Pfizer and Moderna's plans to make mRNA vaccines in Africa, Asia or Latin America may take much longer than 2022. The solution to producing an mRNA vaccine in Asia that could be mass manufactured and distributed throughout Africa, Asia and Latin America is now at hand. Gennova based in India, is partnering with Seattle startup HDT Bio to attack the problem of temperature and scalability in manufacturing for a mRNA vaccine that acts as a global solution using India's manufacturing capabilities. Dr Singh who founded Gennova, says- "We wanted to solve the problem of the scalability issue, and the temperature issue. If we can solve these problems, we are building a solution not just for India, but also a global solution." Gennova received seed funding from the Indian government. Other companies in Brazil and South Africa lack the manufacturing capabilities or financing needed that exist in India. The Indian government has achieved an initial goal of one billion vaccinated in just 6 months. The next step for India in its health infrastructure buildup is a mRNA vaccine that is an improvement over Pfizer and Moderna vaccines that can be stored easily, adapted for variants, and manufactured in large quantity as a global solution. ...

India's pharmaceutical industry gets a major boost with the development of a mRNA vaccine by Gennova Pharmaceuticals called HGC019. Gennova is funded partly by the government and is being developed in conjunction with Seattle based HDT Bio Corp.  The new vaccine can be stored between 35 to 46 degrees Fahrenheit making it easier to distribute in middle and low income countries with less storage and logistics facilities. It also uses a process of freeze drying called lyophilization to produce the vacine in powder form. It can then be more stable and easy to ship, easy to store at moderate temperatures. The powder can then be diluted with a diluent before vaccination. Another part of the new technology of this vaccine is its use of self-amplifying mRNA instead of non-replicating mRNA. This helps to administer smaller doses with less side effects. India's drug regulator has already approved Gennova vaccine in August for second and third phase trials after the shot was found to be safe in early stage study. Gennova Pharmaceuticals aims to get HGC019 approved for emergency use by Indian authorites this year and manufacture 60 million doses by December end. ...

Peter Hotex of Baylor Medicine in the US is a pioneer and leader in getting low cost traditional vaccines to billions in Africa, Asia and Latin America.  Here Peter Hotez of the Baylor College of Medicine Tropical School is interviewed in The Hindu. He talks about how the new mRNA vaccines are "shiny toys" pushed forward in the US in 2020 under innovation drives, and that the vaccines made by traditional methods are just as effective and provide lasting protection. Without vaccinating the entire world population including the billions of people living in Africa, Asia and Latin America, there will be no end in sight for the pandemic, he says, and the best way to do this is through vaccines made by traditional methods, methods used by Bharat Biotech for Covaxin and Biological E for its vaccine. He said mRNA is a brand new technology  and "it will take years to scale it up to make 9 billion doses" of vaccine for poor countries. Baylor has developed the vaccine technology using traditional methods such as yeast fermentation expression technology used for Recombiannt Hepatitis-B vaccine. Its been around for 40 years. Baylor will transfer the technology to Indonesia, India, Bangladesh, all over the world. He says in terms of virus neutralizing anti-body immune responses it is as effective as the mRNA vaccines. Hotez is critical of some pharma companies- "the rest of them want to bicker about patents. we're not going to go down that direction." Baylor is providing its technology for manufacture to companies to fill the need in poor countries, without patent protection or quibbling about legal things such as indemnities, says Hotez. Hotez also thing recombinant protein technologies vaccine with its traditional approach could also overcome vaccine hesitancy, a key factor for unvaccinated in Europe and US which have stuck to mRNA vaccines. The newer technology behind mRNA could make parents hesitate to vaccinate their children with these technologies, and also be a part of the mental attitude of unvaccinated adults having hesitancy.   ...

Most questions about the vaccines from Oxford, Pfizer and Moderna are answered in this Q&A in The Times.  The Oxford vaccine is the only vaccine that is being provided at cost to the public at a cost of 4 pounds, Pfizer using German technology at 15 pounds and Moderna from Cambridge, Massachusetts, at 28 pounds. The Oxford vaccine can be stored in a fridge, the others use mRNA technology of messenger RNA which requires ultra low temperature storage. Astra Zeneca could have handled the trials and methodology for results in a better way. As the two trials one that produced results of 62% and the other results of 90% cannot be combined to give results of 70% but are two distinct and separate trials. However too much emphasis has been placed on the vaccine, as other prevention measures remain important for 2021. Other vaccines are being developed in Britain with new technology and in India by ICMR which are in trials stage and about which not enough is known. This includes the Moderna and Pfizer in the mRNA technology and the viral vector Astra Zeneca, J&J vaccines. The amazing story of Katalin Kariko who came to the US from Budapest, Hungary, in 1989 taking a position as research assistant professor at the UPenn Medical School.The work of Katalin Kariko in mRNA vaccine research that led to the discovery of mRNA vaccines was derided at first at Penn leading her work to be shunted to a lab on the outskirts of town and having her pay cut in a demotion, says this WSJ report. It won the Nobel Prize for Medicine in 2023. Universities are not places where new ideas can get a boost when there is much skepticism and constant pressure for research funding on more conventional lines. Less opportunity for experimentation that can lead to new discoveries that revolutionize science and medicine.  Kariko and others working as research assistant professors were shunned at Penn and referred to as "aliens" because inthe interests of research they took lower paid positions. As it turns out Kariko felt liberated during the period of her being demoted, to work even more patiently on the mRNA molecule, one that was more difficult than the DNA molecule most researchers had focused on. Biotech firm CureVac is developing a vaccine for coronavirus that should be available by fall 2020. It hopes to develop the vaccine using mRNA (messenger ribonucleic acid) as a source of information. Managing director of Hopp Biotech the holding company for CureVac, says mRNA has several advantages, including one related to a vaccine developed by CureVac for rabies. He says 1 microgram could suffice to vaccinate 1 million people because it is very potent, and because it can be mass produced quickly.

