The cuts are about $880 billion over 10 years for Medicaid. The $300 billion less in subsidies over ten years is likely to hurt the elderly. India like China is more interested in modernization that brings equality with Europe and America so that the period of misfortunes that struck India and China- as a result of the vastly superior technology and force of Europe as it found a passage to the East around the Cape of Good Hope- is over.  Think about this. If anything happened to democracy and pluralism in the US Indian democracy and pluralism would still be standing a hundred years down the road or the next hundred years after that. What does that say about India? Why? Because India has learnt its lessons under Vivekananda, Tilak, Gandhiji, Modiji, and understands the need for technology, trade and modernization, which is what Modi as a Gujarati with the trading mentality like the British is really after. The so called Hinduism as it is really about the Upanishads and the Gita and the Buddha, and Communism, are really not the driving force in India or China.The Upanishads and Bhagavad Gita like the Bible offer a way an ethos to resolutely fight the corruption and leakages of funds that take the investments out of modernization leaving everyone poor. And India also benefits when democracy works and acts as an enabling force for a modern economy that creates "a rising tide that lifts all boats" (people). Democracy is the tool for development and to tackle diversity of 1.4 billion people. Adam Smith was right writing then in the 1780's around the French revolutionary period and American independence - "Hereafter perhaps the natives of these countries (India, China, Indonesia) may grow stronger, or those of Europe grow weaker, and the inhabitants of all the different quarters of the world may arrive at the equality of courage and force, which by inspiring mutual fear, can alone overawe the injustice of independent nations into respect for one another." India's leaders fought hard after the 1700's for preserving independence from the Portuguese, the Dutch and the British, only they were divided. Ranjit Singh in the north fought the Mughals and the British in the Punjab. The Marathas on the western front fought the Mughals and the British. The result as Gandhi points out in Hind Swaraj in his question "who made the British Company Bahadur?" It was Indian princely kingdoms vying for support from the armies of the British East India Company interested in profits from seizing Indian princely treasuries and trade. Note that Sri Lanka or Ceylon fell to the Portuguese in 1505. The technology gap between Europe and Asia had opened up even that early by 1500's in ship building, in warships and use of maritime navigation technologies. Consider that in 1534 Jacques Cartier was out on his first trips from St Malo, France across Atlantic to explore past Newfoundland to the mouth of the St Lawrence river. The Portuguese and then the Dutch had already beaten the British and the French by 100 years- Britain's exploration of India through East India settlements in Bengal began much later in the 1600's. India like China built around river based civilizations as Adam Smith points out in his Wealth of Nations, Chapter 7, Part 3, America and East Indies-of the natives of India and China Smith says their struck "a dreadful misfortune" that arisen more by accident, that "the superiority of force seemed to be so great on the side of the Europeans, that they were able to commit with impunity every sort of injustice in these remote countries." Every Indian or Chinese will agree with this so great was the misfortune for India and China from the injustice of European nations in the 19th century so much so that Cordell Hull speaking for Franklin Roosevelt and all Americans broadcast to the world in the throes of World War II in 1942 America's call to the world for a new world order based on freedom and development for all nations of Asia, Africa and Latin America. America's Secretary of State Cordell Hull said: "In this vast struggle, we, Americans, stand united with those who, like ourselves, are fighting for the preservation of their freedom; with those who are fighting to regain the freedom of which they have been brutally deprived; with those who are fighting for the opportunity to achieve freedom."     ...

A study done by the Hudson Institute shows an increase in sales of 5.5% on average in same store sales of restaurants which increased lower calorie items on the menu. The reverse is taking place for restaurants that have neglected to do this, with these restaurants experiencing a decline in sales. This was based on research firm NPD and restaurant data at 21 fast-food and sit-down restaurant chains between 2006 and 2011. Chains that include lower calorie counts on menus include Panera, McDonalds, Denny's and Au Bon Pain chain. Federal regulations will require restaurants with 20 or more outlets to post the calorie counts in early 2014. The process of moving Americans away from eating habits that lead to obesity is moving at a slow pace. Margo Wootan, director of nutrition policy at the Center of Science in the Public Interest, says large serving sizes at restaurants lead to overeating and obesity. Frequency of eating at restaurants is another problem, with studies showing women who eat out more than 5 times a week take in about 290 calories each day compared to women who do not eat out that often. The healthy options at restaurants are still restricted to a small portion of menus and healthy choices are limited....

