Changing the way health care operates in the country, federal and state government agencies and the commercial insurers are looking at paying primary care physicians and family practice physicians more than the average $60 a visit that is paid out so that these physicians spend more time understanding their patients. At the present time the smaller payment per patient results in doctors only spending a few minutes with each patien. In the long run by diagnosing and treating diseases early through a better understanding of patients this upfront increase in payments should result in lower costs resulting from late diagnosis and treatment, and the problems that occur when the patient is not treated early on. It should also result in better and fewer errors in diagnosis.

The Rivlin-Domenici Deficit Report recommends freezing US defense spending from 2012 to 2016 at its current level of over $700 billion a year. This means the Defense department budget would not be adjusted for inflation, and the military would not have the $431 billon in additional spending that the Congressional Budget Office had projected. By contrast Defense Secretary Gates has sought to keep the Defense departmet budget growing at 1% a year after inflation, plus the costs of the war in Afghanistan. And the Bowles -Simpson Deficit Commisssion chairmen have recommended $100 billion in savings by 2015 be used to reduce the deficit. The way Gates sees it the savings of 2-3% annually in department contracts would be used for other military purposes. Rivlin-Domenici and Bowles-Simpson do not see it that way, they want to use the money for deficit reduction and improving the economic prospects for the US.

The 2016 Republican platform is written to express the ideas in the Republican Party for family values, child rearing using the Bible and traditional values, calling pornography a "public health crisis."  The Family Research Council and tradition or family focussed groups played an important role in writing it, with Mr. Trump playing less of a role. Instead of "physical barrier" with Mexico, it says "a wall"  should be built in a taking up Trump's idea, but in general it is a clear expressing of traditional family values that Republicans have supported for years and expressed frustration with the movement of social lives in an opposite direction. The same sex marraige decision by the Supreme Court and transgender debate were not present in 2012 election, so in that sense the Republicans see the country as moving in a different direction. More of the pendulum having swung too far in another direction is how Republicans perceive it, and their response is to go back to how things used to be and the world they knew, based on what they see as positive outcomes for the whole of society. Hence language that encourages teaching of the Bible in public schools, condemning pornography as a menace particularly for children, and insisting on "man made laws being consistent with God given natural rights." There may be psychological hurdles in China's growth with the effects on mental health from lockdowns in major cities, the revolt in the property sector with home buyers losing confidence in developers, the loss of confidence of foreign investors from US and EU. The dependence on the property sector to carry so large a burden of growth for the last 2 decades in China may now look like an error. The dependence on foreign investment may also be an error as the loss of confidence could mean some withdrawal and a lack of sustained investment.  It could even be said that restraints on both sectors property and foreign investors could have created alternative paths to growth, and reduced the shift of factories from the US and Europe to China that have now caused trade friction and and a reverse shift of investment back to home countries of US and EU. Trade friction has it appears backfired in a way that extends to the overall relationship which could have been prevented by preventing the hyper growth that happened. Greg Ip of the WSJ has argued that compared to Japan's growth in the sixties and seventies from a country of 100 million the hyper growth for a country of 1 billion for 2 decades created a massive impact on communities in US and EU that were dependent on factories that were lost to China. This has alienated large sectors of the public in the US and EU which could have been prevented by restraints on hyper growth in China. Ip says the growth was too large and too fast for the US to cope. It may have permanently damaged the relations between the two countries showing that trade and globalization had unintended effects when left to business and governments staying away from keeping an eye on how it was happening. ...

