France and Italy are imposing strict rules for their lockdown and tougher enforcement to control spread of coronavirus. India has imposed a quarantine, a total ban on leaving the home with a televised address by prime minister Modi that set a 21 day lockdown period. Modi said "if the pandemic cannot be managed in 21 days it would set India back by 21 years." 

Fines of $3000 euros are imposed in Italy and cars seized by the police. A special form has to filled out in Italy showing where one is going and checked by police if leaving the home. In France one needs an Affidavit of Movement by Special Dispensation is a filled form required for everyone outside the home. All open air markets are closed in France and drones are used to warn people outside.

All domestic and international flights in India are stopped. All domestic passenger trains and bus services in India are also stopped.

With 3.7 million workers in the informal economy Italy is one of the worst hit European countries. Italy's south, including Naples and its capital Campania is one of the hardest hit. Italy's lockdown ended May 18, with some restrictions. Affected worst are small business owners such as shopkeepers, restaurant owners and market vendors, also hit are workers employed in tourism and entertainment. The Italian government has made a 600 euro emergency payment to self employed or part time workers, and 12 million workers have applied so far for these payments, about half of the workforce. A new payment by the government will cover workers in the informal economy with a55 million euro additional aid package by the government of prime minister Conte. Italy's economy will decline by 9.5% in 2020, exceeded in Europe only by Greece. The country is seeing a further erosion of the lower middle class after the difficult period following both the financial crisis of 2008, the eurozone crisis, austerity cuts which hurt people across southern European countries, Spain, Portugal, Greece, and Italy. It is also true that Italians came together during this difficult period in a way not seen since World War II and prime minister Conte provided much needed leadership for Italy, with growing confidence in his leadership. This provides a new sense of hope that Italy can come to grips with many problems it has faced in the last 2 decades, similar to that in other parts of Europe where investment in  infrastructure and manufacturing has fallen behind. ...

Italy is planning to place a quarantine in the north covering the region around Milan and 11 provinces nearby, in a draft decree from the prime minister's office, says this report in the WSJ. When a final decision is made the quarantine would go into effect March 8 and go on till April 3, 2020. This move is similar to the quarantine steps taken in China which helped limit the health crisis in China from coronavirus. The new decree also includes tight restrictions on travel in the affected areas. 

The new action is being taken as earlier measures have not limited the spread of coronavirus. They were requested by the government in Lombardy, which combined with Emilia Romagna and Veneto are the worst hit of the regions in northern Italy with about 85% of cases. It also means the industrial economy with 40% of GDP is affected.

The Paris lockdown is unique and effective because people are required to carry a signed document showing where they are going and show it to police when going outside the home. About 100,000 police and gendarmes are enforcing the lockdown in France. The lockdowns in the U.S and some other countries lack this kind of enforcement. France implemented the lockdown March 16, after Italy acted late on March 10 with the virus already spreading there. Once this was done the lockdown was done with tight restrictions and enforcement by police unlike other places. In Asia India has imposed a strict lockdown.