Working class and older Americans show their anger at a system that appears to have left them behind with slogans, stickers, T-Shirts. The idea of the wall figures in much of this and shows that the wall has become not jut about Mexico but a metaphor that captures this anger, that reflects this anger. Another aspect of the 2016 campaign is that those most vulnerable and most in need of help have not sought the comfort of knowing about programs to improve middle class and working class wages, incomes, to build infrastructure, create jobs, stop companies from shifting jobs overseas, plans for improving accesss to health care and education, to ask for specifics and delivery. This is the supreme irony of the 2016 election campaign that not enough attention is going to what will be done for the middle and working class, and what specifics will be delivered, in what time frame- which is essential for restoring the condition of the American middle and working class to where it was in the 2 decades after the Second World War. ...

This provides the public with a closer view of the Justices on the highest Court in the Nation. It also means Justices live a sheltered life away from the concerns of ordinary Americans about cost of living, cost of health care and pharmaceuticals, cost of childcare, cost of education, cost of fentanyl and other drug trafficking to American families and mothers. During the Depression only three Justices the most prominent of whom were Harlan Stone of New Hampshire and Wendell Holmes of Massachusetts supported New Deal legislation to provide government help with other Justices rejecting New Deal legislation right in the middle of the Great Depression when people were literally without food and shelter, without jobs and in despair. Today there is a permanent exhibition of Harlan Stone in the US Supreme Court for the longest service on the Court. It took till 1940 for FDR to appoint new Justices to get the Court to support the New Deal. Today Justice Barrett is a lone voice among Conservatives willing to listen to all opinions on both sides along with but more than Justice Roberts. This is heartening to both sides. Barrett like Justice Sandra O'Connor before her puts great importance on the Constitution's intent and wording, its pages are in her book. O'Connor from a rural ranch in Arizona carried the Constitution with her at all times and had her own books- the Lazy B about growing up on a ranch on the Arizona- New Mexico border in the middle of big country, big sky, so wide an expanse that she writes about it in her preface to her next book The Majesty of the Law citing Wallace Stegner. ...

In this letter the U.S. sets out some important facts about events that happened during the coronavirus crisis during the crucial 4 month period from December 2019 to March 2020. Every week lost in this time due to reasons of a lack of transparency, openness meant hundreds of thousands of people more infected and tens of thousands of deaths worldwide. There are questions of transparency, of openness and this raises questions about the manner in which the World Health Assembly operates with hundreds of small countries in Africa and Asia having votes equal to that of the U.S., India, Brazil, Mexico with votes taken of over 200 countries. The entire election process can now be seen as questionable, when over a billion people in one country alone such as India or hundreds of millions in Brazil and Mexico would have to bear the consequences of poor decisions made by small countries that can be swayed in one direction or another based on political bias and other considerations that have nothing to do with global health.  At the conclusion of the letter by the U.S. to the current WHO shaped by a controversial election in 2017 the following is stated about the standards set by Gro Harlem Brundtland and which helped the world prevent the SARS crisis which originated in China in 2003 from spreading to the large countries of the world India, Brazil, Mexico, and other such countries in Asia and Africa, Latin America, and the U.S. European Union. "In 2003, in response to the outbreak of the Severe Acute Respiratory Syndrome (SARS) in China, Director-General Harlem Brundtland boldly declared the World Health Organization’s first emergency travel advisory in 55 years, recommending against travel to and from the disease epicenter in southern China. She also did not hesitate to criticize China for endangering global health by attempting to cover up the outbreak through its usual playbook of arresting whistleblowers and censoring media. Many lives could have been saved had you followed Dr. Brundtland’s example." Even this does not come to grips with the flawed way in which the election of WHO head is done. It can no longer be relied on when there is the danger that lack of transparency can emerge in the WHO leadership itself because of a flawed process. It risks endangering the lives and livelihoods of hundreds of millions in countries such as India, Brazil, Mexico, as well as in the relatively small countries of Africa and Latin America where even basic water supplies are at risk but which could tilt elections at the World Health Assembly. Consider that a cyclone just hit the Indian state of West Bengal and Bangladesh on May 20 just as the coronavirus pandemic is spreading. That this region of 1.5 billion people had just 2 votes out of over 200 cast at the World Health Assembly in 2017 shocking. And even these votes cast based on old geopolitical considerations not how good the candidate is, and how good the country he is coming from is in terms of its record  on public health. The irony here is that private foundations in the advanced countries in the U.S. and Europe some of whom are major donors to WHO did not think that more experienced candidates in their own countries with a better record of public health such as in France or Germany are better qualified, in a flawed NGO support mentality left from the Clinton years. Basically the people in these large countries such as India, Brazil, Mexico were disenfranchised, when the austerity policies were consuming the European Union, and the U.S. had just elected a new administration itself groping for ways to reverse years of neglect of public services and infrastructure priorities. They would trust good leaders no matter where they come from, who have a record of transparency, leadership, and all the values we cherish together no matter where we come from. ...

