Republicans wanted to see money spent in the states on the idea that states best knew where needs were. Democrat Clinton called it "ending welfare as we know it," and signed the bill for TANF where $16 billion was sent to states in anti poverty programs and $15 billion added to this by the states. This is how TANF (Temporary Assistance for Needy Families) started under Clinton in 1996. It was put under Department of Health and Human Services. A lot of latitude was given to states on how to spend the money. In 2026 much of this money is not spent wisely. GAO government agency audits show 37 states had 56 severe deficiencies with "opaque accounting practices." This includes Florida, Louisiana, and Connecticut. The Government Accountability Foundation calls the lack of accountability "fraud by design," saying that one fifth of the $31 billion never reaches the people it was intended to benefit. Both Republican and Democratic states are not using the funds the way they were meant to be used. Needy families getting help have come down from 1.9 million in 2010 to 850,000 in 2025.  ...

Obesity in the US is as high as about 40% in West Virginia, Louisiana, and Oklahoma. It is lowest about 25% in Colorado, Vermont and Hawaii. About 22 states have obesity rate over 35%. Compare this with China which is seeing obesity increase from about 15% in 2023 to 20% in 2034. Real competition between the two countries starts with areas like health care coming out of the pandemic when looking at the true interest of both peoples instead of geopolitics creating a huge distraction from problems of health, climate change and education. Meat intake has tripled in China and a return to more vegetable and fruits and ancient grains is something that is needed badly, also helping tackle climate change. The states in the South and midwestern US have higher rates of obesity followed by northeast and western states. This includes in the South Kentucky, Georgia, Texas, Alabama, Tennessee, South Carolina, Mississippi, Arkansas. In Midwest it includes Ohio, Indiana, Wisconsin, North Dakota, South Dakota, Iowa and Kansas. It is useful to note that this is in Voice of America news which is aimed at an overseas audience and this kind of information is not seen widely in US media. Robust food programs ae needed especially for people living in poverty. Health consciousness needs to be emphasized in all aspects of life and worklife, workspaces, living locations and transportation options all need to be devised around this. Bussel of the Robert Woods Foundation says even ten years back no state had over 35% of the population being obese. Clearly headed in the wrong direction with all the discussion in media run by billionaires on everything but what most affects the quality and ease of living of ordinary people. ...

WSJ looks at the changes in the way medicine should be practiced in the light of what we have learned from the pandemic.  Medicine practiced before the pandemic and still today relies mainly on a visit to the doctor or specialist who is short of time. There is a shortage of doctors. Patients have many illnesses as a result of decades of neglect of proper nutrition, and exercize habits. Obesity is at about 40% in the U.S. about 30% in the UK and 17% in France, and high also in other parts of the world. These high rates were unknown throughout history and result in many illnesses and increase by four times the vulnerability to the coronavirus. One authority in medicine calls obesity pouring gasoline on a fire for effects of the virus.  A doctor's appointment with doctors short of time with no coordination around a whole range of factors related to obesity, illnesses, health checkups, mental health, is now seen as a heavily handicapped way to practice medicine or for patient healthcare and wellbeing. The alternative is discussed here as the way forward. A  team will be responsible for a patient's care not just an individual doctor. The team would care for general health after a patient's checkup, cover individual illnesses, weight issues, mental health, exercize nutritional needs and other good healthcare habits. Instead of relying on doctors at a time of shortages of doctors the team would be led by nurse practitioners.  A nurse practitioner is someone with a bachelors degree and a masters degree or doctoral degree in nursing with 1000 hours of clinical training. Studies have shown that they are effective and even more effective than individual doctors. Today particularly with the problem of doctors with limited time compounded by the built up problems of decades of bad habits in nutrition and exercize and poor "cultural" habits getting entrenched, there has never been a greater need for a better way to practice real healthcare for a person's wellbeing. Particularly in rural areas with an even larger shortage of doctors the health practitioner led team will play a big role. Patients will under this setting receive more care virtually and get more followup care by phone and video messaging. The numbers tell the story- there are shortages of doctors in USA, Europe, Africa, Latin America and Asia. In the U.S. shortage of doctors is 55,000 projected to 2033 by Association of American Medical Colleges. There are 290,000 nurse practitioners licensed in the U.S. and 131,0000 physician assistants. The goal will be to get an adequate number of nurse practitioners licensed in this decade to take care of these teams. The pandemic has made virtual visits to doctors and nurse practitioners popular. Medicine reimbursement should and would be practiced on the basis of how well a patient is doing not on a fee for each micro service that is delivered. For this to happen the teams led by the nurse practitioner have to commit to patient education of the benefits from good practices and good habits for nutrition, exercize, caring for oneself. A doctor short of time is hardly the person to carry on this patient education which is where the major opportunities for a new system arise. The virtual care also provides a new medium for patient education and awareness of the risks of getting illnesses, preventive actions to be taken in advance. One approach being tested in California and Texas is for a monthly fee for patients more payments by health plans to doctors or healthcare teams if the patient is healthier. Additional health professionals are added to the team including health coaches, dietitians and medical assistants to increase its effectiveness in counseling and education and monitoring.  The nurse practitioner team approach is already being practiced in parts of the U.S. including the example of New Hampshire shown here, and is predicted to be the approach for primary care in the next decade. ...

