WSJ looks at the changes in the way medicine should be practiced in the light of what we have learned from the pandemic.  Medicine practiced before the pandemic and still today relies mainly on a visit to the doctor or specialist who is short of time. There is a shortage of doctors. Patients have many illnesses as a result of decades of neglect of proper nutrition, and exercize habits. Obesity is at about 40% in the U.S. about 30% in the UK and 17% in France, and high also in other parts of the world. These high rates were unknown throughout history and result in many illnesses and increase by four times the vulnerability to the coronavirus. One authority in medicine calls obesity pouring gasoline on a fire for effects of the virus.  A doctor's appointment with doctors short of time with no coordination around a whole range of factors related to obesity, illnesses, health checkups, mental health, is now seen as a heavily handicapped way to practice medicine or for patient healthcare and wellbeing. The alternative is discussed here as the way forward. A  team will be responsible for a patient's care not just an individual doctor. The team would care for general health after a patient's checkup, cover individual illnesses, weight issues, mental health, exercize nutritional needs and other good healthcare habits. Instead of relying on doctors at a time of shortages of doctors the team would be led by nurse practitioners.  A nurse practitioner is someone with a bachelors degree and a masters degree or doctoral degree in nursing with 1000 hours of clinical training. Studies have shown that they are effective and even more effective than individual doctors. Today particularly with the problem of doctors with limited time compounded by the built up problems of decades of bad habits in nutrition and exercize and poor "cultural" habits getting entrenched, there has never been a greater need for a better way to practice real healthcare for a person's wellbeing. Particularly in rural areas with an even larger shortage of doctors the health practitioner led team will play a big role. Patients will under this setting receive more care virtually and get more followup care by phone and video messaging. The numbers tell the story- there are shortages of doctors in USA, Europe, Africa, Latin America and Asia. In the U.S. shortage of doctors is 55,000 projected to 2033 by Association of American Medical Colleges. There are 290,000 nurse practitioners licensed in the U.S. and 131,0000 physician assistants. The goal will be to get an adequate number of nurse practitioners licensed in this decade to take care of these teams. The pandemic has made virtual visits to doctors and nurse practitioners popular. Medicine reimbursement should and would be practiced on the basis of how well a patient is doing not on a fee for each micro service that is delivered. For this to happen the teams led by the nurse practitioner have to commit to patient education of the benefits from good practices and good habits for nutrition, exercize, caring for oneself. A doctor short of time is hardly the person to carry on this patient education which is where the major opportunities for a new system arise. The virtual care also provides a new medium for patient education and awareness of the risks of getting illnesses, preventive actions to be taken in advance. One approach being tested in California and Texas is for a monthly fee for patients more payments by health plans to doctors or healthcare teams if the patient is healthier. Additional health professionals are added to the team including health coaches, dietitians and medical assistants to increase its effectiveness in counseling and education and monitoring.  The nurse practitioner team approach is already being practiced in parts of the U.S. including the example of New Hampshire shown here, and is predicted to be the approach for primary care in the next decade. ...

During the first days of vaccination drive in India the uptake is lower than expected, below 48% in many states of India. Healthcare workers are coming forward in smaller numbers. The glitches in the app that was used to transmit information is one reason, the other being that not enough information was disclosed about the two vaccines.  On the first day Jan. 16 India vaccinated 200,000 healthcare workers. The target was 300,000. The uptake is lowest in states such as Tamilnadu and Punjab.  One of the reasons for the low uptake is also that India has a lower number of cases, and cases are in decline in India for coronavirus. The first wave peaked on September 13 at over 90,000 and has declined ever since to 13,000 on January 19, 2021. The Indian diet includes vegetables and a large part of the country is vegetarian. Indian diet also includes herbs such as turmeric, ginger and cinnamon, and many others that help build immunity. The lack of cars for large numbers of the population means a lot more exercize walking long distances is essential in daily living. Another factor is that the mandate for masks is strictly followed everywhere. Also helpful is that after independence in 1947 the pharmaceutical industry was built up to serve the large population with prices regulated carefully by the government, enabling steady supply of medicine during the pandemic. ...