Pfizer and Moderna's plans to make mRNA vaccines in Africa, Asia or Latin America may take much longer than 2022. The solution to producing an mRNA vaccine in Asia that could be mass manufactured and distributed throughout Africa, Asia and Latin America is now at hand. Gennova based in India, is partnering with Seattle startup HDT Bio to attack the problem of temperature and scalability in manufacturing for a mRNA vaccine that acts as a global solution using India's manufacturing capabilities. Dr Singh who founded Gennova, says- "We wanted to solve the problem of the scalability issue, and the temperature issue. If we can solve these problems, we are building a solution not just for India, but also a global solution." Gennova received seed funding from the Indian government. Other companies in Brazil and South Africa lack the manufacturing capabilities or financing needed that exist in India. The Indian government has achieved an initial goal of one billion vaccinated in just 6 months. The next step for India in its health infrastructure buildup is a mRNA vaccine that is an improvement over Pfizer and Moderna vaccines that can be stored easily, adapted for variants, and manufactured in large quantity as a global solution. ...

It depends- only if you look at it in actual millions of people. In percent of people Voice of America and this WSJ report show that US obesity rate is much higher with 22 states above 35% and the rate overall at about 25%, compared to China's at about 14%. The fact that this was reported in this way is happening as this issue is taken far too casually in the US, when the quality of life is largely dependent on health. And such high levels of obesity in the US, catastrophic levels in some southern and midwestern states, means one is not doing things right and heading for a poorer quality of life. This report in the WSJ cites estimates of obesity in China of as high as 200 million and likely to grow by another 100 million by 2034, about 14% of the population being obese today and obesity increasing to about 20%. In China the demand for weight loss drugs is growing. The government has a program to reduce the intake of salt, sugar and oil and increase health foods in the diet. Meat in the diet has tripled and there is a need for more health conscious attitudes in China, even more so and urgently in the US. Both the US and China are too auto centric in their culture, particularly the US where public transportation has not been given high priority leading to a lack of enough exercise getting to work. ...

As much as 20-30% of the population leaves, hyperinflation, bad relations with the US, leading to the worst humanitarian disaster in the American continent since 1900.  It shows that the educated classes of every nation bear a major responsibility to  create a healthier society at the beginning so that the glimmers of hope of free services lead to the wrong people ending up in power and mismanagement on a massive scale destroying everything. New York City's educated classes should shoulder the responsibilities that people like Felix Rohatyn as head of MAC describes in his book- Bold Endeavors, that is the true spirit of the Nation (2009). ...