He points out that a serious problem is that American business which is burdened with high health care costs for employees is seriously missing in this debate after years of complaining about high costs. The National Federation of Independent Businesses is actually one of the plaintiffs questioning the constitutionality of the Obama health care law. Pearlstein says business wanted an end to the fee-for-service medicine that increases consumption of medical services and pushes up cost relentlessly, and that Obama's health care law does this. This is not the case as both Democrats and Republican administrations have failed to resolve this side of the cost issue, and this is the hidden reason for the loss of credibility for both sides in this debate, leaving health care problems to be resolved in future administrations. ...

This BBC report shows how contact tracing works in Singapore to control the spread of coronavirus. On 4 February 20 Chinese tourists visited a traditional Chinese medicine shop in Singapore. Only 18 coronavirus cases had been recorded in Singapore on that date. That visit to the medicine shop created a new cluster for the disease to spread. Singapore's contact tracing units traced such locations through its well developed and extensive contact tracing programme, which follows the chain of the virus, identifying and isolating people before they can spread the virus further. For about 40% of the 243 cases so far on March 19 the first indication they had was a call from the Singapore Health Ministry.      About 6000 people have been contact traced so far, using a combination of CCTV footage, labor intensive detective work with phone calling. This includes an incident of a taxi ride of 6 minutes in which the person a yoga teacher was identified and contacted by the Health Ministry. Enforcement is done as the person was contacted at her home by three people who showed her a quarantine order, which said you could not go out or its fine time and jail time. With about 8000 people per square kilometre, Singapore is small and densely crowded. It could easily be overwhelmed with hospitals not able to cope and the health system collapsing if this was not done. Wuhan could have happened here, says infectious diseases specialist Dr. Leong at Mont Elizabeth Novena hospital, who advises the government.  Because the crime rate is low the police were given this task of tracing as a priority in addition to hospital units working on tracing. It also included the armed forces personnel helping with tracing so that it could be done quickly without delay. This provides a lesson on how countries that have faced the health crisis have used innovative methods to tackle it with good results. ...

The National Health Service in Britain is struggling under the effects of budget cuts, covid delays, and an aging population, says the WSJ. With the cost of living crisis and the Tory resistance to wage increases when nurses are found turning to food banks there is now a strike by healthcare workers. A former head of the NHS says the UK healthcare system is facing a crisis like nothing he has seen in his career. The UK has mistaken cheapness for efficiency in its approach to health, and it is now coming to roost. It is coming apart- people with heart attacks have to wait average one and half hours for an ambulance. Hospital beds are scarce tuning patients away. One in ten are on waiting lists for non emergency surgeries.

The fight against cancer cannot be done only on the finding a cure,  it is also about returning to the time when ingredients such as titanium dioxide and others were not added to food and when people did not consume processed foods. This report says the FDA spokesperson says contact the manufacturer. After the EU has banned titanium dioxide for causing the inflammation that causes cancer, the FDA should have banned it. See the recent article in The Washington Post on the microplastics that release nanoparticles that cause cancer. The culture in the US has developed a tolerance for behaviours and actions that harm the health and mental health of the American people making it necessary to change this so that microplastics and ingredients such as titanium dioxide are removed from the widespread use that is happening.