China's government is taking up stakes in private companies with large debt and needing financing. Private enterprises have less access to cheap bank loans and other types of financing than state owned firms, and are squeezed by China's efforts to reduce pollution and overcapacity. The tariffs war with the U.S. has also hurt the economy and taking stakes in private companies is way to ensure business stability for China. Its an effort to keep employment stable in the private sector that has 60% of the jobs. Zhejiang Great Southeast Company is a plastics packaging company with founder Huang selling his entire 29.5% stake in the company to state owned Zhuji Water Group Co for $168 million. He did this to repay holding company loans for which he pledged two thirds of Zhejiang Company shares. Beijing stepped in to ensure there is no sharp rise in unemployment. In the first 6 months of 2019 Beijing took 47 such stakes, according to Fitch Ratings, with 52 stakes taken for all of 2018.  The purchase of stakes includes state run companies and investment vehicles of local governments. Even this does not reflect the whole effort of China to ensure no sharp increase in unemployment. From October 2018 local authorities and state linked entities put together about $100 billion of "relief funds" very quickly, estimates from TF Securities. These funds are for passive investments, state owned enterprises normally take on a hands-on role in running the companies. Oxford Economics estimate is that China's private sector provides about 60% of all urban jobs in 2017, increasing from 36% in 2010. Researchers say China stepped in in this way after failing to get banks to lend more to the private sector. The tight supervision to reduce risk of supervisory agencies has made it harder for private companies to get loans. Shadow banking and trust loans was an early target, and stock market selloff hurt entrepreneurs who used shares as collateral for loans. ...

Academy Heights Elementary, of Pinehurst, N. Carolina, is expected to close as part of the cost cutting efforts in North Carolina. The state is trying to close a budget deficit. Monroe County's superintendent proposed closing the school to save $500,000. The school has a 98% pass rate on state exams, an award winning math program. It is ranked the second best kindergarden to 5th grade schools in North Carolina. Parents and teachers point out that many of the students come from middle class families where both parents are working, and military families from Fort Bragg, and this is not just the demographics of being near Pinehurst resort; as higher income parents tend to send their children to private schools. They point to the way staff, parents and students work to create an environment that challenges children to learn. The questions here remain ones that are being taken up throughout the country- how to invest or disinvest in education in the face of budget deficits. Should education take cuts and how much? Another twist here is that Academy Heights is in a depressed black neighborhood of Tarrytown whose residents go to to other schools even though Academy Heights urges students from Tarrytown to apply. It is an elementary school and as elementary education has a special role to play in early childhood development, how much of these cuts should fall on elementary schools? Should the best schools be cut under any circumstances or should they be supported with sacrifices made elsewhere or do moderately higher taxes make sense in these situations? Does it make sense to preserve existing excellent schools even as the search for improvement in educational systems takes place -with investments like the $290 million the Gates foundation is committing to improving selected schools? produce excellence in other schools?...

China's tariff of 25% on cars imported from the U.S. is a  poor target says this report in WSJ, as most of the cars China imports from the U.S. are made by BMW, Mercedes and Tesla. China already has a 25% tariff on U.S. made cars.

The German cars are made at the Spartanburg plant and other plants of BMW and Mercedes in the southern U.S.

Tesla cars would also be hurt yet Tesla has supported the Trump administration tariffs as the existing 25% tariff makes it harder for Tesla to compete in the Chinese market. U.S. and European carmakers cannot hold more than 50% foreign ownership under China's rules in its auto market. As a result U.S. carmakers already have joint ventures in China and make most of the cars they sell inside China.

Eni has an excellent record in oil exploration and is expected to increase its production by 2-3% over the 2013-2018 period with a 15% return on capital, according to Citigroup. "The NHS is on its knees," says Starmer, one more reaon to ban outdoor smoking. Labor in 2007 put the ban on indoor smoking. Sunak had a plan for the Tories to ban smoking. Starmer is now following this to reduce the burden on the NHS and improve public health in the UK. The bill would gradually ban smoking for people born after 2009, an idea proposed by Sunak and the Conservatives. The bill would place new restrictions on outdoor smoking, including outdoor spaces at, and pavements outside, clubs and restaurants, as well as at universities, children’s play areas and small parks. Asked about this during a visit to Paris, Starmer said: “My starting point on this is to remind everybody that over 80,000 people lose their lives every year because of smoking,” he said. “That is a preventable death, it’s a huge burden on the NHS and, of course, it is a burden on the taxpayer. So, yes, we are going to take decisions in this space, more details will be revealed, but this is a preventable series of deaths and we’ve got to take action to reduce the burden on the NHS and the taxpayer.”  The prime minister said-“It is important to get the balance right, but everybody … who uses the NHS will know that it’s on its knees.” Dr Layla McCay, the director of policy at the NHS Confederation, told BBC Radio 4’s Today programme: “It is absolutely the health challenge of our time. It’s the leading cause of preventable illness in the UK, so we are heartened to see that progress is being made and that the intention is moving forward to really address one of Britain’s main drivers of health inequalities.” ...

China's dollar for dollar retaliation on $16 billion of U.S. imports with 25% tariffs set to take effect August 23 excludes oil which was on the original list. China takes in about one fifth of the total U.S. oil exports, and in the space of 2 years has become the largest importer of U.S. oil. Experts say China could be shooting itself in the foot if it decides to place tariffs on oil imports from U.S. China is dependent on foreign sources for 70% of energy needs and this trend continues. Another reason say analysts is that by keeping oil out of this trade dispute there is more chance that China can continue importing Iranian oil through a waiver  after U.S. sanctions on Iran go into effect in November.