It took a week longer for each country to impose a lockdown. In China first Wuhan then the whole country went into lockdown and quarantine. The same process is repeated in Europe and in America as authorites see numbers of infections increasing rapidly without strict controls. First the Lombardy region in Italy around Milan, then the provinces in Northern Italy, followed by a complete lockdown in the country on March 10 as infection spread faster without lockdown and enforcement of lockdowns. Germany and Britain follow Spain and Italy on March 20. France followed Spain in the days after Italy's complete lockdown. Macron ordered the lockdown on March 16 with stringent enforcement. Infectious Disease specialists at Imperial College warned of "unintended consequences for the entire nation" if a lockdown of Britain did not take place. The goal is to limit the spread of infections from rapid to slow as public health systems and economic measures are ramped up in preparation for the crisis. Most countries were lacking the preparatory steps having lost time waiting to see what happens next or analyzing data in the vain hope the virus does not spread.  Bad economic results of lockdowns were initially a concern, but this concern became less important as the coronavirus spread rapidly in Europe. Decision makers in Europe decided that not acting forcefully would lead to equally or worse economic outcomes. Public health systems overwhelmed would diminish public confidence rapidly and lead to equally bad or much worse economic outcomes. The European Union executive body has supported state aid, stimulus action and border controls in this crisis. In America and in Europe the hope is that shoring up the safety net with massive aid to businesses and households would buy time to tackle and overcome the coronavirus through a combination of lockdowns, quarantines, contact tracing, large scale testing and medical technology measures. Governments such as New York and France, UK, Germany are including this key variable in their determination of how long a lockdown lasts, and for determining if the reopening is not going the right way or failing. In such situations the lockdown would be reinstated, or if it is a phased reopening such as in the U.S. and other countries go back to the previous phase. In Italy and Germany the RO reproduction ratio for coronavirus is estimated by official experts at 0.8. Germany's RO estimated by the Robert Koch Institute and Italy's by Franco Locatelli, scientific advisor to the government. In New York the margin is thin- with RO of 0.9, estimate from the state's governor. In France which has one of the tightest lockdowns of all with a document required to go outside it is at 0.6, the figure coming from the prime minister Mr. Philippe. In the UK it is below 1.0 but no accurate figure is reported. As Dr. Birx- leading the coordinated response in the U.S. - emphasizes over and over again this is a very contagious virus, about which not much is known. Social distancing, wearing masks, basic prevention measures such as frequent handwashing, and not gathering in large numbers of people, is essential for defeating this virus. About half of American states went on lockdown including the largest New York and California, covering about 100 million people. Here is a state by state account of the measures taken by each state to limit movement outside the home to essential activity such as getting groceries from stores and medicine from pharmacies. In German states there is a fine of 2500 euros for violations and similar fines in other countries. Italy is paying a heavy price for the delay in implementing a complete lockdown and other measures. The first delay was on Day 6 and Day 12 after the first coronavirus infection on Feb. 20, when the regional authorites in Milan region kept bars and cafes open and reopened Milan Cathedral to tourists as reported here in the NYT. This was happening even as the pandemic was surging in a case of ample denial. Then again on the 18th day after the detection of the first case on March 8, the lockdown announced was leaked leading to people leaving Milan and other towns for the south and other parts of Italy. This lockdown of March 8 was not specific enough and lacked details so that it was not properly implemented immediately. The final lockdown of the entire country came as the situation worsened on March 10. As a result the virus continued to spread leaving hospitals and health personnel overburdened and the country in severe crisis. The Veneto region fared better than the Lombardy region because of strict quarantines, higher rate of testing and contact tracing to enforce isolation of clusters. The lessons from Italy, the epicenter now of the coronavirus pandemic, are clear- act early and decisively, impose quarantines and lockdown quickly to prevent intermingling. These steps need to be put early, with absolute clarity, and strictly enforced. This report in the NYT shows how decisive action early is the only way to prevent the crisis from becoming a large one that threatens the health system and millions. German states such as Bavaria and others now have a $25,000 euro and some have jail time. Singapore has jail time and fines for violating the stay home quarantine orders which it imposes after contact tracing. Taiwan has similar measures. Enforcement and strict control are part of the tools that have worked in these places and in China, South Korea.  Britain has still to close flights from Iran, Italy, which The Times reports on March 20 are still landing at Heathrow. This report in DW.com looks at the response of Germany to the coronavirus epidemic and says Germany may have let a window of opportunity to act quickly slip from its grasp. It says Germany's leader Merkel has not shown the leadership required by the health crisis. Germany DW.com points out recorded its first case on January 27, yet Merkel's first press conference on the subject of coronavirus came on March 11, when Italy was on lockdown quarantine for 2 days.  Germany lacks a quarantine and effective government action to mandate and require social distancing across the country to limit the spread. The steps this report points out pale in comparison with the actions taken in other neighboring countries. Spain earlier and Belgium on March 17th joined a lockdown in Italy. Merkel called on Germans to stay home, yet enforcement is lacking.  In this situation the calm and reacting with reason may be obsolete, a proactive approach being the right one. And a braver one because it would anticipate what happens a week two weeks from now based on experience of China and Italy, and act quickly with a lockdown and quarantine to prevent spread. Andy Haldane the chief economist of the Bank of England says roughly half of the 25% lost output in the economy in the lockdown in April and May has been gained back in UK. This is good news for Europe after the bad hit it took from the pandemic since March. The contrast between lack of effective measures taken in the Lombardy region with the aggressive action in Veneto that has proved effective. Veneto followed the method of quarantining, mass testing for clusters and isolating the affected people.  The Italian government took the first steps to close off northern Italy Feb 22, and it was not till March 10 that a nationwide lockdown was done. The action taken in the Veneto region is shown here in this WSJ report with the town of Vo as an example of steps taken that worked. A microbiology professor and infectious diseases expert at the University of Padua, Dr Crisanti, developed a test for the coronavirus as early as mid-January using the information made public by Chinese doctors. Dr. Crisanti oversaw the testing of 95% of residents of Vo, a town of 3400 people in Veneto region. He found 3% of the population was infected, with half testing positive asymptomatic. Following the aggressive lockdown the tests were done two weeks later and the rate of infection had fallen to 0.1% with only 8 new infections. "The main lesson from VO is that when you have a cluster of infected people, you should do a very aggressive lockdown and then test as many people as possible," Dr Crisanti says. The results from Vo led to Veneto increasing testing in the rest of the region carrying out 80,000 tests, compared to 88,000 in Lombardy, with double the population and 5 times more infections. Lombardy followed government directives to test only those with symptoms. When it spreads it is harder to do the test isolate clusters, test isolate clusters, in a continual loop, yet this remains the method cited by Dr. Brx in the U.S. today as the right way to target clusters in a laser approach. In yesterday's briefing at the White House Dr Brx said this is a method the U.S. is familiar with and has used in Africa to tackle HIV, Ebola Virus. It is possible using GPS to target down to a specific clinic in a specific place, which is how it was successfully done in Africa. ...

