One of the lessons of the pandemic is to take society back to basic priorities such as wellness, wellbeing, lifestyles that promote wellness, eating right and exercizing. During the last three decades obesity has emerged as a problem in the U.S. and Europe and in all parts of the world. It is simply harder to fight the virus carrying on so much extra weight. Much needs to be done in school with education on food in children's books and in classrooms.

Eating more fruits and vegetables, ancient and healthy grains is a must do for all. To do this the supply chain of locally produced healthy foods has to function well , and more acreage has to be devoted to get the supplies we need.

This WSJ report looks at the efforts of sugarly cola companies such as Pepsico under a new CEO to push their cola products aggressively with advertising, and modern logistics. It cites Barry Popkin, nutrition professor at the University of North Carolin School of Public Health that they are making products that are killing us more slowly. With less sugar than before but still at a time of dangerously high obesity levels in the world just as dangerous or more dangerous to humans, because they are not as healthy as previous generations. The pandemic proved the danger of higher obesity levels. The numbers say it all-1% of children 5-19 years obese in 1975 going up by 8% to 9% in 2020, and doubling to 19% in 2035, says the WSJ. That is doubling by 2035 to 19%-  simply astounding. Popkin says the fact that Americans are living more years with disabilities, and fewer disability free years, is very much linked to the food intake. On The Guardian's pages was an article about a surgeon who has a startup in Austin, Dr. Attia of Early Medical, that promotes "healthspan." It focuses on getting healthy living habits  through better nutrition, exercize, to start at an early age as being critical for a healthy life span. It is not the same starting at an early age with good food and exercize habits vs starting later in life as this means fewer disability free years when starting later in life.  ...

WSJ looks at the changes in the way medicine should be practiced in the light of what we have learned from the pandemic.  Medicine practiced before the pandemic and still today relies mainly on a visit to the doctor or specialist who is short of time. There is a shortage of doctors. Patients have many illnesses as a result of decades of neglect of proper nutrition, and exercize habits. Obesity is at about 40% in the U.S. about 30% in the UK and 17% in France, and high also in other parts of the world. These high rates were unknown throughout history and result in many illnesses and increase by four times the vulnerability to the coronavirus. One authority in medicine calls obesity pouring gasoline on a fire for effects of the virus.  A doctor's appointment with doctors short of time with no coordination around a whole range of factors related to obesity, illnesses, health checkups, mental health, is now seen as a heavily handicapped way to practice medicine or for patient healthcare and wellbeing. The alternative is discussed here as the way forward. A  team will be responsible for a patient's care not just an individual doctor. The team would care for general health after a patient's checkup, cover individual illnesses, weight issues, mental health, exercize nutritional needs and other good healthcare habits. Instead of relying on doctors at a time of shortages of doctors the team would be led by nurse practitioners.  A nurse practitioner is someone with a bachelors degree and a masters degree or doctoral degree in nursing with 1000 hours of clinical training. Studies have shown that they are effective and even more effective than individual doctors. Today particularly with the problem of doctors with limited time compounded by the built up problems of decades of bad habits in nutrition and exercize and poor "cultural" habits getting entrenched, there has never been a greater need for a better way to practice real healthcare for a person's wellbeing. Particularly in rural areas with an even larger shortage of doctors the health practitioner led team will play a big role. Patients will under this setting receive more care virtually and get more followup care by phone and video messaging. The numbers tell the story- there are shortages of doctors in USA, Europe, Africa, Latin America and Asia. In the U.S. shortage of doctors is 55,000 projected to 2033 by Association of American Medical Colleges. There are 290,000 nurse practitioners licensed in the U.S. and 131,0000 physician assistants. The goal will be to get an adequate number of nurse practitioners licensed in this decade to take care of these teams. The pandemic has made virtual visits to doctors and nurse practitioners popular. Medicine reimbursement should and would be practiced on the basis of how well a patient is doing not on a fee for each micro service that is delivered. For this to happen the teams led by the nurse practitioner have to commit to patient education of the benefits from good practices and good habits for nutrition, exercize, caring for oneself. A doctor short of time is hardly the person to carry on this patient education which is where the major opportunities for a new system arise. The virtual care also provides a new medium for patient education and awareness of the risks of getting illnesses, preventive actions to be taken in advance. One approach being tested in California and Texas is for a monthly fee for patients more payments by health plans to doctors or healthcare teams if the patient is healthier. Additional health professionals are added to the team including health coaches, dietitians and medical assistants to increase its effectiveness in counseling and education and monitoring.  The nurse practitioner team approach is already being practiced in parts of the U.S. including the example of New Hampshire shown here, and is predicted to be the approach for primary care in the next decade. ...