This report in the WSJ shows American households are acting prudently by building up savings of $1.6 trillion, according to the Federal Reserve Bank of New York. As much of these savings are not distributed evenly across the population, and coming back from a period after the 2009 financial crisis when savings in the lower classes had dropped to alarming levels, this saving is good for the future of the American people by building a path to sustained growth for the long term. Readers responses to this report show their dismay at calling savings hoarding, dismay at the idea that saving 3-6 months of expenses would be considered prudent when 1-2 years would be a minimum  and 2-3 years desirable would be considered decent protection in times like the last 2 decades of manmade disasters (shipping out American manufacturing, 2009 financial crisis) or nature driven disasters (the pandemic). For the Biden administration the saving also provides hope that the mistakes of the last two decades and the 2009 period can be avoided. By targeting the $1 trillion in infrastructure spending plan to projects that build synergy throughout the economy and generate more growth for every dollar spent in a long term Renewal America project. Recent WSJ reports show this is happening. The $2 trillion Families and Workers Plan works in a similar way to bring hope in improving the quality of life in America through children's education, childcare, paid leave, health care, affordable housing, climate change investments. The public in America is showing equal prudence by aligning the savings to this approach to set America on a path of long term renewal and development that could be sustained to 2030 or 2035. Germany's national broadcaster DW.com looks into the details of the vaccination drive in countries around the world, including Germany, and finds that there is more to the story than meets the eye in headlines about safety in vaccinations. Many headlines do not tell the whole story carefully. DW revewed reports from Italy, Austria, South Korea, Germany, Spain, the USA, Norway, Belgium and Peru, and found that in most cases health authorites have not found causal links between the vaccination and deaths.  As of March 15 it says 360 million people have been vaccinated in 120 countries, or about 9.25 million a day.  DW.com cites the Pharmacovigilance Risk Assessment Committee (PRAC) of the European Medicines Agency (EMA) which states: 'there is no indication that vaccination has caused these conditions, which are not listed as side effects with this vaccine. Information available so far indicates that the number of thromboembolic events in vaccinated people is no higher than that seen in the general population." As of March 10, 2021 30 cases of blood clots are recorded for 5 million people vaccinated with Astra Zeneca vaccine in the European Economic Area.  The Paul Ehrlich Institute which is in charge of vaccination in Germany has looked into 113 reported deaths in 46 years to 100 years old patients in Germany. Of these 113 deaths PEI finds that 20 died of the Covid 19 infection as it takes 14 days after the second dose for full protection, and 43 died of pre-existing conditions or other infections. For the patient population it says "they were seriously ill patients with many underlying diseases." PEI says "based on the data we have we assume they died of their underlying disease- in a coincidental time with the vaccination." A virologist at the Technical University of Munich, says that the deaths after vaccination are below the expected number of deaths without the vaccination.  ...

From now on the vaccination program in India will be run nationally by the federal government. This will ensure fair access to vaccines to all parts of the Indian population. Earlier vaccine costs were bid up as states and private hospitals bid up prices. Under the new national program 75% of vaccines will be given out by the federal government and 25%  by private hospitals and other private health institutions. The government in New Delhi under prime minister Modi will offer adults free vaccinations. Modi said "We will increase the speed of procuring vaccines and also increase the pace of the vaccination program." Even in private hospitals the cost of vaccine will be kept at Rupees 150 or $2.06. Experts say this is the right policy and the government has learned from errors in letting states and other private institutions run vaccine policy, which made it too fragmented and subject to too many variables, resulting in inequity, and slowing vaccination drives. The Supreme Court stepped in asking for clarity, leading to the clear policy from the federal government announced today.  Advantage of the new policy is that the responsibility lies in one place, and the federal government also has the clout to make things happen, to negotiate with companies and other parties involved effectively. India has vaccinated 222 million people but because of the population being so large at 1.2 billion this comes out to be a small fraction of the population. This puts the task of getting vaccine supplies and getting the vaccination drives to work in the only place that has the determination and the resources to deliver results by vaccinating 1.2 billion people by December 2021. It has never been done before in history says Mr. Modi, and it is a challenge that India is now taking up for itself and for the global community. It also lays the ground for India to help its neighbors in Asia and in Africa, Latin America in 2022.   ...

When the president and his administration are investing trillions of dollars in the economy as Biden is doing with support from friends in Congress from both parties and the US economy is growing with Made in America reviving American manufacturing- this changes the way labor and immigration can be viewed. There is an expanding demand for labor in such an economy and this is true today. Paul Krugman in the NYT shows evidence that the native born Americans have not lost jobs to immigrants in 2019-2024. Much of the demand in the restaurant, hotels and health care industries, in construction, agriculture and occupations native born Americans are less interested in filling are filled by entry level workers who are immigrants. The Wall Street Journal showed in a recent report that Topeka, Kansas is trying to recruit new immigrants to come and live in Kansas where the unemployment rate is lower than the national average today under Biden of 3.7%, and there are thousands of jobs to be filled. This is why Senator Graham of South Carolina and Tillis of North Carolina, the senior Republicans in the Senate, were trying to fix asylum and parole policies in immigration with the help of president Biden to close the border and yet allow an organized flow of new immigrants to the US to fill jobs that would otherwise remain unfilled. Not everybody wants to live in Topeka but there are immigrants such as the Venezuelan and Colombian immigrants shown in that report who are happy to live in the Kansas winters in the prairies of the American heartland. Many come from educated backgrounds and are similar to other Americans already in Topeka such as the mayor of the town, and fit in well say officials in Topeka promoting economic development in the state. It is noteworthy that Kansas is a Republican state for decades.  ...

