Obama Affordable Care Act ACA and its downfall are covered by two experienced authors over 2 book written over 2 decades. The authors are James Morone and David Blumenthal followed the healthcare issue over 25-30 years through the Clinton, Edward Kennedy and Obama efforts and wrote two books. The first was "The Heart of Power" on the healthcare situation from FDR to 2008. The last titled "Whiplash" in 2026, for which the authors are interviewed in Harvard Medicine magazine. C-SPAN has a book program on this book at a Washington DC bookstore. From the discussion on C-SPAN between Senator Michael Blumenthal, borther of one of the authors, James Morone and David Blumenthal physician, couple of conclusions are seen that may be new to readers. Q. What was the one single factor that doomed the Affordable Care Act? A. The deep antipathy towards the Obama administration influenced the response to the Obama handling of healthcare. The likelihood of Republicans accepting healthcare from a black person was simply not there say the book's authors in the discussion and Q&A on C-SPAN. Yet there were other reasons for the ACA failing. Obama had not gauged the mood of the nation well. UK Labour's Starmer won by a big majority in 2024 yet that does not reflect the mood of the British nation just 2 years later- by election year 2012 Obama's campaign was faltering and had to be rescued with Hispanic votes and a weak candidate in Utah's Mitt Romney. Obama lacked maturity and came in the way Bush came in when the list of candidates were mediocre in the US, similar to the period in the UK with David Cameron and Boris Johnson. To take on the health care issue required someone with the experience and caliber of LBJ, which Obama clearly lacked, coming from the minority community was not going to help in credibility. Obama's presidency was thus premature and to gain experience he would have done better in a key cabinet position such as at Department of State where an intellectual could have influenced world opinion in favor of emerging countries, a doable and necessary. Obama's lack of experience showed when he told Republicans two words in the first months in 2008- "We Won," perceiving arrogance it would set Republicans against him. The years 2008-2016 cost the US dearly in that the US needed a withdrawal from all of the Middle East which would require a strong president  with deep roots of support in all parts of the country including the south, to avoid recriminations. In the end by continuing the wars Obama weakened the US and let China move ahead. Q. Did Obama consider Medicare for All? A. Obama told Congressmen of his party according to Morone- if you can get 60 votes in the Senate for Medicare for All we can try.  Q. Would it take a major upheaval for Medicare for All to be accepted now that the health system is failing all Americans in 2026? A. It will take a world war or a economic depression- some major disaster for Medicare for All to be accepted in the US, say the authors. A pandemic happened in 1918 and again in 2019 the results were not positive, as the authors believe it unleashed the war on science after the vaccination for and against camps, leading to the culture wars in America seen today. Q  Obama's analytical mind thought he learned from the Clinton efforts in healthcare that failed. But he did not see things from the heart. There is good reason to think that the lessons learned of moving fast, letting Congress write the legislation, settling for what can be done not what needs to be done, were exactly the wrong lessons to be learned as opposed to writing off the Clinton experience entirely as Clinton's, and starting from scratch without preconceptions. In the end Obama if he was older, had more experience, and listened to the mood of the country would have realized that healthcare was for another day, and got right down to the most difficult challenge, to end the wars in the Middle East. Even small steps in the right direction would still have earned appreciation him today. Instead Bush and Obama, the most inexperienced of presidents will be remembered for wars they continued that weakened America.       ...

The Nordic Prime Ministers reiterated the support of the Nordic countries for permanent membership for India in a reformed and expanded UN Security Council. The Nordic leaders welcomed India’s application to the Nuclear Suppliers’ Group (NSG), and emphasized the importance of the international multilateral export control regimes in upholding non-proliferation and international peace and security. Trade, investment and economic cooperation 8.  The leaders emphasised the importance of a continued central role for the World Trade Organization in the multilateral trading system and global trade governance. Analysis of its findings in the WSJ story on August 18 shows the FDA unnecessarily delayed private labs from developing and using their own tests from Feb 9 when FDA test for coronavirus failed for its third component. The FDA said it would correct the flaws but repeatedly failed to do so until it finally allowed private labs to go ahead on their own- a costly delay of 3 weeks that made the test and contact trace strategy inoperable, because the time window was lost in those 3 critical weeks. In March through August the pandemic has now taken up about 5 million cases in the U.S. and 170,000 deaths, with no end in sight. During times like these and in a swiftly moving current of a river such as the time of a pandemic, the  teaching hospital labs and labs with resources and scientific reputation with their lightning speed have to have the freedom  to immediately respond. In this case the FDA should have released the private labs of teaching hospitals and the the highly reputable labs of well known medical companies to immediately start developing their own tests and using them, starting  on Feb. 10 the day after it was evident that the FDA test's third component was not working. ...

