and poor management of the economy as growth slowed in 2012-2013.

The Congressional Budget Office report in 2011 shows after tax resource flow that a family has to pay for consumption, a better approach to measuring the growth in incomes since 1970 including government help to lower income people and gains in the stock market for upper class Americans. This report shows after tax resource flow for the top 1% in the U.S. tripled from 1970 to 2011. For the middle fifth of the distribution families experienced real net income gains of 36 percent, and the bottom fifth of the distribution real net income gain of 50 percent.This suggests gains of about 10 percent a year if averaged over 30 years for the top 1 percent compared to 1% a year for the middle fifth and 1.5% for the bottom fifth. The report was done in 2011 and this could skew the results. Between 2011 and 2015 the stock market recovered and this would suggest a much higher gain for the top 1% of incomes and the top 10%, while also providing improvement in incomes for the middle fifth and the bottom fifth as unemployment decreased. Working class and minimum wage slowly recovered, and interest income on savings extremely low, with large student and other household debt, so that even at 10-12% gains per year for the top 1%, and 1-2% for the middle fifth of the distribution and 1.5-2% for the bottom fifth the last three decades have not been good for working class and middle income Americans compared to the the period 1950-1970 early postwar period recovery....

The Egyptian military announced the names of a eight member Constitutional Committee. The committee will be given the task of rewriting six articles of the constitution. These articles relate to presidential term limits, judicial oversight of elections, and parts related to curbing the president's powers to amend the constitution and restrict civil liberties, including sending civilians to military tribunals. The committee is asked to come up with recommended rewriting of these articles in 10 days. Reports say the constitutional changes will then be put to a popular referendum. The committee will be led by Tareq al-Bishry, who is a retired judge and a prominent historian. Mr Bishry and a Cairo University law professor, Atef al-Bana, are thought to be followers of a modern Islamic school of thought. Other members are Sobhi Saleh, a popular member of the Muslim Brotherhood who represented Alexandria in parliament from 2005 to 2010. About the selection of the committee, Abdel Yousef, a senior official of ElBaradei's National Association for Change, says the names are a good sign from the military. Brazilians turn to pawn shops lending by government owned bank Caixa, in a regulated portion of the lending industry, as credit card rates increase. Brazil experienced a huge surge in credit card debt in the years when consumer loans were freely made in the last decade. Between 2004 and 2014, consumer credit in Brazil increased 658% to $297 bilion, according to the National Association of Executives in Finance, Administration and Accounting. Central bank figures show 6.7% of personal bank loans and 26.3% of credit card accounts being in default. As in Turkey much of the country's growth was fueled by increased spending and consumer credit. The credit binge and the lower revenues from a decline in commodity prices is leading to slow growth and a stagnant economy.
