For generations schools required vaccination proof- by 1980 all 50 states had laws covering students first entering school. And caught in vaccine politics legislatures are creating religious and other exemptions that have weakened laws. Wash. Post says it's examination shows not a single County in Idaho, Louisiana, Oregon Tennessee, Utah, and Wisconsin meet the 50% vaccinated requirement for measles required for herd immunity. This could mean more of these diseases will be brought back home including whooping cough to affect elderly and infants. Democratic districts such as in St Louis and Chicago also see drop in measles vaccination rates. In the sense that newly decolonized countries since 1950 such as China and India have emerged with good health systems and mandated vaccination , other public health action, there is a great need for the US to focus on bringing back the public awareness that existed after the 1940's in the US that resulted in significant advances in public health in the US in the FDR, Eisenhower, Kennedy, LBJ and Reagan administrations. It shows there is no victory in public health. A lot of work needs to be done, as much of the gains can get undone by events and public awareness is necessary. As pharmaceuticals, chemicals and plastics and bad nutritional habits took over American lives there is an effort under Kennedy at HHS to tackle that health crisis, but it brings with it challenges that date from the pandemic and different responses in different parts of the US to mandatory vaccination which also have to be met through education not social media. ...

Germany's national broadcaster DW.com looks into the details of the vaccination drive in countries around the world, including Germany, and finds that there is more to the story than meets the eye in headlines about safety in vaccinations. Many headlines do not tell the whole story carefully. DW revewed reports from Italy, Austria, South Korea, Germany, Spain, the USA, Norway, Belgium and Peru, and found that in most cases health authorites have not found causal links between the vaccination and deaths.  As of March 15 it says 360 million people have been vaccinated in 120 countries, or about 9.25 million a day.  DW.com cites the Pharmacovigilance Risk Assessment Committee (PRAC) of the European Medicines Agency (EMA) which states: 'there is no indication that vaccination has caused these conditions, which are not listed as side effects with this vaccine. Information available so far indicates that the number of thromboembolic events in vaccinated people is no higher than that seen in the general population." As of March 10, 2021 30 cases of blood clots are recorded for 5 million people vaccinated with Astra Zeneca vaccine in the European Economic Area.  The Paul Ehrlich Institute which is in charge of vaccination in Germany has looked into 113 reported deaths in 46 years to 100 years old patients in Germany. Of these 113 deaths PEI finds that 20 died of the Covid 19 infection as it takes 14 days after the second dose for full protection, and 43 died of pre-existing conditions or other infections. For the patient population it says "they were seriously ill patients with many underlying diseases." PEI says "based on the data we have we assume they died of their underlying disease- in a coincidental time with the vaccination." A virologist at the Technical University of Munich, says that the deaths after vaccination are below the expected number of deaths without the vaccination.  ...

With vaccine hesitancy in the US the goal of vaccinating a large part of the US population is still elusive. Less than half of the US population is fully vaccinated as of July 30. The vaccination rates in the US are now much slower than India even taking into account the population difference. The vaccination in India is at the rate of 4 to 5 million a day for the past week in India. In the US this report in The Guardian shows 4.7 million vaccinated in the past 2 weeks. On July 30 856,000 people were vaccinated in the US compared to over 5 million in India.  Newspaper reporting that encourages vaccine hesitancy people to delay getting vaccinated because of poorly reported headlines is adding to vaccine hesitancy in the US. This has led to much frustration at the White House. The threat of the Delta variant is making more people to take vaccination. Southern states are seeing a jump in vaccinations. Many prominent Republicans are now endorsing vaccination.  One of the problems in tackling the Delta variant is the confusing nature of the CDC guidance on masks. On May 13 CDC issued guidance saying masks were not necessary indoors and outdoors. Now 3 months later in July the CDC says the threat of Delta variant means mask wearing is essential particularly indoors because of the higher transmissibility of the delta variant.  ...