UN projections show median age of Chinese citizens will overtake that of Americans in 2020. Yet China's median income is only a quarter of that in the U.S. Life expectancy in China today is 76, very close to that in America. In 1960 a Chinese person born that year had life expectancy of 44 years.  China is aging at the pace of Japan, and a bit slower than South Korea, but wealth per capita was three times higher in South Korea and Japan than China when the aging accelerated. A Chinese woman fertility rate today is 1.6 compared to 4.6 in 1973. A prominent Chinese economist says in a recent report that median age in China in 2050 will be nearly 50 compared to 42 in America and 38 in India. WSJ cites figures showing China will have gone from 9 working age adults per retired person in 2000 to just two by 2050. So how to pay for retirement of all these workers today? Government spending on retirement is a tenth of GDP, about half the level in older wealthier countries, and increase in spending will impact growth. Today this is about 6.2% potential growth rate. It also pushes wages up with a shortage of workers in cities such as Shenzen and X'ian even with the use of new technology and robots in factories.  Solutions are to raise retirement age currently set at 60 years, increasing labor force participation of women as Japan has done, and increasing productivity. China has transferred 10% equity stakes in four state owned financial firms to the national pension fund to shore up its finances as estimates from the Chinese Academy of Social Sciences show it running out of money in 2035. Traditionally children supported families in old age but the one child policy leads to situations where the child is working or in another city. In Suzhou near Shanghai, a retirement business sends 1800 helpers to private homes and 130,000 retired people, in a new trend. The city administration of Shanghai plans 400 neighborhood care centres for elderly by 2022, with health clinics, drop in facilities, and homes. 12,000 elderly people use one centrre in central Shanghai area of Changning. ...

The major provisions of the Republican House healthcare bill that passed by a vote of 217-213 are- 1. To help people buy insurance coverage the bill offers $2000 to $4000 a year, upto $14,000 a year in credits based mainly on age, reducing them for families making $150,000, individuals making $75,000. 2.  Under the Affordable Care Act insurers cannot charge older Americans more than 3 times for same coverage they offer to younger people, the new bill makes this 5 times. This would increase premiums for older Americans and reduce it for younger Americans. This is the most controversial part of the bill. Older Americans supported the Republican party in the presidential election. 3. The new bill ends Medicaid as an open ended entitlement and places this on a budget with cuts of $880 billion over 10 years. 4. To mollify conservative Republicans a provision allows state to opt out some provisions of the ACA that requires minimum benefits such as maternity care and emergency services. It retains coverage for pre-existing conditions to mollify moderate Republicans. The bill provides states with $138 billion over 10 years to subsidize premiums, provide coverage for pre-existing conditions, mental healthcare and drug addiction. 5. The bill removes the taxes imposed under the Affordable Care Act (ACA) on high income people of about $300 billion over 10 years by repealing a payroll tax increase and tax on investment income. This bill and the ACA offer 2 competing visions on healthcare, both bills passed only by a margin of 4-5 votes in the House. The ACA overlooked the impact on premiums causing discontent among middle income Americans. The new bill lets premiums rise for older Americans in order to keep premiums down for other Americans. This shows the many tradeoffs involved and choices being made, and the lack of a consensus on the issue of healthcare in the U.S., becoming a highly politicized issue instead of the way it is treated in western Europe.     ...

The WSJ is still calling the president's stop fentanyl flows tariffs on CMC Canada Mexico and China economic tariffs in this editorial board opinion. It is incomprehensible that little or no mention is made in most of the media of the magnitude of injury to the US, the 490,000 deaths in America over 12 years as the result of Canada, Mexico and China not taking the needed action to stop fentanyl flows into the US. There is also the added factor of lack of a level playing field in trade which has resulted in the same communities in many cases having suffered from in the case of China loss of 25 million jobs over the last 10 years and loss of $250 billion in infrastructure and public services for schools, libraries, childcare, and health care clinics that were lost from losses in taxes for local communities in the US. This has decimated life in these communities and in small towns across America.  In the case of Mexico the illegal migrant flows that were not stopped at the border have put an added burden on already underfunded and strained public services in local communities in the US. This is the reason for much of the frustration and anger that has built up over time in these communities with the response from the DJT administration to find solutions. CMC countries could have taken action on their own, yet the US had waited too long for this action. Reciprocal in reciprocal tariffs is about fairness, a level playing field, something that China had agreed to in the spirit of the WTO entry in 1994 and American desire to aid China industrialize build a modern economy. Instead US business was coopted by China during the industrialization process 1995-2010, 2010-2020, including in the first term of the DJT administration even when tariffs were imposed. This happened with transfer of technologies happening late into the first term of the DJT administration 2016-2020, which has led to a much of the pent up frustration and action in the first 100 days of DJT in 2025.  ...