The U.S. also exports higher quality oil that is less polluting and a grade which is used in newer plants.

523 European banks borrowed 489 billion euros from the European Central Bank on Dec. 21, 2011, under the newly created Long Term Financing Operation of the ECB. This is designed to meet the financing needs of European banks which are shutoff from normal financing of selling unsecured bonds to private investors because of market anxiety. Much of this is for replacing other outstanding ECB loans, with analysts estimating about 190 billion euros of new liquidity being injected into the banking system. This also has the effect of reducing the borrowing rates for government bonds. In Spain the government sold 5.6 billion euros of government bonds at an auction on Dec. 20, 2011, with the interest rates dropping from 5.7% a month earlier to 1.7%. Small and midsize banks in Spain helped surging demand by buying the bonds to use as collateral for three year loans from the ECB at 1%.

With the death toll over 10,000 on March 7, and a rise of 16% over the previous day, French public health authorites have set new rules for exercize with a daytime jogging ban. Jogging is allowed only when the streets are at the quietest in Paris and not allowed between 10.00 and 19.00 local time.

France's lockdown is strict unlike that in America. Anyone going outside has to carry a document giving the reason- shopping for necessities, visiting a doctor, or exercize within 1 kilometre or half a mile of their address. Police have fined hundreds of thousands of people for breaking these restrictions.

China places a ban on imports of coal, iron ore, wine and other products from Australia. China's largest imports from Australia are in coal and iron ore of $87 billion. China has targeted Australian exports that can be alternatively sourced or are produced domestically such as coal. Australian coal exports to China declined by 90% in 2020 over the prior year. China is the largest destination for Australian wine exports of $1.2 billion. Australia's call for an investigation into the origins of the coronavirus pandemic and new laws to curb foreign interference have resulted in deteriorating relations with China.

Paul McCartney of the original Beatles group does a new solo impromptu effort that ends up as an album. It was done in a very casual manner with no intention of producing work that does a job. During the lockdown period McCartney experimented with unfinished work on the musical instrument that he had used for the work. Each day he would decamp to his studio to relax and have fun. He says it was all about making music for yourself and having a lot of fun not doing a job or work. The album is called McCartney III to be released in December.

Rachel Bernhardt 28 years, is from Drexel University. She started swimming competitively late- at Drexel University and missed a lot of the early training Olympic athletes get. She is now training under longtime Northwestern University swim coach Bob Groseth who is at Queen's University in Charlotte, North Carolina. Part of the training is to focus on the technique that she did not get to develop in the early years. She has persevered through it all including a knee surgery and rehab. She now trains to be part of the US national team for the World Aquatics in Fukuoka, Japan in 2023.

German attitudes of caution towards the coronavirus. This caution extends to facial coverings and social distancing guidelines. About 89% say they wash hands more often. 88% say they keep a clear distance from others. And 72% say they meet friends and relatives only to a limited extent. Survey is by Infratest dimap conducted for ARD the public broadcaster. That caution has helped Germany, and is being followed in Italy and France. It is in sharp contrast to parts of the U.S. and in places such as Texas where masks became mandatory in public  spaces only on July 3, 2020.

Fearing retaliation by Chinese regulators, US executives are silent on trade issues. As a result the issue of wind energy subsidies to Chinese manufacturers was brought up not by GE, a manufacturer of wind turbines, but by the United Steelworkers Union in the US. The US filed a WTO complaint in this matter based on the US Steelworkers petition. GE has stayed silent in this matter in deference to Chinese regulators. Only Solar World, a German company, has stepped forward to strongly endorse the investigation. Solar World has manufacturing sites in Oregon and China, but no plants in China.

The U.S. Senate voted 51 to 49 on a Democratic party measure for further reductions in 2012 Social Security payroll taxes for workers and employers, including a surtax on incomes over $1 million. A measure supported by the Republican party to pay for the payroll tax cut by reducing the Federal payrolls was defeated, with half the Republicans voting against it. Democrats hope to use this issue to show Republicans favor the rich over the middle class, as the payroll tax cut benefits most Americans. Polls show Americans by a large majority see Republican policies favoring the rich. A New York Times/CBS poll in October showed 7 of 10 Americans feel this way. Pollster Geoff Garin says the income inequality issue is beginning to override other issues including antigovernment feeling. This is one way in which the Occupy Wall Street Movement's slogan of "the 99 percent" has resonated with U.S. public opinion. The Democratic party sees this as an opportunity to define the campaign issues for 2012, with Republicans running for reelection cautious about being seen this way....

