There are 100 vaccines being developed all over the world, of these 30 are in India. One is a collaboration with Britain for the Oxford Astra Zeneca vaccine to be produced largely in India. The other is one by ICMR. Other vaccines are the Johnson and Johnson  one dose onl vaccine. These vaccines have the advantage of not requiring ultra low temperature storage. 

Even though the Pfizer BioNTech vaccine is off to a start in Britain this does not mean that the other vaccines are not going to be out soon as early as by the end of December. There is also no clarity on how long the protection lasts. More than the one billion doses of the Pfizer vaccine will be required making the other vaccines just as or even more important. Asia and Latin America, Africa are still not in the picture and will have to be for the protection to be effective.

A conflict is developing between Britain and the European Union over vaccine supplies as both sides try to get access to limited supplies. Britain and the US have moved ahead with their vaccination drives, causing alarm in Europe as Germany, France, member states of the EU lag behind. The problem comes from the delay in approving the vaccine by Astra Zeneca and Oxford University by the European Union. European Union prestige is at stake because its slower process of approving vaccine has led to a delay of 1 month in approving the Astra Zeneca vaccine. The Oxford vaccine is only now approved in Europe. Other problems have emerged. Astra Zeneca has announced that its vaccines made in Britain are now running short of supply and it can only provide 39 million doses to the EU instead of the 80 million originally arranged by EU. Soon after this announcement Pfizer said its factory in Puurs, Belgium, near Antwerp, is running into production issues. This would reduce supplies to the EU.  The EU has responded to this situation by saying it was being treated unfairly by Astra Zeneca. In response it has introduced new paperwork that would limit supply of Pfizer vaccines to Britain from the Belgian plant. Other countries are watching this situation with dismay as richer countries are fighting for the vaccine supplies. ...

Jenny Strasbourg of the WSJ provides this much needed report from London about the courageous decision by AstraZeneca and Oxford University to give vaccines away at no profit to the whole world, to billions of people in Africa, Asia, and Latin America. Without this brave decision by a British company and a British University the world would be a lot poorer, more variants could have happened, making us realize the great contribution Britain has made and how indispensable it is to the planet. Add to this the effort of Indian companies including Serum Institute that provided the manufacturing facilities and capabilities for making most of the British vaccine. AstraZeneca delivered 2.3 billion doses of the vaccine globally as of mid-December, according to the company. The International Monetary Fund estimates that low and middle income countries received 3.25 billion vaccines as of Dec. 11, About half of this or 1.6 billion doses were Astra Zeneca shots. This is a bigger share than any other vaccine by far and a life saver to the world. AstraZeneca stepped up early in a true to the best ideals in Britain to meet the needs of the world-  aiming to deliver 3 billion doses in 2022 and sell them at no profit as long as the pandemic continues. As the shot does not need cold storage it is ideal for India and other Asia, Africa and Latin America. "We are all very proud throughout the company of the impact we have had," says AstrZeneca CEO Pascal Soriot. By far the biggest manufacturing was done at Serum Institute of India which supplied 1.3 billion doses of the AstraZeneca vaccine to 70 countries. Mr. Modi pushed forward the export of vaccine made in India to the world from the beginning in the same spirit of cooperation and the best ideals that Britain was living upto. Serum Institute can produce as much as 250 million doses of vaccine a month making it possible for India to tackle the vaccination population of 1.3 billion people.   None of this could have happened without Oxford University and AstraZeneca and Indian companies with Mr. Modi's active support living up to the best ideals of Britain and India for the world. "When you add up the benefits to humanity, I think you'll find the vaccine holds up pretty well in terms of the ill health it has prevented, and the deaths it has prevented," says John Bell, a senior Oxford academic who in 2020 guided the University through its vaccine-partnership talks with Astra Zeneca. Because in the real world AstraZeneca shot has held up so well it is also a choice for booster shots. ...

Kate Bingham, head of the Vaccine Task Force in Britain is commended for her intelligent approach in placing bets on vaccines in different technologies, "four buckets" as she calls them. This includes the Moderna and Pfizer in the mRNA technology and the viral vector Astra Zeneca, J&J vaccines. The French British collaboration effort will bring a new vaccine to the global effort against coronavirus. This vaccine was expected in February 21. It will be ready by the third quarter of 2021 just as vaccine supplies are being ramped up. The Sanofi Pasteur unit and GSK Glaxo SmithKline are the two companies in this vaccine venture. This vaccine will address the problem of new mutations of the virus in the British and South African versions and can be stored at normal temperatures. 
 