The drugmakers continue their opposition even if this means lower prices for the American people of excessively higher priced American drugs compared to other countries. Both Republicans and Democrats agree prices have to come down and support importing of drugs into the US, then why is it taking so long for the FDA  to approve state proposals? The 22 million ACA subsidies will cost $350 billion over 10 years. Democrats have the government shutdown over this issue of extending Obama ACA subsidies where enrolment increased in the covid and Biden years with generous subsidies. The Washington Post looks at how we got here since 1945, decisions made about employer insurance plans that created a patchwork of plans from private sector and other plans outside it with perverse incentives and inefficient subsidies. Under the One Big Beautiful Act the policy of subsidies for ACA is not being renewed when it expires in 2025. This shows the band aid approach of Obama to healthcare and the lack of a comprehensive approach. The policy on migrants during the Obama and Biden administration also stretched public funding resources. Insurance companies now plant to make up for the los of subsidies from the government by raising prices for this subpopulation in a broken healthcare system in the US by 15-20%. This report in WSJ shows a young woman on ACA insurance in Illinois with a payment of $590 a month to Blue Cross of Illinois facing a new payment of $678, almost the size of a mortgage. ...

The fight against cancer cannot be done only on the finding a cure,  it is also about returning to the time when ingredients such as titanium dioxide and others were not added to food and when people did not consume processed foods. This report says the FDA spokesperson says contact the manufacturer. After the EU has banned titanium dioxide for causing the inflammation that causes cancer, the FDA should have banned it. See the recent article in The Washington Post on the microplastics that release nanoparticles that cause cancer. The US government shutdown ends with Republicans + 8 Democrats in Senate voting to end shutdown on November 10, 2025. This keeps the filibuster voting rule which requires 60 votes to pass. Support of some Democrats was essential to make this happen. After bipartisan negotiations 7 Democratic Senators and Angus King Independent from Maine split with their party leaders Schumer and Jeffries of New York who called for a 1 year extension of Obama ACA healthcare subsidies which the Republicans opposed.  Democrats ensured the funding for SNAP benefits continues to Sept 2026 and the 4000 federal layoffs would be reversed and prevent future layoffs in the federal workforce. Democratic Senators voting with Republicans were Cortez Masto and Jacky Rosen of Nevada, Jeanne Shaheen and Maggie Hassan of New Hampshire. Tim Kaine of Virginia and Dick Durbin of Illinois joined the Senators from New Hampshire and Nevada. John Fetterman who has voted independently of the Democrat party to meet views of Pennsylvania constituents supported the move. This splits New Hampshire, Maine, Nevada, Illinois and Pennsylvania from the Schumer-Jeffries leadership from New York. Tim Kaine voted with Republicans by getting them to agree to reverse federal workforce layoffs, get back pay and stop layoffs. King, Hassan and Shaheen formed the core of Democrats who believed there was no chance Obama ACA subsidies would be extended for another year as long as shutdown continued whereas there was some chance after it ended. Both Senators from Nevada Rosen and Masto were following the needs of their constituents, and so was Fetterman of Pennsylvania. ...