EU Commission president Leyen announces Pfizer will supply EU with 1.8 billion doses of Pfizer vaccine to 2023. The increased supplies will include booster shorts to increase immunity. Another change is the new date for 70% of European Union population to be vaccinated, which is now advanced to end of July instead of end of September. With new waves of the coronavirus affecting Europe, and criticism of the EU's earlier effort in securing vaccine supplies, more urgency has gone into the new effort in 2021. In this interview in The Hindu with Public Health Foundation of India president Srinath Reddy, the importance of safe behaviours including masks and social distancing, public discipline to tackle a potential third wave. This was also pointed out separately by the Director AIIMS.  Mr. Reddy says study by Public Health England shows just one dose of Astra Zeneca vaccine is not very effective against the Delta variant- about 30% effective. As a result he suggests the need to reduce the spacing intervals which are at 8-12 weeks to a shorter interval now that adequate supplies of vaccine will be available starting in July and August till the end of the year. The initial spacing was 4 weeks when Astra Zeneca vaccine was first introduced and at which time the variants had not emerged. The Indian federal government affidavit to the Supreme Court shows that 1880 million doses of vaccine will be available by the end of the year 2021 to vaccinate fully with 2 doses the population of India over 18 years using 5 locally manufactured Made in India vaccines. This does not include the mRNA vaccines that will be made available from Moderna and Pfizer for which cold storage facilities are being prepared by the federal government.   ...

Pfizer coronavirus vaccine is shown to be 90% effective in results from Phase 3 trials. The findings come from a preliminary look at the first 94 infections in a group of 44,000 people in the trial. The vast majority of the cases received a control instead of vaccine. A vaccine is considered effective if it works on 50% of cases. The Pfizer vaccine is based on an experimental "mRNa" technology.

Britain has ordered 5 million till the end of 2020 and another 20 million for 2021. In total Pfizer plans to make a billion doses of the vaccine by 2021.

The story of the Pfizer BioNTech vaccine is the story of 2 chikdren of Turkish immigrants to Germany. Sahin the son of a engineer working at a Ford factory in Cologne, and Tureci the daughter of a surgeon working at a hospital in Mainz Germany. Sahin was born in 1965 on the Mediterranean coast in Iskerundun, Turkey and he went to Germany when he was 4 years old, his father being recruited in a new effort to rebuild Germany with foreign labour. Both are motivated by scientific research and the drive to come up with some method to tackle cancer for patients with new research and cures.  Both did their doctoral dissertation on experimental therapies at the Johannes-Gutenberg University of Mainz in Germany, and both joined the faculty there. Sahin spent years studying the mRNA , genetic instructions that can be delivered to the body to help it defend itself against viruses and other threats. Much of this mRNA research was already at an advanced stage in January 2020 when Sahin heard about the coronavirus in China. At that point he saw the potential of retargeting the mRNA research to tackling the coronavirus. By this time he already had his own company with over 200 million euros invested in it  by investors including Helmut Jeggle, now supervisory board chairman of BioNTech. This report says he sat down one Saturday, January 25, 2020 and working on his computer designed the template for 10 possible coronavirus vaccines, one of which would become BNT162b2, the vaccine now approved in Britain. On the same day he told a surprised Jettle that he would refocus the company on the new virus that had not yet hit Europe. Shain he says cited the Hong Kong flu that claimed 4 million lives. Why Pfizer. Pfizer had already been working with BioNTech on a new flu vaccine based on mRNA technology. A cooperation deal was signed with Pfizer in March for organizing clinical trials, manufacture globally, and distribute the vaccine. Action for a global response-The WHO can act as the vehicle for action globally-  restructure it and empower it to do the job. Put the right people in place who have the confidence globally. Set a new pandemic treaty and put it in place. 2.  Get a more advanced version of the mRNA vaccine which does not need a cold chain. India is reportedly working on one. Put funds and people in place for vaccine drives in poor countries.  Other action needed is learning from the situation in South Africa where the HIV patients are reported by Stephanie Nolan in NYT to have been more susceptible to mutations of the new variant. How can resources be put in place in poor countries so that patients in vulnerable populations get their medicines regularly, and are vaccinated. Overall how can these populations have the vaccine supplies needed- including ones without cold chain suggested by Larry Brilliant, potentially through vaccine work in India. Can international teams be developed with developed countries in Europe and US financing the effort and India offering its manpower and knowhow?   ...

The Moderna vaccine can be more easily shipped and handled than the Pfizer vaccine making it easier to use at smaller hospitals and in rural areas. Moderna would add 20 million doses in December to that of Pfizer's 25 million doses for the U.S. Moderna vaccine is developed in Cambridge Massachusetts Labs and is manufactured in the U.S. and Switzerland.  Moderna vaccine can be stored in freezers does not require ultra low temperatures as Pfizer vaccine. Once thawed it can be stay refrigerated for 30 days compared to 5 days for Pfizer vaccine. Moderna ships in containers of 100 vaccine doses or larger, Pfizer minimum container size is for 975 doses. Both Pfizer and Moderna vaccines are in th 94-95% effectiveness range. Moderna can be injected as is, Pfizer's requires to be diluted in a separate solution. On Dec. 11 the U.S. ito double its purchase of Moderna vaccine to 200 million doses by end of June, Pfizer 's is still at 100 million doses. Moderna may get its approval from FDA in U.S. by Dec. 18. Both Moderna and Pfizer would require a second dose in 3 weeks, both use the same MrnA technology. J&J is developing a vaccine that requires only one dose. J&J finishes its trials in January and expects approval in February.  ...
