After a sudden surge in the beginning of May the coronavirus wave in India, its second wave, is decreasing. It is 186,000 new coronavirus cases on May 27, down from over 350,000 at the peak. In India's largest state Uttar Pradesh with a population of 210 million the coronavirus cases have dropped sharply to 4000 a day. New coronavirus cases in Delhi and Bombay are around 1000 a day. The Indian government has moved quickly to tackle this wave with decisive action to meet the sudden surge in May 2021. For the rest of 2021 and into 2022 the most important action by the Indian government has been to create a government sponsored effort of India's entire pharmaceutical company sector to reunite in production of over 2 billion vaccine doses by December 2021. After independence in 1947 one of the steps taken by India with great foresight was to create a strong pharmaceutical sector with fair and transparent pricing of basic drugs and vaccines. Something that does not exist in this manner and scale anywhere else in the world. Today this is giving India a tremendous asset in the fight against coronavirus. OECD forecasts show an acceleration of US economic growth in 2021 with the $1.9 trillion aid package of the Biden administration. OECD forecasts show pre-pandemic levels of output reached by mid 2021, 6 months earlier than expected. Global output is expected to grow by 5.6% in 2021, after declining 3.4% in 2020. Main reason- US economy is seen expanding at 6.5%, twice as fast as previously forecast and fastest since 1984. OECD sees the importance of stimulus coinciding with vaccination of the population. The pace in the US with 18 million vaccinated in March and the goal of vaccinating the whole population by May is part of the reason given for the vigorous growth. Astonishingly the OECD sees the US economy larger in end of year 2022 now than it had forecast before the pandemic. For the first time the number of people vaccinated in a single state in India reaches 40 million. In Maharashtra state in western India, with state capital Mumbai, the vaccinations crossed 40 million on July 20, 2021, according to the state's Additional Chief Secretary for Health. India has vaccinated 410 million people by July 20, 2021. By preventing serious illness in patients two pills for coronavirus from Pfizer and Merck can change the course of this pandemic. Pfizer will give a royalty free license to Medicines Patent Pool, an organization backed by the United Nations for its coronavirus pill. Merck has also given a royalty free license to Medicines Patent Pool for it pill. This organization will then grant a license for the manufacture of the coronavirus pills to poor developing countries. India is included in the list of countries yet China, Russia and Brazil are excluded for the Pfizer license.  Merck has given Indian generics manufacturers approval to make the pill. This will provide supplies of the pill to 105 developing countries, South Africa will also make the pill to provide it at $10 a course in Africa. Pfizer will begin manufacturing of large quantities in 2022 with 50 million planned for 2022 and 21 million in the first half of 2022. Pfizer pill will be made available in 95 countries. The Merck pill is called Molnupiravir. Pfizer pill is called Paxlovid with 30 pills taken over the course of 5 days. Pfizer pill is effective in studies when given to people who had not been vaccinated. It has to be given within 3-4 days of getting coronavirus confirmed through a test which makes testing critical for its use. ...

The two waves of the coronavirus in India can be compared looking at the graphs and this report in the Times of India. The case volume and deaths in the worst hit state Maharashtra is shown here. The case volume increased by about 50% in the second wave but deaths were about half that in the first wave. Most of the deaths were in the people over 65 and most of the cases in the ages below 40 years. In the over 65 age only 5% have been vaccinated which means that medical management is still the best way of tackling the coronavirus. Vaccine supplies are the bottleneck and this is beginning to change- so that by August ample supplies of the vaccine should make the difference in bringing down cases and deaths.  Lockdowns are managed carefully so that the economy can recover in the second half of 2021 and in 2022.  Any assessment of the crisis management must take into account the speed of the response, its effectiveness, and keeping in mind the economic recovery needed following the pandemic. ...