The new vaccines will boost supplies just when the situation is bleak for vaccine supplies in Latin America and Africa, South East Asia, places left out in the vaccine race in advanced countries and in India. Britain has ordered 60 million doses.

Germany's chancellor Angela Merkel says she will not support the moves by the EU commission to impose export bans on export of Astra Zeneca vaccines made in EU countries to Britain. A Dutch factory has 4 million doses of this vaccine ready for shipment.  Merkel says, "there are a huge range of international interdependencies when it comes to vaccine production. You have to be very careful when it comes to imposing export bans. You have to take a close look at supply chains."  Merkel's action comes as Britain makes an effort to talk to German and French leaders for a fair way to allocate supplies of vaccine. France and Germany see the need for the principles of "reciprocity" and "equivalence" to be covered in settling the differences on vaccine supplies. British regulators say they have"absolute confidence" in the Pfizer vaccine after analysis of over 1000 pages of data on the vaccine. The vaccine was approved by the Medicine and Health care products Regulatory Agency. The first vaccines to be shipped to Britain are being packed in Belgium. Britain has secured 40 million doses enough for 20 million people. Vaccination will begin as soon as doses reach Britain. The NHS will prioritize, first care home staff and residents, then healthcare workers, followed by people over 80 years age. Clinically vulnerable people will get a jab alongside people ages 70-74. People with severe obesity and underlying conditions will get jab after people over 60 years, followed by people over 50 years. About 34% of the 66 million population of Britain is over 50 years age, which is about 22 million. This means the Pfizer vaccine ( with doses already secured by Britain enough for 20 million people) covers over 90% of these people or 19 million people and the 1.1 million workers in NHS. Rapid progress in vaccinating these people would make Britain the first country in the world to have done this, a remarkable achievement. This report in The Times shows the need to counter misinformation in parts of the media about vaccines. Inadvertently or through a poor comprehension of the data, German media reports in Handelsblatt and the Bild have stated that vaccine effectiveness for older people is 8%. Here in The Times of London, Oxford University and Astra Zeneca point out that the 8% figure is for the number of people in the trials who were given the vaccine in the age group 56-70 years. This does not refer to how effective the vaccines were in older people.  The first dose increases monoclonal antibodies for people of all ages, say Astra Zeneca and Oxford. We are now beyond trials in a sense today as Israel has vaccinated large parts of the population and the UK, India are vaccinating millions of British and Indian citizens. Israeli reports from one of the major medical centres show that the second dose increases monoclonal antibodies by multiple times and provides effective protection. As British data is available from medical research institutions from the vaccination drive in Britain, and from India, the effectiveness of the vaccines used in Britain and India will be shown more clearly. India today has used a package with near 100% compliance to tackle the virus relatively effectively by combining safety protocols (masks+ social distancing+ hygiene) with nutritional, medicinal protocols, restricted overseas flights. Cases are down to 13,000 for 1.2 billion people, with positivity rate in testing down to 1.66%. One readers comment in The Times says a lot- She says her 79 year old Irish mother was given the vaccine today in Coventry, England. She was given the Astra Zeneca Oxford vaccine jab by a British Asian doctor who took the time to talk to her, and listened to her and thanked her for her service as a midwife for 40 years. Most questions about the vaccines from Oxford, Pfizer and Moderna are answered in this Q&A in The Times.  The Oxford vaccine is the only vaccine that is being provided at cost to the public at a cost of 4 pounds, Pfizer using German technology at 15 pounds and Moderna from Cambridge, Massachusetts, at 28 pounds. The Oxford vaccine can be stored in a fridge, the others use mRNA technology of messenger RNA which requires ultra low temperature storage. Astra Zeneca could have handled the trials and methodology for results in a better way. As the two trials one that produced results of 62% and the other results of 90% cannot be combined to give results of 70% but are two distinct and separate trials. However too much emphasis has been placed on the vaccine, as other prevention measures remain important for 2021. The Oxford vaccine also uses existing technology for vaccines and manufacturing is being done in India with the world's top manufacturer of vaccines. By using existing technology unlike the Pfizer and Moderna technology Oxford has taken an approach that could prove to be unique by minimizing side effects for vaccines that are being developed with such speed. By not requiring refrigeration at very low temperatures the vaccine makes itself ready for immediate and widespread uses all over the world. By use in its home country India with its large population Oxford vaccine can gain even wider acceptance because of India's long experience in pharmaceutical technology and manufacturing. Of particular interest is the study of 23,000 participants showing that the 90% effective dosage is one that only requires half a dose for the first shot. This say scientists is because the vaccine first dose prepares the body for a more powerful second dose and creates the maximum effect. This means the vaccine can be used for more doses than 2 full doses. It can be stored in a fridge making it easy to use in many countries. A major British and Indian collaboration and scientific achievement of both countries is not given the recognition it should get because of mismanaged communication of the results of clinical trials. Tom Whipple science editor of The Times says do not make the mistake of thinking oh Pfizer vaccine scores a 9 out of 10 and Oxford's a 7 or 8 out of 10. Pfizer vaccine says it 94% effective. But this is only part of the story. It is the first exam paper in a long number of exam papers and the final score will require scoring them all. "Oxford vaccine is complex, and we are happy with the complexity," says Adrian Hill, Oxford researcher and head of the Jenner Institute. It is not highly unusual in this complex field for a half first dose to work better than a full first dose in a two dose vaccine treatment. This happened with the Oxford vaccine. As a result the study results were harder to communicate. This happened by accident. Much of medical research and much of medicine's biggest breakthroughs in the last 200 years happened by accident, as one researcher looked for something and accidentally discovered something else profoundly useful. Whipple's points are turning out to be true now that Britain's medicine regulator has asked that Pfizer vaccine not be given to people with history of allergic reactions after 2 NHS workers had strong allergic reactions. A lot of questions remain for all vaccines. How long will the protection last? WIll it prevent transmission of coronavirus? Are there any other complications? Which vaccines can work without ultralow refrigeration storage? Ahead lie the prospect of billions of doses. Two are in final stages in India including Bharat Biotech request for emergency authorization. Johnson & Johnson has a competing one to Pfizer's in the U.S. As many as 30 are being developed in India and 100 around the world. Countries like South Korea say they will wait to find out which one works best and where cost overall combined with benefit is attractive. Some of the vaccines are coming out only weeks apart. The vast majority of the cases received a control instead of vaccine. A vaccine is considered effective if it works on 50% of cases. The Pfizer vaccine is based on an experimental "mRNa" technology.