The contrast between modernizing, developing East and South Asia ( from Mumbai to Shanghai) with war torn desolate West Asia (from Tehran and Baghdad to Kabul and Islamabad) is so striking today that it is something to reflect upon for wisdom and understanding. UAE support for Sudan's RSF Rapid Strike Force and Saudi support for the military - fracturing of Sudan, errors piled on errors led to the civil war in Sudan. A civil war in a country neighboring Saudi Arabia just across the Red Sea. Saudis and UAE were on opposite sides briefly after UAE pulled out of Sudan, UAE acting in this way to object against Saudis requesting US sanctions on UAE.  Once close partners have moved apart as they spread their influence in different conflicts in the Middle East.  This has not created a region that can grow economically without the disruptions of conflict in the way other parts of Asia have emerged to modernize the countries as in Taiwan, Korea, China and India. In neighboring Pakistan another conflict has emerged as partners split, with looming conflict between Afghanistan and Pakistan. Yemeni Houthis are in conflict with the US and affect the Persian Gulf shipping lanes.  Iran with it's pursuit of weapons programs and nuclear weapons is using capital that is badly needed to improve the economic situation on arms buildup for the regime and for allies in Lebanon and Yemen, leading to protests and crisis. In this way the Middle East has failed to use oil wealth to modernize the entire region. Much of it was wasted in Iraq and now in Iran by policies that led to war and regional conflicts not modernization and technological transformation that has happened in Asia. The US has inadvertently becoming a partner to this as when the Obama administration helped fund Iran's economic rebuilding which was instead used to fund the military, and before that the Reagan administration support for Iraqi socialist ideology regime. The challenge for China was how to modernize after the Japanese invasion and civil war. In Korea it was how to modernize after the civil war. In India it is how to modernize with a smaller neighboring country Pakistan promoting terrorism and wars now with China's support. In Asia all these challenges were and are being met to steadily and persistently modernize to European standards with a singleminded focus and determination to meet the aspirations of the people with the US business working alongside Taiwanese, Korean, Chinese, and Indian governments and private industry. In West Asia various ideological (Iraq), military (Pakistan), religious Shiite (Iran), religious + modernizing (Saudi +UAE) with erratic leaders and little representation of the people, has destroyed the tranquillity of the region and destroyed democratic forms of government, destroyed bottom up education and health of the population except for priviliged groups in countries in the region of West Asia. Involvement of US and Europe or Russia in West Asia has led to distintegration of Soviet Union (Boris Yeltsin) and deindustrialization of US and Europe (Reagan, Bush, Clinton, Bush, Obama administrations) with business shipping out manufacturing to China while wars engaged the attention of American and European elites in Iraq, Iran, Pakistan, Afghanistan. The entire west Asian scene for 1950-2030 has been a disaster, one massive disaster for all involved. The contrast with East Asia and South Asia reminds one of the words from Robert Frost of New England in Mowing- that reflects on the enduring value of honest labour. "My long scythe whispered to the ground. What was it it whispered? It was no dream of the gift of idle hours, or easy gold at the hand of fay or elf: anything less would have seemed too weak to the earnest love that laid the swale in rows. Just taking a walk with bare feet in the grass, seeing the trees and the flowers and the birds in Spring outdoors gets you to feel better and great, nothing that artificial drugs can do. The next thing your blood tests will surely come out better, your next health exam will be better. NEFTY is the process and it is all about process, when the process is good the results for overall health are good one can be sure of that. And this converts into higher productivity and a better feeling at work creating a virtuous cycle. As a people one has neglected it for too long. A study done by the Hudson Institute shows an increase in sales of 5.5% on average in same store sales of restaurants which increased lower calorie items on the menu. The reverse is taking place for restaurants that have neglected to do this, with these restaurants experiencing a decline in sales. This was based on research firm NPD and restaurant data at 21 fast-food and sit-down restaurant chains between 2006 and 2011. Chains that include lower calorie counts on menus include Panera, McDonalds, Denny's and Au Bon Pain chain. Federal regulations will require restaurants with 20 or more outlets to post the calorie counts in early 2014. The process of moving Americans away from eating habits that lead to obesity is moving at a slow pace. Margo Wootan, director of nutrition policy at the Center of Science in the Public Interest, says large serving sizes at restaurants lead to overeating and obesity. Frequency of eating at restaurants is another problem, with studies showing women who eat out more than 5 times a week take in about 290 calories each day compared to women who do not eat out that often. The healthy options at restaurants are still restricted to a small portion of menus and healthy choices are limited....

Americans Save Early and set aside for savings 10% of your pre-tax income, is the advice to ensure a safe and healthy savings retirement. This is absolutely critical. What the government can do is to ensure that incomes keep uo with inflation with fair wages in industry. It also can and should protect Americans from unexpected medical costs by ensuring that all Americans are covered by health care and for catastrophic situations. Then it is the task of Americans to build a culture of careful saving that their ancestors had and considered a essential part of virtue. For this to help build savings for retirement the government and the Federal Reserve together- as Biden and Powell have shown one with capital investments to build a strong economy and the other by protecting savings and cost of living action- must ensure that no financial crises take interest rates to zero or 1-2%. At interest rates of 5-6% for returns this helps build savings for retirement. For this to happen banks have to go back to their traditional work in the economy and no speculation risk, and Silicon Valley go back to inventing and not a culture of capturing capital allocation in capital markets and paying little in taxes. A new culture would put government in its right place to ensure that it plays a significant role in building manufacturing and science and technology in the US as president Biden has done through government investing in infrastructure and renewable energy, chips and science, and in education, healthcare.  ...

Britain's Home secretary, Yvette Cooper, is setting up a Young Future's unit to help teenagers exposed to social media, mental health issues, and other pressures who could get into trouble with the law. This was seen during the UK riots with persons ages 12-15 in court for throwing stones or rioting. Cooper says- “It’s always been tricky to go through the teenage years, but it feels like for generation Alpha it’s got much, much harder,”  “You’ve got the pressures from social media, county lines and child criminal exploitation, the rise in the antisocial behaviour that we’ve seen, and … pressures on child and adolescent mental health.