From now on the vaccination program in India will be run nationally by the federal government. This will ensure fair access to vaccines to all parts of the Indian population. Earlier vaccine costs were bid up as states and private hospitals bid up prices. Under the new national program 75% of vaccines will be given out by the federal government and 25%  by private hospitals and other private health institutions. The government in New Delhi under prime minister Modi will offer adults free vaccinations. Modi said "We will increase the speed of procuring vaccines and also increase the pace of the vaccination program." Even in private hospitals the cost of vaccine will be kept at Rupees 150 or $2.06. Experts say this is the right policy and the government has learned from errors in letting states and other private institutions run vaccine policy, which made it too fragmented and subject to too many variables, resulting in inequity, and slowing vaccination drives. The Supreme Court stepped in asking for clarity, leading to the clear policy from the federal government announced today.  Advantage of the new policy is that the responsibility lies in one place, and the federal government also has the clout to make things happen, to negotiate with companies and other parties involved effectively. India has vaccinated 222 million people but because of the population being so large at 1.2 billion this comes out to be a small fraction of the population. This puts the task of getting vaccine supplies and getting the vaccination drives to work in the only place that has the determination and the resources to deliver results by vaccinating 1.2 billion people by December 2021. It has never been done before in history says Mr. Modi, and it is a challenge that India is now taking up for itself and for the global community. It also lays the ground for India to help its neighbors in Asia and in Africa, Latin America in 2022.   ...

Already vaccination experts at the Baylor College of Medicine cited here say they foresee another wave of the coronavirus in the Southern US because of vaccine skepticism barriers to vaccination, and much of the population not being vaccinated. The percentage of the population in the Carolinas, Georgia, Georgia and Louisiana, that has both doses of vaccine is low. About 50% have had just one dose. In southern states Mississippi and Alabama the vaccination rates are much lower leaving large parts of the population unvaccinated. The widespread presence of the Delta variant in the UK and the R rate being between 1.00 and 1.2 in UK shows that the UK even with higher vaccination rates than the US is still facing a danger of another wave as it reopens.  The ominous aspect of this is that from just over 400,000 the vaccinations per day dropped to 316,000i n the US. This means there will be a period when vaccination drive is stalled while the reopening is fully underway with a new contagious variant twice as contagious as the original coronavirus. This leaves the US at risk of another wave, with the south and western US, younger people who are out more and unvaccinated, more at risk.  ...

Latin America has made a huge turnaround through successful vaccination drives. Today more people are vaccinated as a percentage of the population in Latin America at 62% than in the US at 56% or Europe at 60%, according to Our World in Data project at Oxford University. There is little resistance to vaccines in Latin America after successful vaccine campaigns against yellow fever and other diseases. During the first year of the pandemic Latin America had one third of the deaths in the world with 8% of the population. Deaths after vaccination drives have dropped to 8%.  Brazil with 617,000 deaths from coronavirus was second only to the US with 800,000 deaths. Brazil is now back to normal after a successful vaccination drive that has 66% of the population fully vaccinated, and 80% with one dose, some of the highest rates in the world, according to Our World in Data at Oxford University. In Colombia with 50 million population about 50% of people are fully vaccinated. Cases have dropped from 30,000 in June to 2000 a day and deaths from 700 daily that month to 50 a day in December 2021. In Buenos Aires, Argentina's capital, 83% of three million population are fully vaccinated, 14% have received a booster. Buenos Aires city health minister says Argentine society has an affinity for vaccination campaigns. "They rapidly accepted receiving them," he says. Yet from the point of view of new variants emerging there is a different situation in rural areas. The UK NHS is urgently tracking down parents of 35,000 children in London who are not fully vaccinated against polio, says this report in The Times. Health officials have detected the first outbreak since 1984. They are trying to trace it back to a "single household or street." London has lower rates of polio vaccination than the rest of England, dropping to levels that are seen as risky for the health of children. Vaccination rates are low in north and east London says this report in The Times. In London as a whole the vaccination rate for one year olds in 2021 is at 87%,  5 percentage points lower than that of the UK as a whole of 92%.  

Lack of investment in public health and services has weakened the public health system even as hundreds of billions of dollars were misallocated by capital markets gone astray. The pandemic exacerbated a bad situation in public health lowering vaccination rates even further.

