Matthew Tsien, a vice president in GM China, will be the new president in Jan. 1, succeeded Bob Socia. Tsien will report directly to Dan Akerson, CEO of GM. Tsien is currently vice president of planning and program management and has experience working wih GM's joint ventures. The direct report helps to provide direct contact at the highest level with CEO Akerson. GM China chairman is Tim Lee, who is also executive vice president of global manufacturing. China provides about 30% of GM's global vehicle sales. GM is taking a new look at its China operations as increasing competition is eroding its market share. VW sales in China increased by 18% to 2.35 million cars and SUV's, in comparison GM sales were up 11% to 2.31 million, for the first 9 months of 2013. GM's plans going forward are to invest $11 billion in China through 2016 for 4 new assembly plants. This will boost annual production to 5 million vehicles in China by 2016.

One in six dollars generated by the U.S. economy goes to pay for health care, almost twice the average for rich countries. It hurts America in many ways; by being a burden on the taxpayer when it comes to Medicare and Medicaid paying for the poor and the elderly, on companies being one reason GM went bankrupt, it eats up federal and state budgets, rising costs make any form of future coverage for all unsustainable, and it robs other priorities such as infrastructure building and other national scale investments. The Economist says that if it had to design a system from scratch, it would go for a system based mostly around publicly funded health care. For the uninsured the solution of an employer mandate is now well accepted, so this is not an issue. What is an issue is how to make the new system affordable? Here the Economist says that whether in stages or in one move, the tax deductability of employer paid health insurance, which is costing the U.S. government $250 billion ayear, has to go. It is necessary to remove this deduction, and its something all interests involved will have to swallow, as other savings are smaller and will not be adequate. The deductability of insurance makes the true cost of insurance transparent, so it supports gold plated insurance. This does not make cost control the pressing priority it needs to be. So the deducatability of employer paid health insurance hurts both ways. The other necessary action is in the area of moving out of the current culture where most doctors work on a fee-for-service basis, where the more tests they prescribe or procedures they perform the greater their incomes. This acts as a perverse incentive, and has aruinous effect in mushrooming health care costs in America. Cutting back on unnecessary tests and procedures, and prescriptions , would save 10% to 30% of health costs says the Economist. And it says this has been proven with the Mayo Clinic in Minnesota and Kaiser Permanente in California showing that cutting back doesn't hurt care and outcomes., so much so that cutting back would occur along with improved outcomes. But Americans with employer paid insurance just take things for granted as its not much out of pocket expense for them. THis creates the lack of a force for controlling costs even as employers are shouldering abigger and bigger burden, and the employee who thinks he is doing fine actually is seeing more of his salary dollars going to pay for his health insurance. In a way the consumers of health care are stuck with the perception that they are not somehow paying for these mushrooming costs and too manytests, procedures and prescriptions. This perception leads them a false sense of comfort with the system they are in, and a fear of something new fanned by the medical lobbies, that any change will impact users negatively. This makes the whole discussion on health care or the process of finding solutions to become an exericize in which terms like "rationing" and "choice" play a distorting role. ...

This essay in the Economist warns that most of the public does not understand the dangers of the idea of no Brexit as a preferred option to a Brexit deal that gives too much to the EU. It says this is dangerous in terms of the harsh effects at the border with Ireland and on the economies of Ireland and Britain. It points out that the private view of the EU is very negative towards Brexit compared to the diplomatic comments, so that little should be taken for granted. The European Union and Britain would in the event of no deal on Brexit not follow agreed  terms such on as the 40 billion pounds exit bill, guarantee of EU citizens rights, averting of a hard border in Ireland. The unfriendly nature of such a no deal would lead to aggravating its effects, argues the Economist.  The Economist estimate is that about 4% of GDP would be lost over 5 years for Britain and Ireland. Supply chains would be disrupted. Depending on WTO rules alone is not sufficient as the EU has bilateral deals with many countries. The car industry is particularly vulnerable as it employs 800,000 people and exports 80% of output- it would lose EU certification and face 10% tariffs. And 32.5 million Americans took advantage of the annual physical without co-pays in 2011. About 50,000 Americans insured temporarily for people with a pre-existing condition also may lose their coverage. Another provision which prevents states from tightening eligibility rules for Medicaid before 2014, may be questioned also as states feel burdened by these rules during a period of budget cutting in areas such as education....