Beer made from locally grown cassava and other ingredients and government tax incentives is making it possible for SABMiller and other large beer companies to make low priced beers for the African market. African governments such as Kenya have eliminated excise taxes on low priced beer, and the production of locally made Senator beer by Diageo's East Africa Breweries is increasing rapidly in East Africa. SABMiller makes low priced beer brands in Ghana, Uganda and other countries. The government sees this as a way to reduce the consumption of locally brewed beers of poor quality, and a source of revenues. Health workers expect an increase in health problems as a result of increasing consumption of low priced beer. Obesity is amajor problem in S. Africa and in other African countries. This will have effects in the higher rate of diabetes and other diseases related to obesity. Alcohol consumption in Africa per person is about 10 liters of alcoholic beverages a year, with consumption at 70 liters in S. Africa, the country with the highest rate of obesity. The figures globally are 35 liters, and 91 liters in the U.S., another country with high rates of obesity and diabetes, according to 2011 figures from Euromonitor. ...

Only 12% of Americans take the minimum daily recommended fruit for healthy living, and only 9% of Americans take the minimum daily recommended vegetables, according to the Centers of Disease Control and Prevention (CDC). The minimum for fruit is daily taking one and half cups fruit. For vegetables it is daily taking two to three cups of vegetables. Women consume a bit more at 15% for fruit. An interesting finding in this study that explains the widespread obesity in the U.S. regardless of incomes is that of affluent and wealthy Americans only about 12% consume enough vegetables. This is very close to the percentage of poor people eating the recommended 2-3 cups of vegetables a day, which is at 7%. This is an alarming fact in that all sections of society are doing very badly, creating acatastrophic effect for healthcare. A diet without fruits and vegetable brings higher rates of obesity, cancer, heart disease, diabetes. If rich and poor upper middle class and lower middle class are all sharing the same lack of awareness it points to the lack of education in eating right as the big culprit. This is one area where government, universities, and the informed private sector, can change things if they wanted to. A challenge as big as that in literacy and education for the U.S. Alarmingly even though it is in the top ten read articles in the Guardian newspaper online edition on November 16, 2017, we checked the other sites. We could not find it under Health in CNN, where other topics such as sexual harrassment, and sugar cravings, were covered. NBC covered a different CDC report showing 71% of Americans are overweight or obese with BMI over 25, but made no mention of this report by CDC. Equally alarming is the statistic cited in the Guardian from the Union of Concerned Scientists that shows only 2% of American farmland is used to cultivate fruits and vegetables. That this would have to go up at least to 4% if all Americans are to get their daily required fruits and vegetables. Meanwhile little change is to be seen, and no alarm bells are ringing in the U.S.. These facts are hardly mentioned in any healthcare discussion in media, as if they can be ignored or shoved under the carpet. This is the kind of thing that will never go viral, as a discussion on sexual harrassment or some other topic would, yet deserves just as much attention and education. ...