Hit hard by the pandemic the people of Argentina, Brazil, Chile and Mexico are showing less vaccine hesitancy. This is enabling this region to catch up with the US in vaccinations. The difference is that people trust vaccinations. About two thirds of people in Chile and Uruguay are vaccinated. Supplies of vaccine are coming in from US, UK, Russia and China. Brazil is now doing 1.5 million vaccinations a day, compared to 750,000 in US daily. This report says that on a per capita basis taking into account the populations US is currently vaccinating at half the rate of Mexico and a third of the rate in Peru and Colombia, Argentina. With 8% of the population Latin America had one third the deaths making vaccination urgent and essential for this region. People are taking the threat of Delta variant seriously in the region. Astonishing as it may sound after all the publicity of deaths in Brazil this report in WSJ says 88% of people in Brazil want to get vaccinated, while 67% say this in the US, poll from Ipsos. After a good start the vaccination drive is slowing both in India and the US. Vaccine supplies need to be boosted in India and the government has given approval to the Pfizer, Moderna and J&J vaccines to boost supplies. The Sputnik vaccine from Russia has also gained approval with new supplies of that vaccine expected in India. Vaccine skepticism is a problem in the US and Europe. Supplies of the Pfizer vaccine will be boosted in Europe with the EU contract for 1.8 billion doses signed recently. This should give the slow vaccination drives in France and Germany a boost.

Vice admiral Henrique Gouveia de Melo, is a former submarine commander who has instilled confidence in Portugal's vaccination drive. The drive had a faltering start in January 2021. Gouveia has reminded Portuguese people of the vaccine campaigns that came before- against measles, polio and other deadly diseases. Public memories of these campaigns including the first national vaccination plan in 1965 have helped take the fully vaccinated percentage to 83%, highest in the European Union. Israel and UK are at 61% and 66% fully vaccinated after starting much earlier.  Spain is also close to Portugal in fully vaccinated people. In Portugal the focus remained on protecting people, and vaccine skeptics played a very small role. Portugal used large scale vaccination centers in sports facilities with the help of the military and municipalities.  Gouveia brought with him a team of 30 military strategists, mathematicians and doctors to work with health ministry officials to coordinate a network of 300 vaccination centers, mostly in municipal sports stadiums, with 5000 doctors, nurses and volunteers. 154,000 jabs were given daily. Six European Union states pass Britain, France and Germany in fully vaccinated percentages on August 5, 2021- Belgium, Denmark, Ireland, Spain, Portugal and Malta. The vaccination drives in Britain, France and Germany have now stalled due to the vaccine hesitant groups. In the US southern states vaccination drive never got off to a good start adding to the problem of unvaccinated who are most of the people in new daily cases, leading to a new surge by August 4. France will soon overtake the UK in fully vaccinated percentage, showing that an early start as in UK is not enough. One in three in the UK in the 18-30 years age group has not taken the first shot, which is alarming. The US has a similar problem in the southern states in an aggravated form and also across the US in some form where young people have not taken up vaccination. ...

The Japanese government is trying to work with universities and companies to speed up the vaccination drive. The government provides the Moderna vaccines and the companies and universities provide the facilities and the medical staff.

The Tokyo Olympics open shortly. This makes it critical to get the elderly population vaccinated. Government released data June 8 shows just 3% of elderly over 65 years are fully vaccinated or 1 million people, and only 8.4 million people or 24% have 1 dose. Since then the vaccination drive has stepped up and about 18 million of the elderly have been vaccinated with one dose or 51% of the elderly population, and about 17% have been vaccinated fully by June 26.

The Indian prime minister reviewed progress of the vaccination drive. On July 26, in the last 6 days about 38 million doses of vaccine have been adminstered in India. In a separate affidavit to the Supreme Court the government laid out its plan for making enough vaccine supplies to vaccinate the entire population over 18 by December 31, 2021, over 940 million people requiring over 1880 million doses.  "Out of this requirement for 1880 million doses 516 million doses will be made available by July 31, 2021, in about 34 days. The breakdown of vaccine supplies manufacturing is also given to SC. Of the total of 1350 million vaccine doses the following is how it will be manufactured in India in the next 6 months. From this new vaccine supplies picture it is clear that Covaxin and Bio E Unit will play a major part in the second half of 2021 in vaccinating India's entire population with 2 doses. This would be a historic achievement for the federal government after the lessons learned and the action taken with a new vaccine supplies policy for India in June 2021. There is also the challenge to be overcome of maintaining the vaccination drive at 6-8 million doses a day which would be another historic achievement for the federal and state, and local governments in India working with community education and logistical setup, medical staff.  ...

German Heath Minister Karl Lauterbach says vaccine mandates are the only way out of this pandemic. Speaking to German daily Welt he said, gaining immunity through infection was a "dirty vaccination" that made little sense as it would not protect one against a more harmful variant. He saiid that it "would be naive to think that omicron is the end of the pandemic." Omicron would not make one immune to the next viral variant. "No one can guarantee that a variant will not develop soon that is much more dangerous."