The 2011 State of the Union address by President Obama. A calculated effort to move the debates that will frame the future election to a different place. He emphasizes the importance of investing in the future, in global competitiveness, through spending on education, infrastructure, alternative energy and other projects. But there was little in the way of specifics for reducing the high jobless rate which stands at 9.4%. And little in the way of specifics of how the investments in the future for global competitiveness and infrastructure spending are to be achieved. Especially when the fiscal imbalances are growing after the compromise on the Bush tax cuts and the passage of health care legislation. The Washington Post says that a majority of Americans approve of his overall performance, yet they are generally negative in their evaluation of how the Obama administration and President Obama has handled key issues relating to the economy. And this is more so among independent voters who will be crucial in the 2012 elections....

The Republican House health care bill AHCA  keeps some some of the more popular parts of the Affordable Care Act such as not taking pre-existing conditions to deny insurance, keeping children covered on parents insurance till age 26, and increase contributions to Health Savings accounts. It is different in that the expansion of Medicaid at 138% of poverty rate threshold is rolled back. Age is used for tax credits instead of income, hitting those approaching Medicare age harder. The maximum charged to older people is now set at  5 times compared to 3 times what young have to pay. In general the Republican bill is seen as targeting the elderly to keep premiums down. The elderly on low incomes are hit hardest. Fox News O'Reilly Show showed the host questioning House Republicans, citing the CBO estimates that elderly on low incomes may have to pay as much as $14,000 a year for insurance making it basically unaffordable, and 52 million Americans would be affected adversely. Large companies are no longer required to offer mandatory health insurance under the new bill. Conservatives from Freedom Caucus wanted to see the essential areas of benefits covered by the law limited to fewer than the ten areas in the Affordable Care Act. The ACHA Republican bill leaves to states to determine what are required essential benefits. At one point maternity benefits were to be dropped but this was changed to let states decide. As a result the bill is 33 votes short of the number needed for passing the house in March 2017.   Neither the Democrats Affordable Care Act or the Republican House bill of Speaker Ryan do much to tackle the real problem- the absolute amount and increases for health care dollars for treating each disease in the U.S. compared to European and other countries. As a result health care has become more of a partisan struggle between the two parties than a real effort by all to overcome the problems that have to be tackled. Republicans want to see premiums drop and keep the burden on the deficit down- but with the level of U.S. health care costs disproportionately high compared to Europe and the rest of the world the arithmetic is tough and ends up leaving out vulnerable groups such as the elderly on low incomes, thus making the whole proposition prone to fail. For the same reason the Democrats failed to keep premiums down with a wider safety net leading to calls for repeal of their version.   ...

Obesity in the US is as high as about 40% in West Virginia, Louisiana, and Oklahoma. It is lowest about 25% in Colorado, Vermont and Hawaii. About 22 states have obesity rate over 35%. Compare this with China which is seeing obesity increase from about 15% in 2023 to 20% in 2034. Real competition between the two countries starts with areas like health care coming out of the pandemic when looking at the true interest of both peoples instead of geopolitics creating a huge distraction from problems of health, climate change and education. Meat intake has tripled in China and a return to more vegetable and fruits and ancient grains is something that is needed badly, also helping tackle climate change. The states in the South and midwestern US have higher rates of obesity followed by northeast and western states. This includes in the South Kentucky, Georgia, Texas, Alabama, Tennessee, South Carolina, Mississippi, Arkansas. In Midwest it includes Ohio, Indiana, Wisconsin, North Dakota, South Dakota, Iowa and Kansas. It is useful to note that this is in Voice of America news which is aimed at an overseas audience and this kind of information is not seen widely in US media. Robust food programs ae needed especially for people living in poverty. Health consciousness needs to be emphasized in all aspects of life and worklife, workspaces, living locations and transportation options all need to be devised around this. Bussel of the Robert Woods Foundation says even ten years back no state had over 35% of the population being obese. Clearly headed in the wrong direction with all the discussion in media run by billionaires on everything but what most affects the quality and ease of living of ordinary people. ...