Britain has ordered 5 million till the end of 2020 and another 20 million for 2021. In total Pfizer plans to make a billion doses of the vaccine by 2021.

Warnings to governments and leaders in industry and pharmaceutical research about epidemic preparedness by Bill Gates were ignored. He spent hundreds of millions of dollars to develop new vaccines and create disease tracking systems. But only governments could tackle this problem. He tells the WSJ in an interview that he feels terrible and that he wishes he had done more. His fear that a once in a century pandemic has come true. Governments did respond to the public health preparation needs as reported in France 24 to both SARS and the H1N1, both in Britain and France. It was the disbanding of this effort in the period of the global financial crisis and the eurozone financial crisis that led to the level of unpreparedness that Western Europe finds itself in today. This was caused by irresponsible banking practices. The response was austerity measures in Britain, France, Germany and Spain that led to leaving public health system investment being neglected, without fixing the original source of the problem. Misallocation of capital and lopsided priorities continued through most of the period leading up to the pandemic. There is a lot that Gates and other public spirited leaders could do now do in the new reordering of priorities and shifting the allocation of capital to public services and investments in infrastructure, and supply chain renewal to safeguard national interests. Today he is working with pharmaceutical executives and governments to produce billions of doses of vaccines while they are being tested. His foundation has reserved space in a manufacturing plant so that production can begin quickly once an effective vaccine is found. He says nobody has made 7 billion vaccines so that it will need all the help that it can get and international cooperation.  In an earlier interview with WSJ he told the interviewer in November 2014 that the world as a whole did not have preparedness. France and Britain prepared and then abandoned the effort for epidemic response by 2012 following the global financial and eurozone financial crises. Gates repeated the warning to 2016 presidential candidates in the U.S.  In 2017 at the Munich Security Conference he reminded people- "getting ready for a global pandemic is every bit as important as nuclear deterrence and avoiding a climate catastrophe." One focus of Gates was to come up with faster ways to a vaccine by using ready made components and then customizing it.