U.S. healthcare spending of about $3 trillion comes with poor results. The cost per person is about three times that of countries such as France, Britain, yet the results show U.S. doing extremely poorly. The author points out that less spending on healthcare and more on social services in areas such as education, nutrition, housing and other services would improve the outcomes and results. It is this missing factor that is helping other countries in Europe, Japan and China achieve better outcomes and returns per dollar spent.  Social spending can affect such things as nutrition, exercize levels, lifestyles, housing and result in dramatic improvements in obesity levels for instance, or inflammation levels in the population that play a role in many diseases. The idea is prevention. This is not happening because higher health dollar spending has lobbies to support it. Social spending also is a dollar expense that shows up immediately whereas results are spread out into the future as a longer term benefit with healthier populations that need to consume less healthcare and treatment. Elizabeth Bradley, president of Vassar College is an expert on this subject. She says social services spending gives more bang for buck  in health outcomes.  The point is relevant also for countries in South Asia and Africa that have taken some on some aspects of the U.S. health system resulting in health care spending that does not deliver the most for the dollar spent, and ignores the critical role of prevention.  The solution lies in moving these health care dollars out of the health care spending and into education for health outcomes, lifestyles, exercize habits education, and into social services that enable prevention and better health. ...

In parts of Mexico sugary softdrinks are easier to access than clean tap water, says this report in DW.com. This is a problem that existed in Mexico for many years. Amy Guthrie in the WSJ August 28, 2013, described the problem in -Health Problem over Soda Flares in Mexico- which was shown in Lyrarc.com in 2013, showing the US, Chile, Mexico and Argentina with high consumption of sugary softdrinks and high rates of diseases related to this. Mexico's government has made efforts to increase awareness about the risks and dangers of overuse and Bloomberg philanthropy has made efforts to increase awareness. Yet the problem has persisted. The risks are high for countries such as India, China, Vietnam. One ad in Mexico City subways showed 20 ounce sugary softdrink bottle and asked "Would you take 12 teaspoonfuls of sugar?" Mexico passed the US in countries with high obesity rate over 100 million people in 2013. Higher all cause mortality was shown in a European study of 451,000 people for people drinking more than 2 glasses of sweetened softdrinks a day, with data collected between 1992-2000 and supporting public health campaigns limiting the use of such sweetened softdrinks. ...

There is no treatment for nonalcoholic fatty liver disease linked to obesity. It is twice as prevalent in Hispanics compared to other whites and African Americans. The fatty liver is a result of bad eating habits and lifestyle with little exercize. It is expected to be the leading cause of liver transplants in the U.S. by 2020. At the current rate about 25 millon Americans are expected to have the disease by 2025, according to the chief of liver transplant division at UCLA School of Medicine.

The migrant workers are the ones who work the hardest. And productivity is low. Among the higher classes there are longer hours with the work pressures, family obligations and long work hours leading to insomnia, fatigue, obesity, and ill health conditions. A comparison shows Britons working 1677 hours on average according to the OECD. The average Chinese worker is shown to work 2000 hours, by a researcher at Beijing Normal University. A labor economist in Beijing says as the economy improves and working conditions get better workers are working fewer hours every year. He says China lags behind in productivity. The longer working hours he says are not good for worker's health and for productivity. This was said in 2015 when China was still chasing GDP growth without the level of technology the US and Europe had. Now the focus has shifted to better quality growth in advanced technologies and old factories closed. ...

Sales of tea drinks are up 15.6%, while soft drinks are down 5.6%, for the first 10 months of 2010. An example is chick peas eaten as hummus which has grown rapidly. The former Surgeon General of the U.S., Vivek Murthy, has tackled issues such as the opioid crisis, obesity and poor health outcomes. In his new book "Together: The Healing Power of Human Connection in a Sometimes Lonely World," Murthy looks at the loneliness he says is pervasive in the new culture of iphones and social media that have perversely reduced not increased social connection and the feelings coming from good relationships and social concerns.  A 2020 Cigna survey shows 61% of American adults are lonely, up from 54% in the 2019 survey. It is not about having a lot of people around you, it is about feeling a genuine connection and the quality of relationships. Murthy finds this lack of genuine social concerns and social connection as fairly pervasive from his conversations with people all across the country. Normally it is felt in the fifties as life changes, and in the eighties with loss of mobility. Today the tech devices and what is called scoal media that appear to have increased communication have actually reduced the level of quality connections and interactions. Dr. Murthy suggests volunteering, and service in the community or larger communities worldwide as a way of breaking this. Capitalizing on informal day to day contacts, including with people one has not known before, is another way. This shifts the focus to the people around us and brings a new dimension and quality to our lives.  Saying hello often and smiling genuinely, says Murthy. Improving the  quality of time in day to relationships is another. This can also give us the confidence to connect with people on a regular basis.  ...