The low number of confirmed cases and small number of deaths in India and Pakistan is being analyzed for what to expect in the coming months from June to August. Some experts see this as just the beginning that the peak will come later on. India has about 75,000 cases and Pakistan 35,000. Testing has lagged behind with only 1.5 tests per thousand people compared to 26 per thousand in the U.S.  one reason the confirmed case count is lower. Some confidence is being drawn from the deaths- 2415 in India and 737 in Pakistan. This is because only 2.2% of cases in India and 3.3% in Pakistan led to deaths, compared to 14.3% in UK and 5.5% in China, 6% in U.S., using John Hopkins database. One reason given is that only 6.4% of India's population and 4.3% of Pakistan's is over 65 years age. Compare mean age in Italy 46 years to 27 in India and 23 in Pakistan. And there is plenty of sunlight which appear to destroy the virus. Other factors that may influence the virus- taking of the tuberculosis vaccine and routine exposure to more pathogens in both countries. Prime minister Modi in India is taking no chances considering the size of India's population. He has put forward a $280 billion economic package and is moving in deliberate carefully prepared steps to lift lockdowns with the current phase No. 4 allowing more reopening. The shift now is to a more self reliant economy in industrial production making "local more vocal."  During the lockdown the large rail network and postal networks with their millions of employees, IT technology driven banking with Aadhar identification for direct deposit to hundreds of millions of the most needy citizens, and farmers, proved to be the most reliable and supportive. ...

Bars and restaurants filled with people just increases the risk. Consider that on one day June 20, 500,000 people went to bars in Los Angeles county the day after they reopened, as reported in the WSJ. This is similar to what happened in soccer stadiums in Lombardy, Italy, spreading the virus like wildfire. Experts say social distancing is easier to do in office locations and at work, than at bars, restaurants inside, and at soccer stadiums, or large gatherings of any sort. In just one situation 138 new cases were traced to a bar in East Lansing. Michigan.

Most of the restlessness about the lockdown was about not being able to get to work after weeks inside. And getting outside to a park for exercize was always safer because it was easier to keep social distancing in these places.

Germany Economy Minister Peter Altmaier says Germany expects a shallower recession. GDP in 2020  is expected to be down by 5.8% much lower than the 10-15% in other countries. Exports in June were up by 15% to China and down by 20% to the U.S. Economies of Spain and the UK are expected to see twice the decline in GDP in 2020. Italy and Germany are seeing a increase in manufacturing output, Spain and France a decline. 

Still Germany remains exposed to other trading partners than China, such as the U.S. and Britain, total exports are expected to be down 12% in 2020. About 11% of workers are using short term work subsidies to stay at home. Cases of the virus are surging in France and Spain. In Germany there is a surge but it is slowing since last week. Mr. Altmaier thinks Germany can avoid a second lockdown.