States in America's Deep South have a much lower rate of people having taken one shot of vaccination, in the 30-40% range by May 2021. This report says states such as Alabama, Arkansas, Georgia, and others in the South are at risk of seeing a new wave of the coronavirus  because people will spend more time in airconditioned spaces in the summer. In contrast to the north with cold winters and indoor heated spaces people in the southern states can spend more time outdoors because of the warmer weather in winter. This may have protected southerners during the winter and spring months. This may reverse with more time spent in airconditioned indoor spaces in close proximity where the coronavirus infections can increase. This report comes as new reports show the Indian coronavirus variant becoming more prevalent in the UK and other countries. This variant spreads about 50% more rapidly than an earlier UK variant, say experts. Another analysis in The Times of London shows that the imperceptible rise phase of the new coronavirus variants is the most dangerous part of the coronavirus as it dulls the sense of danger in the population that makes it take notice and prepare countermeasures early enough. India is an example of how this can happen as the sudden rise actually started with a first imperceptible increase in March and early April 2021 that changed into a rapid escalation of the virus in the population by May 2021. Restrictions on travel from Portugal are being put in place in UK as a precaution. Daily coronavirus cases dropped to the lowest level since March 30, 2021. Cases on June 19 were at 58,000 and deaths at 1154 for India's population of 1.2 billion people. The only state with cases over 10,000 was Kerala, and close to 9000 was Tamilnadu. Karnataka and Andhra Pradesh were at over 5000. All these states are in southern India. Only Maharashtra with about 9000 cases was in northern India. The positivity rate in Maharashtra state was 3% and in Mumbai 2%.  The Indian government has a clear vaccine policy and it is for vaccine supplies and vaccination drive to be under the federal government. This puts responsibility in one place and makes it possible to achieve the target of vaccinating 1.2 billion people by December 31, 2021, with the federal government putting all the resources it can muster into the effort. The economy is also linked to how the vaccination supply effort and vaccination drive progresses in the next 6 months, so that the goal of vaccination is closely linked to economic recovery and progress for India as a whole. The lack of vaccine supplies in Africa and Latin America, parts of Asia, is a major problem in 2021. Of the 66 million doses of vaccine planned to be given to Africa under COVAX plan only 19 million have been delivered. In total about 49 million doses have been delivered. Vaccine shortages are a result of the huge wave of coronavirus in India in April, so that vaccine shipments from India have stalled. Of the countries in Africa a few have made some progress- Egypt, Ghana, Nigeria. Other problems in Africa are lack of trained people to give vaccination. Last week 2.3 billion dollars in additional funds were raised at a donor conference for COVAX, the initiative for poor countries vaccines. That is enough to buy 1.8 billion doses. US and UK have not exported vaccines. India has made a good start in shipping vaccines to many countries in Africa, Asia and Latin America as shown in the Ministry of External Affairs website of the Indian government. On the first day of the new vaccine policy on June 21, 2021, India has vaccinated 6.9 million people. India has now vaccinated 287 million people out of a population of 1.2 billion. This is a race against time as new variants caused the second wave of coronavirus in April and May of 2021 with cases peaking at over 300,000 a day.  The shortcoming of the old vaccine policy are being corrected. The entire vaccine supply process and the vaccination drive is now being handled by the federal government. Earlier during the second wave vaccine supply and the vaccination drives were under an arrangement with no clear overall responsibility. States shared responsibility with the federal government and target vaccination goals were missed, vaccine supplies were inadequate.  A similar arrangement in Germany failed and Germany's vaccination supplies were inadequate and vaccination drive stalled. This caused immense frustration in Germany in April-May 2021. Germany's troubled history before World War II led to a reliance on decentralized actions, and state governments imposed different rules in a relatively small country compared to India. This was corrected with the federal government taking on the entire responsibility for the vaccine supply and vaccination drive leading to good results today in vaccines. With India's huge population and political process of different state governments, some lacking experience in administration for a complex process, and others failing to coordinate well with the federal government, the lack of overall responsibility at the federal government posed serious risks of missing targets for vaccines and letting the coronavirus wreck the economy and public confidence. Complex negotiations with other governments in Europe and the US for vaccine manufacture in India could only be handled at the federal level. The resources and planning at the federal level were already in place in India for infrastructure and other projects, experience and setting targets in that area at the federal level could now be transferred to this task in vaccines. Somewhere in the range of 8 million vaccines a day need to be reached and sustained from August to December 2021 for India to reach the goal of vaccinated all 1.2 billion people ahead of any further attack from a third or fourth wave, say experts. This is not a choice for the federal government, it is simply something India has got to accomplish to be a healthy nation that can grow with neighbors in Europe, the US, Australia and Japan and build confidence in its Asia-Pacific region. The vaccination will start with health care workers, moving on to essential service personnel in the military, police and other occupations, and to vulnerable parts of the population based on age and health conditions. Vardhan who is also the chair of the executive board of the WHO as India's representative, says the prime minister is personally holding two 3 hour long meetings to monitor the preparations for the vaccine and its distribution. Vardhan lists the achievements of the Modi administration and the quality of leadership provided by the prime minister- 2100 testing labs, 97% of the country having a testing facility within 3 kms, testing 1-1.5 million people each day, 1 million testing kits produced daily, 2 million beds in India with oxygen support or in ICU, 13,000 quarantine centers. By personally visiting the vaccine development facilities in Ahmedabad, Pune and Hyderabad, the prime minister also directly supported and encouraged scientists and their efforts. ...

After reading this editorial in the WSJ about the origins of the coronavirus, readers will have more questions about EcoHealth Alliance nonprofit which sent $3.4 million to the Wuhan Institute of Virology (WIV) for  research with potential for risky consequences to world public health if something went wrong.  What is gain of function research and what purpose is served in making virus more infectious and contagious such that the National Institutes of Health would send funding overseas for such research are questions that readers may have. Were all the risks considered for such research overseas. The editorial says the US Congress is not showing interest in the origins and the US president Biden has to take up this- some readers might have a question what would make the US Congress not interested in this when coronavirus has affected the US, India, Brazil, Europe, and the poorest countries on earth in unimaginable ways. Can such important matters as world public health fall under the authority of one person, such as Dr. Fauci and other health officials from one country or two countries, when other countries such as India, Brazil, UK, France, Italy, Germany, EU countries, are gravely affected. Global public health has suffered badly and billions of people around the world, in the poorest countries as well as the people in US and Europe have suffered unimaginably in 2020 and 2021 as a consequence of poor decisions. Much needs to be done to get the people themselves informed about public health and how it can be shaped in the future. ...