US Business has considerable apprehension about the former president in 2024 compared to its willingness to consider Trump in 2016. At the time executives from investment bank Goldman Sachs and heads of oil companies joined the Trump administration. This time US business and corporate interests are apprehensive about becoming the target of a tweet they might find the next morning under a Trump administration. They are not supportive of student loan forgiveness, but when it comes to the CHIPS and Science Act they see president Biden as effective and helping industry. Business leaders have a negative view on the Trump effort through appointment of 3 Supreme Court Justices of overturning decades old rights of women on abortion, and on this issue alone many will support Harris-Walz, overriding other concerns they might have. The visions of Harris and Trump are so vastly different with one calling climate change a hoax and hyping up social issues and infrastructure needs without any record of delivery when in office, and the other a strong position on climate change, wages and income, delivering on infrastructure and CHIPS that US Business. The result is that it leaves US Business with no better option in 2024 than to support the vision  that takes America forward. There are different sections of the business community which have different priorities.  Silicon Valley, and oil, pharmaceuticals because it profits most from light regulation which brings with it social costs is a special issue not addressed here. Other business, banking, automobiles, and a range of other industries have other priorities yet also see the need for the economy and the US to move forward with a different vision than one that simply ignores climate change, and fails to address child care, child poverty, wide disparities in wealth, and other issues facing of wages, cost of living facing most Americans.  ...

T Though it is not readily apparent in an America where about 22% of 8th graders are able to get a passing grade in civics and history in NAEP tests, a rereading of the writings and speeches of Abraham Lincoln show the same concern for the rights of all segments of society that are found in Wilson and FDR or Truman. This is also what Biden means when he calls this a struggle for the soul of America. More not less funding for education, more not less funding for health after the pandemic and cost of living crisis, more not less funding for public services at a time like this where there is a fracturing of society as well as isolating certain segments of society from others such as the growing distance between workers and families from places such as Silicon Valley and capital markets in America.  ...

Latin America has made a huge turnaround through successful vaccination drives. Today more people are vaccinated as a percentage of the population in Latin America at 62% than in the US at 56% or Europe at 60%, according to Our World in Data project at Oxford University. There is little resistance to vaccines in Latin America after successful vaccine campaigns against yellow fever and other diseases. During the first year of the pandemic Latin America had one third of the deaths in the world with 8% of the population. Deaths after vaccination drives have dropped to 8%.  Brazil with 617,000 deaths from coronavirus was second only to the US with 800,000 deaths. Brazil is now back to normal after a successful vaccination drive that has 66% of the population fully vaccinated, and 80% with one dose, some of the highest rates in the world, according to Our World in Data at Oxford University. In Colombia with 50 million population about 50% of people are fully vaccinated. Cases have dropped from 30,000 in June to 2000 a day and deaths from 700 daily that month to 50 a day in December 2021. In Buenos Aires, Argentina's capital, 83% of three million population are fully vaccinated, 14% have received a booster. Buenos Aires city health minister says Argentine society has an affinity for vaccination campaigns. "They rapidly accepted receiving them," he says. Yet from the point of view of new variants emerging there is a different situation in rural areas. Tobacco use has reached levels of catastrophe in Jordan as a major public health crisis develops. This Guardian report says there is influence of Big Tobacco in developing countries. About 80% of men in Jordan use tobacco or smoke cigarettes, according to a government and WHO study, smoking an average of 23 cigarettes a day. This could soon spike rates for cancer and other lung diseases, and warnings of a future public health catastrophe comes from the Amman King Hussein Cancer Center.  Indonesia is one of the other nations with high level of tobacco use. In countries in Europe and America where tobacco use is controlled there are restrictions on advertising and efforts to control use which are less prevalent in developing countries that put there health systems more at risk from lung diseases. This is true even of countries such as India and China, where a lot remains to be done. At one time Japan also suffered from this problem. Revenues to the state from tobacco and other financial interests, as well as lack of strong public health protection agencies is a problem that needs to be tackled for public health. In China about 50% of men smoke and there are 300 million smokers, with one third of tobacco consumption in the world in China. In India the figures are 234 million and 47% of men smoke, with the highest smoking at 80% in Kolkata and over 70% in the northeastern states. The damage from this to public health is enormous. In China the China National Tobacco Corporation CNTC is the state run tobacco company, the third largest company by profits in China. In India the largest tobacco company is Imperial Tobacco Company ITC. Tobacco was brought to India and China by the Portuguese from Brazil 400 years ago and may have taken the most lives in the two countries over that period. ...