Lauterbach says 25 million doses of the Moderna vaccine have been procured that would be enough for 50 million booster shots to be done in the first quarter of 2022.

Obama Affordable Care Act ACA and its downfall are covered by two experienced authors over 2 book written over 2 decades. The authors are James Morone and David Blumenthal followed the healthcare issue over 25-30 years through the Clinton, Edward Kennedy and Obama efforts and wrote two books. The first was "The Heart of Power" on the healthcare situation from FDR to 2008. The last titled "Whiplash" in 2026, for which the authors are interviewed in Harvard Medicine magazine. C-SPAN has a book program on this book at a Washington DC bookstore. From the discussion on C-SPAN between Senator Michael Blumenthal, borther of one of the authors, James Morone and David Blumenthal physician, couple of conclusions are seen that may be new to readers. Q. What was the one single factor that doomed the Affordable Care Act? A. The deep antipathy towards the Obama administration influenced the response to the Obama handling of healthcare. The likelihood of Republicans accepting healthcare from a black person was simply not there say the book's authors in the discussion and Q&A on C-SPAN. Yet there were other reasons for the ACA failing. Obama had not gauged the mood of the nation well. UK Labour's Starmer won by a big majority in 2024 yet that does not reflect the mood of the British nation just 2 years later- by election year 2012 Obama's campaign was faltering and had to be rescued with Hispanic votes and a weak candidate in Utah's Mitt Romney. Obama lacked maturity and came in the way Bush came in when the list of candidates were mediocre in the US, similar to the period in the UK with David Cameron and Boris Johnson. To take on the health care issue required someone with the experience and caliber of LBJ, which Obama clearly lacked, coming from the minority community was not going to help in credibility. Obama's presidency was thus premature and to gain experience he would have done better in a key cabinet position such as at Department of State where an intellectual could have influenced world opinion in favor of emerging countries, a doable and necessary. Obama's lack of experience showed when he told Republicans two words in the first months in 2008- "We Won," perceiving arrogance it would set Republicans against him. The years 2008-2016 cost the US dearly in that the US needed a withdrawal from all of the Middle East which would require a strong president  with deep roots of support in all parts of the country including the south, to avoid recriminations. In the end by continuing the wars Obama weakened the US and let China move ahead. Q. Did Obama consider Medicare for All? A. Obama told Congressmen of his party according to Morone- if you can get 60 votes in the Senate for Medicare for All we can try.  Q. Would it take a major upheaval for Medicare for All to be accepted now that the health system is failing all Americans in 2026? A. It will take a world war or a economic depression- some major disaster for Medicare for All to be accepted in the US, say the authors. A pandemic happened in 1918 and again in 2019 the results were not positive, as the authors believe it unleashed the war on science after the vaccination for and against camps, leading to the culture wars in America seen today. Q  Obama's analytical mind thought he learned from the Clinton efforts in healthcare that failed. But he did not see things from the heart. There is good reason to think that the lessons learned of moving fast, letting Congress write the legislation, settling for what can be done not what needs to be done, were exactly the wrong lessons to be learned as opposed to writing off the Clinton experience entirely as Clinton's, and starting from scratch without preconceptions. In the end Obama if he was older, had more experience, and listened to the mood of the country would have realized that healthcare was for another day, and got right down to the most difficult challenge, to end the wars in the Middle East. Even small steps in the right direction would still have earned appreciation him today. Instead Bush and Obama, the most inexperienced of presidents will be remembered for wars they continued that weakened America.       ...

The vaccine strategy has changed to where the federal government is taking over the overall responsibility of coordinating the production of vaccines in the country and providing access to vaccines from other countries. Federal government is also taking on overall responsibility for distribution of vaccines and setting up the logistical effort. Vaccine supply is being opened up by opening India to multiple vaccines including Pfizer, Moderna, and other vaccines. Production of Covaxin is being stepped up. This strategy is designed to get India to somewhere closer to the 8 million doses a day needed and to ensure distribution and logistical efforts are in place. More resources are put into the effort. The speed of economic recovery also depends on the vaccination drive. Lessons were learned during the second wave in May 2021 and the government is better prepared for the hard work ahead. ...