Renault SA sales increased by 3.2% to 2.7 million cars in 2014. Renault does not sell vehicles in the U.S., and it has only a small operation in China. Sales in emerging markets outside of Europe declined from 50% of sales to 46%. Sales in Brazil were up 0.3% and sales in Argentina declined by 40%. Sales in Russia declined. The sales outlook in emerging markets Brazil and Russia is poor for 2015. Renault has been a laggard in China, and plans to make large investments to catch up with competitors. Doctor are suffering from a "demoralization syndrome" says Eric Reinhart, physician at Northwestern University. He sites one study that shows universal access to health care in America could have prevented at least 338,000 deaths during the pandemic. Overwork comes from physicians retiring from the healthcare workforce in large numbers. In 2021 117,000 physicians retired and only 40,000 physicians joined the workforce. Another 20% will retire in 5 years. Burnout itself comes from a realization that the system is failing its people and you are working within it without the social and cultural narratives that supported previous generations of physicians. It is these narratives of honesty and service that are the essential parts of the human condition that are lacking without access to good health care for all the people and the education to promote good health.

Arizona, Utah and South Carolina are 3 states that are cited by the US Labor Department for not adopting any portion of the Occupational and Health Safety Emergency Standard for health care workers. The Labor Department says that by not adopting these standards for social distancing, mask use and paid time off for vaccination, these states are risking the health and safety of health care workers. OSHA sent letters to these states revoking the states abilities to run their own occupational health safety programs.

This helps clear emergency rooms of people seeking bsic medical care as for astrep throat or flu. About one thousand clincs are operating in the US at drugstores, supermarkets and big stores since the idea took root 4 years ago. Wal-Mart has partnered with hopitals like the Christus Medical Group in Texas, Aurora Health System in Wisconsin and COxHealth in Missouri to setup clinics. Mayo did so after employees and patients said they wanted more convenient treatment for minor medical problems, so there may be a need here that as not been met. The lower costs at these centers compared to primary care doctors offices or emergency rooms make it possible for them to price lower and meet the needs of the 45 million or so uninsured people in the US, numbers growing as jobless rate increases. They are typically staffed by nurse practitioers or physicians asistants. Dr Herman at Mayo Clinic, who supervises its retail store clinics, says rather than fight this trend primary care doctors should learn from it, and work with hospitals around the country to offer more convenient locations and consumer friendly office hours, including periods of walk-in care with no appointment....

Now the half million vending machines that dispense cigarettes to some 26 million Japanese smokers can only dispense to adults because users have to have a special card called Taspo or tobacco passport that they insert into the vending machine. This will shift sales to convenience stores like the Lawson's chain. In Japan 29.2% of all adults and probably an even higher percentage of men smoke according to OECD data. In the US its about 17%. Like obesity and poor eating habits and the availability of fast food in the USA, smoking in Japan remains largely untouched by any efforts to educate young people and the public about the great dangers, with companies largely uninterested and the government and schools and universities largely apathetic about what are really huge health concerns that reverberate in so many ways across the fabric of society. For example a obese person consumes more health care dollars, a smoker also consumes more health care dollars. And health care dollars are scarce dollars and need to be spread out in better ways than wasted on preventable things like obesity and smoking through partnership in education and other measures between all groups and organizations in society and the government. ...

Biden calls ending the war in Afghnistan a "wise decision" for the American people. He says in his foreign policy speech that "it is about ending an era of major military operations to remake other countries." A Pew Research poll shows 54% of American adults support the decision.  In a sense the decision had already been made. Biden cited the Doha agreement president Trump signed a year ago with Taliban that called for the release of 5000 Taliban prisoners which included most of the top commanders, and no agreement on the future of Afghanistan. The decision had come much earlier than that when the wars in Iraq and Afghanistan from the period of George Bush were rejected by the American people for the cost and lack of purpose during the presidential election of 2016. That period marked the rejection of policies set under Reagan, Bush and Obama for starting American involvement in the Iraq-Iran conflict first on one side and then on the other side. All the time precious resources that were needed for infrastructure and services in education and healthcare were diverted to these wars, impoverishing America and also Europe. Looking beyond the words thrown around for political advantage both Trump and Biden and the American people, had decided to put these wars behind them 5-10 years earlier. Biden said assertively that America had made a tragic wrong turn, that was all he could say about Reagan, Bush, Obama policy. In the meantime he stated something else was happening- the US was losing its position in the world by wasting its resources in these wars that do not serve the interests of America. "There is nothing China and Russia would want more in this competition than the US to be bogged down for another ten years in these wars."  Biden was saying that he had the courage and tenacity to make a decision that was the right one and a wise one for America against all the transient opinion of people who lacked a grasp of what was happening to the American people- the increasing impoverishing of America in both rural and urban areas. And a similar situation in Europe. It was time to take a new turn, close this chapter, and write a new one in American history, brighter and with new sense of hope. ...
