Pfizer's coronavirus pill is 89% effective in preventing people at high risk of needing hospitalization if taken within 3 days of diagnosis. It is called Paxlovid. In the drug protease inhibitors work to disrupt and prevent smaller molecules being formed of the virus needed for virus spread in the body.

Hospitals in Europe are filling up quickly in the second wave of coronavirus. Coronavirus patients had to be transferred by helicopter to Germany from the Netherlands because of overburdened Dutch intensive care units. National Guard troops were flown in from the U.S. to the Czech Republic to help. In France as cases approach cumulative 1 million about 2000 patients are admitted to hospital for coronavirus every day on October 29. At some point French hospitals could be overwhelmed this winter, and doctors having to choose which patients to save, says president Macron. In the Czech Republic a collapse of the health system is expected by mid-November says the prime minister. No one expected this to be this severe, he says.  About 40,000 patients are hospitalized for coronavirus in the U.S. During the last week the case are increasing by over 40% in the U.S. and increase in hospitalizations are expected. Recovery rate is improving from the first wave. At NYU Langone hospital system in New York with 5000 coronavirus patients hospitalized recovery rate is improving from 25% in March to 7% in October. Better handling of cases and knowledge gained by doctors is a big part of this. ...

The chief scientific adviser to the government of Britain says "go hard, and go early" to prevent a surge of coronavirus in Britain in the autumn and winter with more activity indoors. Things are flattish at the moment, but if hospitalizations pick up the government needs to quickly shift from Plan A without vaccine mandate and without mandatory mask use to requiring vaccinations and mask use, calling for working from home, as part of Plan B. Unlike France and other countries in Europe Britain has not taken the strong action that is necessary. Prime minister Boris Johnson is pushing for new efforts in getting the remaining people vaccinated.

The U.S. and India see a surge in coronavirus cases. Per day cases surged to 40,000 on June 25 in U.S. and 17,000 in India. Alabama, Idaho, Nevada, Arizona, California, Florida are seeing a surge in new cases. Texas, California and Florida all recorded more than 5000 cases each. In California cases for hospitalization surged 32% with more than 4200 in hospitals.

India now has 490,000 cases with death toll of 15,301. Indian Railways cancelled all bookings. In the U.S. Texas is paused reopening plans.

The latest late stage trial for Astra Zeneca vaccine in US, Chile and Peru, offers new hope. It is shown in that trial that it is 100% effective in prevention of hospitalization and deaths, and 79% effective in prevention of symptomatic illness from the coronavirus. It is also seen as safe by experts as it goes for FDA regulatory approval in the US. It is provided at cost, and storage is in ordinary refrigerators for long periods, with production in India of large quantities of the vaccine, making it a vaccine that could reach large parts of the world's population.

The second wave of the coronavirus is bigger than the first with the U.S. exceeding 100,000 cases a day for many days in November and hospitalizations doubling to 93,000 from the beginning to the end of November. There is also the fatigue with the virus for healthcare workers and the people, and loss of income for workers leading to income and food insecurity. In this situation a second stimulus to help people and businesses is a urgent priority for Congress. A group of bipartisan senators have put together  $908 billion stimulus plan to get through the Congress by December 11. This is a compromise between the two parties. Supporting the bill are Cassidy, Romney, and Collins for Republicans and Manchin, Warner, Cassidy for Democrats. It would provide- 1. $300 a week in federal unemployment benefits for 4 months. 2. $160 billion for state and local governments. 3. Temporary moratorium on coronavirus related lawsutis. 4. Additional funding for small business, schools, health care, transit, student loans. There is growing agitation among influential senators against the leadership in both parties of McConnell and Schumer, with the sense that the leadership has failed to recognize how critical the issue of emergency relief is for tens of millions of Americans. This is its only hope for passage with the bickering of the leadership on both sides. ...

The variant first identified in India called the Delta variant is 60% more infectious than the Kent variant found in the UK. The Delta variant is now the dominant variant in the UK. There is concern that this could lead to another wave just as the UK is reopening in the summer. There are over 6000 daily cases in the UK this week. The estimated R number is now 1.00 to 1.2 following the number being 1.00 to 1.1 in the earlier week. A R number over 1.0 suggests greater spread of the coronavirus. An R number of 1.1 suggests the number of cumulative cases is taking off meaning that the UK is at risk of a sudden surge in the coronavirus in June or July 2021. India faced a wave from the new variant's higher rate of infectious spread. leading to a sudden surge in May 2021 to 400,000 daily cases before it was brought down by June 1 to about 100,000 The number of hospitalizations in such a wave is estimated to be higher in UK than the previous waves, requiring the government to be more vigilant today. Restrictions on travel from Portugal are being put in place in UK as a precaution. After repeated waves as a consequence of complacency with the coronavirus the lesson now is to take steps early and take aggressive action in advance. ...

This report in NYT suggests that coronavirus in New York is becoming endemic. Endemic means chronic instead of the acute crisis situation in 2020. This is because hospitalizations are not filling up ICU's the way they did in 2020. The burden of the coronavirus pandemic is now on people who are unvaccinated and people under 45 years.  New York has a higher rate of vaccination than the rest of the country. About 76% of people ages 12 and over in New York are vaccinated. Yet the 24% of younger people who remain unvaccinated is a part of the population that has fears about getting vaccinated which are harder to overcome. As many of these younger people also spend more time outdoors, and are part of the community, more likely to be in contact with families, relatives and friends indoors in winter, and less likely to wear masks and follow social distancing the pandemic, one could see another surge in the winter of 2021.  Another problem could be that some portion of the population of vaccinated has not taken booster shots by the middle of winter which would create a population of a third of the people even in cities such as New York that remain unprotected. The emergence of another difficult variant could also be a complicating factor. In this way chronic or endemic may still be elusive where masking, social distancing and other preventive actions are not taken. For the community as a whole there will be the risk to upward mobility from the people who risk losing jobs because of fears of the vaccine.     ...

People in California did not feel the early blows of the coronavirus like New York. This is now turning into a disadvantage as people in California have now failed to follow the guidelines for social distancing and masks as carefully as they should be. The state reopening  took place as the case numbers were increasing as the economy and unemployment became an issue.  State hospitalizations are up 40% on July 1 from 2 weeks ago. Percentage of tests coming positive are close to 6% but in some counties much higher- in Riverside county has rate positive in tests at 11.7% and bars are only recommended to close. In Los Angeles county it is 8.2%. On June 20 the day after the bars were allowed to open 500,000 people visited bars in Los Angeles County. A big problem is that for lockdown the whole state was asked to lockdown by the governor. For reopening it is done by county and each county is doing this differently. Pressure to reopen has led to counties with increasing and poor metrics for cases still reopening. Some counties felt pressured to open when other counties had reopened. Even when a county such as Riverside or Los Angeles county is doing poorly the governor waits 14 days for it to be on a watch list before acting. This is too long for the extremely contagious virus giving it time to spread quickly. Governor Newson is now facing serious problems tackling the coronavirus. ...