Mark Carney, Governor of the Bank of England, in meetings with bankers and business leaders says Britain should remain in the single market 2 years after exit from the European Union, according to the Sunday Times. Theresa May plans for Britain to exit the EU in 2019. The reason is that this would protect business as it adjusts to leaving the single market, a kind of transition or Brexit buffer period. This period "really informs what businesses need to do because you transition and restructure during that window," Carney told a House of Commons Treasury Committee. About the changes in the politics in the U.S. and Europe Carney has said about basic fairness in bankers language- "market fundamentalism can devour the social capital needed for capitalism" to work, referring to the moral failures in operations of the banks by 2009 and how it hit the middle and working class incomes and wealth.

This opinion in the Hindusthan Times points out that prime minister Modi's speech at Kozhikode following a militant attack in Kashmir in September 2016, reflects a long standing policy since the late 1970's of Congress party and BJP or Janata party administrations. The idea is to encourage cross border exchanges to reduce tensions. The emphasis in back channel talks between India and Pakistan also emphasize the idea of CBM, cross border movement. The prime minister of Pakistan, Nawaz Sharif, has also expressed in the past the importance of cross border movement and trade as ways to improve the economies of both countries. The idea of building up trade and increased exchanges between the two countries is supported also by the U.S. and other western countries. The example of Ireland and Northern Ireland where trade and cross border exchanges are considered important by all parties after Brexit, is an example of how important this is.

German chancellor Merkel met with leaders of Germany's 16 federal states to come up with an exit plan for reopening the economy from the lockdown in phases. In the first phase shops with up to 800 square metres of space will reopen on April 20. Bookshops, libraries, car dealers, bicycle shops, and museums will open too. Larger retailers will wait till May 4 to reopen. On May 4 school children in primary school can attend school and teenagers can take exams. Germany has 133,000 infected cases, 3592 deaths. Merkel warned that the performance with coronavirus was "fragile and provisional success" and the need for social distancing measures. A ban on gatherings of more than 2 people from separate households will remain in place till May 1. The government will strongly recommend that face masks be worn in public and in shops, public transport. Mass events will be prohibited till September. Extreme positions on business related issues taken by politicians in the Republican party. An effort to influence the Republican party's platform through the ouster of moderate Republicans like Jim Bennett of Utah. The impact on the US Congressional elections in 2010. Positions such as the abolition of the Federal Reserve, prohibiting stimulus funding, sealing the borders and doctrinaire positions on the role of government. By appealing to popular discontent with the Obama administration, and anger with the bank bailouts, what is called the Tea party movement has taken shape. It is built around politicians Sarah Palin of Alaska, Jim DeMint, Rand Paul of Kentucky, and other candidates such as Haley in South Carolina who are using it to tap into discontent. It has the support of Fox News, and Dick Armey former Congressman from Texas who heads FreedomWorks, and libertarian billionaire David Koch. Built around seductive principles of small government, the movement has no clear program considering the diverse positions of the different politicians and different extreme positions adopted, including a general hostility to large corporations without differentiating a BP or a Goldman Sachs from a GE. GE's operation of MSNBC puts it in the same category as a Goldman Sachs. The lack of a clear position by the US Chamber of Commerce, because of its opposition to the Obama administration. The movement carries with it risks, as the Republican party's control of the House of Representatives is not ensured. Ideology of a vague kind has become a substitute for good credentials and experience, in the fast-forward effort by activists such as Dick Armey to capture popular discontent. Says Dick Armey, "We live by the creed 'hard work beats Daddy's money,' " raising serious questions about how a statement like this would help the jobless or the economy. ...

A vote on Brexit and whether Britain should stay in the EEA (European Economic Area) in the British parliament, has 75 Labor MP's supporting staying in EEA and 15 opposing it. Labor party leader Jeremy Corbyn wanted Labor to abstain in the vote. 

Corbyn and Brexit Shadow Secretary McDonnell are socialists who see the embedding of the capital markets model in the EU as a mistake. They favor full access to the single market area EEA without the obligations of membership. As parliament votes on Brexit there is the prospect that parliament could overturn Brexit and a fall of the government of Theresa May, with opposing factions in each party voting across party lines on the issue of the European Union's relationship with Britain.

The landing of the lunar probe Chang'e-4  on January 2, 2019 makes China the first country to land a probe on the far side of the moon. This follows a landing by Chang'e-3 lunar probe in 2013. A followup mission Chang'e-5 in Dec. 2019 will collect samples of the moon's surface and send them back to earth. The goal is to build a lunar base by 2025 and man the facility by 2030, and in the long run mine the moon for energy resources.  The U.S. space program  has lacked the steady commitment and vision needed to put astronauts on the moon for a second time, and is now being given a fresh boost for challenges ahead.