A "Best Countries" ranking was announced in Davos forum by U.S. News and World Report and its editor-in-chief, Mortimer B. Zuckerman, in Jan. 2016. This is the first time this rating is being done. It gives top spots to countries based on surveys of 16,000 people, 8000 from informed elites of better educated people, the rest from the public. The rankings are in many categories including quality of life, cultural influence, heritage, entrepreneurship, citizenship, raising kids, green living. Canada takes the top spot for quality of life, followed by Sweden, Denmark, Australia and Netherlands. France takes top spot for cultural influence, Italy for heritage, Germany for entrepreneurship, and second spot for forward looking, Sweden takes top spot for citizenship, raising kids and green living. Japan takes the second spot for green living, Canada takes the fourth spot, with Germany in the third spot, the U.S. and the UK in the top ten for green living. India takes top spot for movers with its rising economy. The U.S. suffers because of its obesity levels in some groups. Overall Germany comes first, Canada second, UK third and U.S. fourth. Overall ratings included correlation with per capita GDP purchasing power parity level figures from the IMF....

The NYT's Kristof gives this exceptional report on the use of cancer causing chemicals all around us in many consumer products. With the increased use of chemicals has come the tripling of asthma rates in the last 25 years, leukemia up by 1% a year, increasing obesity, and breast cancer rates increasing from 1% in 1975 to 12% today with only some of it from better detection. Doctors at the Mt. Sinai School of Medicine in New York, say American girls who had puberty at the age of 17 in 1800, now have puberty at the age of 14, even 12. Earlier menstrual cycles increase the risk of breast cancer because of increased exposure to estrogen. Studies show exposure to pesticides, PCB's and other cancer causing chemicals increase the risk of cancer, and a link to early puberty. Asian women moving to the U.S. are also experiencing higher rates of breast cancer as they move to societies with higher use of chemicals in daily life, say experts at Mt. Sinai. Poor eating habits and lifestyles with less exercize are also to blame, but chemicals also play a role. Americans are moving towards shunning packaged processed foods for fresh food in their diet, and more are learning the benefits of regular exercize, but the same degree of public awareness is lacking for the extensive use of chemicals in our consumer society. In other societies around the world that are copying us such as in China, India and Brazil, the situation is even worse, with the spread of a reckless idea that modernization requires jettisoning health safety concerns. Even a simple pizza box has PFAC's chemicals made to make the box resistant to grease. In 2015 where products are labeled environmentally friendly by large companies to attract buyers and build the right image in the minds of consumers, consumers asking questions and making better choices make a difference by offering a seal of approval. Carpets have chemicals, and most disposable plastic containers contain chemicals that could seep into the food if heated. The use of plastic containers and microwaves is common practice in todays society, where less cooking is done on the stove than in the period before 1960. The sheer size of the chemicals developed since 1950 is staggering- more than 80,000 chemicals according to the Children's Environmental Health Center at Mt. Sinai. And this Center says less than 20% have been tested for toxicity. ...

The figures are huge and it takes the World Health Organization Framework Convention on Tobacco Control to do something however small in Japan about smoking. Now the half million vending machines that dispense cigarettes to some 26 million Japanese smokers can only dispense to adults because users have to have a special card called Taspo or tobacco passport that they insert into the vending machine. This will shift sales to convenience stores like the Lawson's chain. In Japan 29.2% of all adults and probably an even higher percentage of men smoke according to OECD data. In the US its about 17%. Like obesity and poor eating habits and the availability of fast food in the USA, smoking in Japan remains largely untouched by any efforts to educate young people and the public about the great dangers, with companies largely uninterested and the government and schools and universities largely apathetic about what are really huge health concerns that reverberate in so many ways across the fabric of society. For example a obese person consumes more health care dollars, a smoker also consumes more health care dollars. And health care dollars are scarce dollars and need to be spread out in better ways than wasted on preventable things like obesity and smoking through partnership in education and other measures between all groups and organizations in society and the government. ...