Ryan Tracy and Anthony DeBarros try to address the question of patchy internet service for America's heartland, rural areas from the prairies of Iowa to the west, and in the south and southeast. Public funds were allocated through the Rural Digital Opportunity Fund for broadband service with the latest optic fiber technologies in 750,000 census blocks in all states except Alaska in the US. This was supposed to bring digital internet with fast speeds enjoyed by urban users to every American home. Instead after this and another program the Connect America program why is internet service serving some customers and not others in rural areas, with patches of areas in each rural part of a state without internet service at the speeds one should expect for streaming and other uses? This WSJ research looks at data and conducted interviews on this important issue and found that internet service providers were given public funds by the FCC yet allowed to pick customers leaving some customers out. FCC rules till 2021 did not require service for all customers equally as long as they provided service to a minimum number of locations statewide say former senior FCC officials. One senior former FFC official says it is not surprising that companies made the decision to do the bare minimum required.  In Heavener, Oklahoma this meant that during the pandemic and lockdown when schools were closed the lack of good internet service affected learning from home. Many students could not get online from home. In 2021 another effort was made. This time funds will not go through the FCC but through the states. The Biden $1 trillion infrastructure spending for workers and families includes $42.5 billion for a rural broadband program in America. This WSJ report does useful service to America by putting the spotlight on one of the issues that divides America today the gap between the quality of life in rural vs. more affluent areas of urban America. It also shows that it is the federal bureaucracy that is at fault in this case for poor internet service in rural areas. Careful attention to this is needed so that rural America gets the attention it deserves from the prairies of Iowa to the mountains, the breadbasket of the country, and the heartland.   ...

The Republican House Health Care pLan in 2017 relies on tax credits of between $2000 and $4000 based on age. Under the Affordable Health Care Act the elderly poor in high cost insurance areas received additional help. These people would lose over $2000 per person and may forgo full coverage or coverage entirely under the Republican House Health Care Plan. A report by Standard & Poor's estimates about 2 to 4 million people who are in 50's and 60's not yet qualifying for Medicare might lose their coverage they now have under ACA. The Republican plan also gives incentives through tax credits higher for older people, $4000 for a 60 year ol and $2000 for a 25 year old. Under the ACA the insurers are not allowed to charge more than three times what is charged for younger people, under the Republican plan this goes up to five times.

Why are home covid test kits harder to find in the US compared to Europe and other places? Part of the reason is that the FDA was more stringent in approving home tests than in Europe. During the focus on vaccination in mid 2021 there was less demand for home tests so that Abbott and other makers of home tests actually cut back on production. European governments continued buying them in bulk to distribute them free as the tests were required to enter restaurants and public transport in Europe. With Omicron in December 2021 pharmacies in the US quickly ran out of tests and prices were high at $25 for test kits.

Astra Zeneca vaccine effectiveness gets a new boost from the late stage trial results for US, Chile and Peru that has 32,000 volunteers. The study shows Astra Zeneca vaccine 100% effective to prevent hospitalization and deaths, and 79% effective to prevent symptomatic illness. The vaccine is also known as the Oxford vaccine because it was developed at labs in Oxford University, England.

The vaccine is now preparing for US FDA regulatory approval. Its worldwide use will give new hope to the world's population because it is being given at cost and can be stored in ordinary refrigerators for long periods. Conditions that give it wide access in poor countries. It is also manufactured in India by The Serum Institute, one of the largest vaccine manufacturing labs in the world, which would make it possible to make the billions of doses needed.

The US Senate filibuster rule that requires 60 votes for the budget to pass is a relic of the civil rights days, today it prevents Republicans from ending the shutdown by majority vote. Republicans used it when Democrats controlled the House and Senate and Senate Majority Leader Thune does not want to drop the filibuster as it acts to ensure no hasty legislation or budget is passed without some form of overall consensus. It also means that situations such as a government shutdown can happen when the opposition party in the Senate uses it to make it demands be met. In this case Democrats Schumer and Jeffries are asking Republicans to include money for subsidies for the Obama Affordable Care Act enrollees- about 22 million people signed up for this part of the ACA to receive generous subsidies for healthcare. These subsidies are not part of the Big Beautiful Bill of DJT that passed Congress and was signed into law. Unions and airline executives are calling on Democrats to end the shutdown as it disrupts air traffic at airports, and leads to cuts in SNAP and other benefits, and to lack of pay checks for government employees. ...

