Mike Clancy of The Prospect writes in The Times of London that he has found that two thirds of remote workers working from home would like to have the right to unplug from work and not be expected to connect with the office at some point in the day, and that this would have a hugely beneficial impact on mental health.   The biggest problem that workers working from home have found during this pandemic for one year now is that it blurs the line between work and life outside of work having a negative impact on one's mental health. Surveys have found that about half of remote workers like the idea of working from home, having time from commute and being able to be have more freedom from being tied down to the office space. Companies are looking at cutting about 25 to 50% of office space but have not looked at the problems workers have and not set new rules about when the line for work is set and workers can cut off from work and not be expected to be reachable by the office. Clancy even goes on to say that companies have looked at the problems of the last century not this one and largely stayed away from tackling the real problems of workplace- stress, burnout and surveillance. Lyrarc recommends readers look at the way Germans approach this idea of blurred distinctions between work and life outside of work in the idea of "Feierabend" that literally means breaking away from work at some point say 4.00 pm or 5.00 pm to take a bike ride or walk or do something else. ...

This article in the NYT provides a look at the features of the Republican House Health Care Plan- Both the Affordable Health Care Act and the House Plan provide incentives for buying insurance- the ACA bases these incentives on income levels whereas the House Plan does not provide additional help for low incomes or elderly. Incomes at $20,000 would see a loss greater than  $2000 under the House Plan and as many of the elderly poor living in high cost areas may not have the resources to make up for this loss of subisidies they may forgo buying insurance or have insurance coverage that protects only in a limited way. President Trump has given assurances that all will be covered. For people with incomes of $50,000 or $75000 the loss of $2000 subisidies would also have some impact. At larger incomes or the well to do the subsidies are not handed out under either plan. Under the ACA the emphasis was on income levels and high cost insurance areas the subsidies were greater, under the House Plan the subisidies would be higher for the elderly compared to the young but very low income levels are not given additional help.     ...

The head of the U.S. Department of Health and Human Services, Mr. Alex Azar, repeatedly assured the president as early as January 29, that there never had been a better interagency response to coronavirus, says the WSJ. This WSJ report says based on interviews with administration officials and others that Mr. Azar waited for weeks to brief the president on the threat from coronavirus, did not coordinate effectively across the health divisions,  failed to ramp up testing development, and oversold what had been done. Testing is one of the key issues raised at this time about the coronavirus effort. Mr. Trump tweeted on April 12 that Mr. Azar "told me nothing until later." The president was dissatisfied with Mr. Azar's handling of the flavored e-cigarettes ban. This report says Mr. Azar was slow in working with other agencies such as FEMA, and in involving directly the FDA, CDC, and other agencies, to create a strong and effective response to a fast developing public health crisis of enormous proportions. In February White House advisors were critical of Mr. Azar for delays in the testing program. As the delays progressed this report says Mr. Pence and the White House gave more responsibility to FEMA, and Dr. Hahn and CMS's Ms. Verma were made active members of the White House Task Force. A sanitary health expert Arne Panesar of the German Corporation for International Cooperation says for about six of ten persons in the world there are no sustainable sanitary systems for the waste. It is not enough he says to build toilets, equally important  is how the waste is treated and disposed. Prime minister Modi of India has stated that toilets are more important than temples in India and the goal is set for all Indians to have secure sustainable toilets in 2019.  Panesar says of Dacca, Bangladesh, that for its 16 million only 2% have securely managed sustainable sanitary provisions. Waste that is not treated and correctly disposed off ends up in streams and neighborhoods creating health hazards- diseases like cholera, typhus and polio thriving in such conditions. It is not clear says this report that sanitation needs can be met by even 2030. In India the focus is now not just toilet building but on on safe sanitary provisions to make health a priority, as well as women's education which suffers without adequate toilets. ...

Employees can control their own hours in a flexible way, working longer some days and shorter other days. This kind of healthy work culture actually increases productivity of employees. The Medicare for All movement in the U.S. gathers momentum after the U.S. midterm elections with states. Governor Newsom of California says he will seek federal authority to setup a state single payer plan for health insurance. An expert at Georgetown University says there is a new burst of energy for broader coverage or universal health coverage. States with new governors or seeing this momentum are California, Washington, New Mexico, Maine, Nevada, Delaware, Oregon, Connecticut, Wisconsin, Michigan, Illinois. Polls show a large majority of Americans now support Medicare for All as a form of universal health coverage for all Americans, a system that prevalent in Canada for the last 50 years. Two hearings will be held in Congress on Medicare for All in 2019.