Rate of diabetes is rapidly going up in India. In addition scientists have shown that Indians are more prone to diabetes if they are obese to the same extent as someone from Canada or the U.S. This is called thin-fat diabetes- a higher impaired glucose intolerance- and comes from centuries of body changes following famine in parts of the country from failure of monsoon rains. The obesity rate is going up dramatically and with it diabetes is up significantly.  From 6.4% obese or overweight in 1990, by 2017 18.8% are obese or overweight, according to Health Metrics and Evaluation Institute of the University of Washington. The International Diabetes Federation now predicts 123 million with diabetes in India by 2040. By 2017 diabetes rates went up from 1990- from 5.5% to 7.7% or 63 million people. The major problem in South Asian countries and in China is the growing use of packaged and processed foods, fast food and carbonated drinks. Efforts to prevent the sale of junk foods is a battle being fought between private citizens and the large processed foods companies such as Coca Cola, Pepsi and Nestle. This is the subject of this article with Mr. Verma taking the case to the Delhi High Court facing large opposition. Mr. Verma left his job as a marketing executive as he took care of his sick child. He filed a case in the Delhi High Court in 2010, and faces the opposition of the India Food Processors Association- so far there is little progress.   ...

This shows that the US and the EU are today more affluent than 100 years back but with it has come a dangerous neglect of healthy living that is destroying happiness for people as they reach the age of 70 yers. A lack of education about healthy living in schools and society is showing up.  Only 10% of people in US and EU have good cognitive, physical and mental health in a major study from 3 Universities. The study tracked 100,000 people over 3 decades their eating habits and exercise and other variables and was done by University of Montreal, University of Copenhagen and Harvard. It shows how the quality of life has deteriorated in the US and the EU as diseases become more chronic for the heart, diabetes, and higher prevalence in population of obesity and substance use disorders. So that older people have more conditions and few can live healthy lives- less than 10% if this study is an indicator of the state of the Nation's health. One of the findings is that a plant based diet with moderate animal protein such as diary and fish is the best for reaching 70 years of age with the best quality of overall physical and mental health. ...

RFK Jr. nomination heads to the full Senate for confirmation as Health Secretary to bring new ideas and solutions to tackle obesity and disease in the US. He called the US health condition dismal as it is, not just for what is spent. RFK Jr called the US a sinking ship when it comes to the health of its people as he answered questions from Senators at Senate hearings. Ron Johnson, Senator from Wisconsin, said RFK Jr. was an answer to his prayers for someone who could help tackle this crisis. The recent effort to eliminate funding for Planned Parenthood clinics in the U.S. because some of the money goes to abortions has run into a cloud of misinformation. In reality only 3% of the funding goes to abortions. Title X funds that help support these clinics cannot be used for abortion care at any time. Medicaid funds going to the clinics in 17 states can be used to reimburse abortion providers only if the life of the mother is endangered. The clinics see 5.2 million low income and uninsured women who need tests for infections, breast exams, pap smears, preventive services and screenings, contraception services. By reducing low income women's access to such health services through defunding the clinics would only increase the number of unwanted pregnancies and abortions. Planned Parenthood centers provides contraception to about 2.5 million patients each year and educates women about birth control. By burdening the U.S. healthcare system- adding most of the 5.2 million who access these clinics -with problems ranging from cancer to other serious health issues when they could have been detected by tests at an early stage and treated earlier or prevented altogether, would also add to the burden of healthcare costs. In addition the 800 Planned Parenthood Clinics in the U.S screen 3 million patients each year for other problems such as blood pressure, diabetes, smoking and obesity related issues, also help treat these problems at an earlier stage, which is essential if costs to be brought under control. ...