This important WSJ report shows how the CDC labs failed to come with the test after contamination of a component and failure of the third part of the three part test components. In the absence of a working test from CDC where delays cost three weeks of February the private labs all over the country and state labs could have developed their own test, yet HHS and FDA required approval for these labs to develop their own test and use it. From Feb. 8 when state and city public health labs detected problems with the CDC test to Feb 29 when solutions were developed including simply excluding the third part of the three component test, and letting labs around the country do their own test, were 3 crucial weeks that let the virus spread out of control. The CDC, HHHS, and the FDA and their managers and heads of departments bear responsibility for these errors. Week after week the delays continued, instead the two component test which detects if the genetic material in the sample is coronavirus material, could simply have been approved at the outset instead of the approval for this given weeks later. The third component of the test checks if the virus mutated, according to this report. There is no explanation why the labs all over the U.S. were not allowed to go ahead on February 9 itself or within a few days after that to develop their own tests once it was clear the CDC test did not work on that day. CDC officials failed to recognize that there was a possibility that they may not be able to fix the faulty third component of the test and the risks if they gave false reassurances. There is also no explanation of why a German designed test was not used once the CDC test failed on Feb. 9, which would be a proper way for action considering that this pandemic had already shutdown parts of China by this time. Alarming also is the mention in this report that on Feb. 22 a FDA official in charge of lab diagnostics  flies to Atlanta where the CDC Respiratory Diagnostic lab which developed the test is located. His boss FDA medical device center director is cited from later information as describing the lab as "filthy" meaning the lab had the potential to contaminate, and going so far as saying that if it had been any other lab it would have been shut down. Considering that investment in public health has deteriorated over the last two decades and that there has been a massive misallocation of capital in the country away from public infrastructure this is appalling. The thought of critical labs for emergency health needs as not being up to high quality standards in the U.S. as a result of two decades of misspending, that this shows, is very disturbing.  This WSJ report is based on interviews with people who know about the testing crisis, and undisclosed emails, correspondence on the issues involved. Community transmission began in January 2020 in the U.S. These delays were costly in February and could have been prevented either by going with the German design on Feb 10 or asking labs across the U.S. to develop their own test, and letting other labs immediately use the modified 2 component test of CDC that worked instead of doing this action weeks later. Mardi Gras on Feb 25 and other places where large crowds gathered in sports stadiums could have been stopped had testing gone forward and shown the true extent of the community transmission in these critical weeks.  First China delayed a U.S. team of experts coming into the country for weeks, and then the CDC, FDA, HHS, failed to get testing started, creating  a false sense of complacency. Two crucial errors outside and inside the country that caused so much damage to America and the world.   ...

This article in the NYT provides a look at the features of the Republican House Health Care Plan- Both the Affordable Health Care Act and the House Plan provide incentives for buying insurance- the ACA bases these incentives on income levels whereas the House Plan does not provide additional help for low incomes or elderly. Incomes at $20,000 would see a loss greater than  $2000 under the House Plan and as many of the elderly poor living in high cost areas may not have the resources to make up for this loss of subisidies they may forgo buying insurance or have insurance coverage that protects only in a limited way. President Trump has given assurances that all will be covered. For people with incomes of $50,000 or $75000 the loss of $2000 subisidies would also have some impact. At larger incomes or the well to do the subsidies are not handed out under either plan. Under the ACA the emphasis was on income levels and high cost insurance areas the subsidies were greater, under the House Plan the subisidies would be higher for the elderly compared to the young but very low income levels are not given additional help.     ...

Coronavirus testing is taking off in the U.S. for the first time after the U.S. Federal Drug Administration relaxed testing regulations on February 29, and as more universities and hospitals develop their own testing technology and apply it. The FDA has approved more than 15 tests and many more are awaiting approval. 

This includes UC Davis, Washington University School of Medicine, Brigham and Women's, Harvard Medical School, Mount Sinai Medical Center in New York City, centers that are running the tests limited only by the shortage of reagents supplies.