What is the right retirement age for health is an important question. Dana Smith points out that the number 65 that started with the system of social security started in US  by Bismarck in Germany in 1889 and Social Security in the US in 1935 by president Franklin Roosevelt has no basis on the grounds of health of the population and longevity. Since that time people live much longer to about 74 years and for 45% of the people in the US who are in the knowledge based work the ability to work continues past 65 or 67 years.  For the remaining people who are in professions involving physical work such as construction or in the restaurant industry the situation is quite different, requiring a category based retirement age that takes this into account. For these people health outcomes would deteriorate if they continued to work in stressful work for longer. Another factor to be considered is to ask what this means as a national goal. Would a nation aspire to give its citizens an opportunity to travel, broaden their minds and engage in other activities they would like to do which they could not do while working full time. In this situation these years after retirement could give people a chance to live happier lives. It is not to be taken lightly as the current protests in France show. Age discrimination in France also plays a part as there may be fewer years of work opportunity if employers stay away from people over 50 years or discriminate against women. With childcare and care for elderly, part time jobs, women work longer for smaller pensions than men, leading to a sense of unfairness. French protests show that the outcomes need to be weighed carefully from a health and national goal standpoint and the retirement age set accordingly with flexibility for harder work.  Following the pandemic years and the cost of living crisis the protests in France show the need to develop a national consensus on the issue of retirement age, and rules plus culture change in industry that ban age discrimination for workers. Special provisions for women and people in construction so that the system is seen as fair to all parts of the workforce. ...

Liberia, Guinea and Sierra Leone estimate the need is for about $1.47 billion in the next 30 months to build and equip a new health system. The World Bank and other donors have come up with $1 billion for Ebola recovery. A particularly urgent problem is to see that adequate funding goes to build the infrastructure for the health system and to train people to provide health services....

This WSJ report looks at the efforts of sugarly cola companies such as Pepsico under a new CEO to push their cola products aggressively with advertising, and modern logistics. It cites Barry Popkin, nutrition professor at the University of North Carolin School of Public Health that they are making products that are killing us more slowly. With less sugar than before but still at a time of dangerously high obesity levels in the world just as dangerous or more dangerous to humans, because they are not as healthy as previous generations. The pandemic proved the danger of higher obesity levels. The numbers say it all-1% of children 5-19 years obese in 1975 going up by 8% to 9% in 2020, and doubling to 19% in 2035, says the WSJ. That is doubling by 2035 to 19%-  simply astounding. Popkin says the fact that Americans are living more years with disabilities, and fewer disability free years, is very much linked to the food intake. On The Guardian's pages was an article about a surgeon who has a startup in Austin, Dr. Attia of Early Medical, that promotes "healthspan." It focuses on getting healthy living habits  through better nutrition, exercize, to start at an early age as being critical for a healthy life span. It is not the same starting at an early age with good food and exercize habits vs starting later in life as this means fewer disability free years when starting later in life.  ...

Chris Molloy, the founding director of the Lighthouse Labs network says the lessons learned in testing for covid, the technology and training developed can now be used to tackle other diseases. It is this network that processed most of the 207 million free PCR tests in the UK. It was created after the public health network had suffered a series of cuts from successive governments and shows the resilience and strength of the British people and scientific community. He sees the opportunity to create another legacy for the Lighthouse testing system. He says having this kind of advanced lab capacity can help tackle public health in the UK where 1 out of 3 adults have some kind of long term condition of ill health. He said this is not Beveridge 1.0 where the establishment delivers for the people. This is Beveridge 2.0 where the people engage in monitoring their own health using smart diagnostics at an early stage in their 30's, 40's and 50's when something can be done to steer away from disease instead of when it is too late and one can only treat it. William Beveridge published the report in 1942 that was the basis for the founding of the welfare state and the NHS. Molloy hopes that governments from now on will have the vision to do this. ...

"Perhaps most importantly though, Bobby created a nationwide movement made up of millions and millions of mothers and fathers and young people and concerned citizens of every background who want to end this horrible chronic disease crisis that exists, exists in America.