Svenja Schulze brings new hope and dynamism to Germany's Development Ministry. As head of the Development Ministry she brings international experience in fighting climate change as SPD minister in the last government heading the climate change related Environment ministry. There she launched the climate protection package measures aimed at making Germany climate neutral by 2045. She now heads a ministry with a budget of $13.5 billion (12 billion euros). She wants to cooperate better with the Global South with an effort to tackle poverty and help developing nations. After the shocks of the pandemic this is an essential and important task. Her predecessor as Development minister Heidemarie Wiezcorek-Zeul, SPD minister 1998-2009 says the ministry needs clout in decisionmaking and for this it is important that the Development ministry is separate and an independent entity not lumped in with the Foreign Office as in Britain. That would be quite disastrous she says.  Climate change issues are also seen as development issues and about poverty reduction. This is a useful point that Mr. Modi was trying to make as he addressed the COP26 Summit- that climate change has to be done in the overall context of mitigation, that climate change control is part of poverty reduction and brings in new opportunities when done this way. Examples are zero budget farming, and solar energy as low cost energy for rural areas in India. Here Schulze talks to employees at the Ministry and tell them "We must all strive to make a good life possible for everyone in the world, That may sound overly emotional, but it is our aspiration."  Martina Schaub, chairwoman for VENRO whivh represents 140 private and church development organizations in Germany sees Schulze as a sign of optimism. The need is great particularly in the weak health systems of many countries. It is a sign of hope, and of the new Germany under Schulz. ...

Scott Anderson of the NYT provides an indepth look at the Arab World and its fragmentation through the eyes of five people from each part of the Arab world- Egyptian, Kurd, Syrian, Iraqi and Jordanian. He says the countries that fell apart are precisely the ones that were formed by the British and the French, and Italy, following the defeat of the Ottoman Empire  using divide and rule policies- Britain in Iraq, France in Syria, and Italy in Libya- without much thought given to setting up viable nation states. This is why Iraq has a Sunni-Shia divide, Syria has similar divisions, and Libya with a largely tribal based structure, never really held together after the colonial powers left, and were held together only by strong dictators. Today's problems trace back to these historical events. This is complicated by the largely young demographic and restlessness of the people for change coupled with problems of underdevelopment in education, tribal loyalties, religious loyalties, and lack of political and social structures that could keep the countries together as change and transition to democratic processes took place. The role of the military further complicated matters in Egypt. Even Iran experienced these divisions because of the intervention of the great powers including Russia in Iran since 1900, leading to swings between liberal governments, foreign power supported governments, and a swing back to religious leadership as at present. This is one view of the region, others are presented by Ramadan (Oxford),  Bernard Lewis (Princeton), and leaders in Qatar and Emirates, other experts, some of whom point to the failure in leadership and the elites to find solutions to the problems of underdevelopment, in education, health, infrastructure, and aspirations for a voice in their governance. As the same divisions left by colonial powers affected Asia- in India, China, and Korea, but a larger vision of progress prevailed through crises and difficulties.        ...

During the Bush, Obama, Biden administrations Russia was treated as a secondary economic power on Wall Street, with the focus shifted to China, which damaged relations with Russia which has always seen itself as a Northern European economic power. Some of the roots of the conflict go back to this period. In a nuclear world the size and historical relation in Northern Europe of Russia cannot be ignored purely on economic grounds about the size of it's economy in the way China could not be ignored in the 60's and 70's when it's economy was not what it is today. History and culture are not in Wall Street or Silicon Valley's understanding or grasp of international relations which go beyond economic and business considerations. The Economist cites estimates from the Bank of England showing Britain's national output peaking at 1.5 trillion pounds in 2007 and not likely to return to that level till 2015. It points to fears of a lost decade. Meanwhile debt is rising from 600 billion pounds in 2008 to 1.1 trillion in 2012, making reducing the debt to GDP ratio by 2017 even more difficult. Lower growth affects tax revenues even as social benefit costs increase. Part of the problem is that from 2009-2010 to 2011-2012 public sector net investment declined from 48.5 billion pounds to 28 billion pounds. The Economist suggests Chancellor Osborne take up an additional investment in infrastructure of 28 billion pounds, even borrowing 14 billion pounds in the bond markets if needed, as a prudent step to revive growth. Small improvements in rail, roads and bridges could make up for a lack of large projects. Other suggestions include expanding the "funding for lending" scheme with banks to get capital to small business, finding more savings in the National Health Service, and changing the way Britain taxes development land that remains undeveloped. Britain, now joins, Portugal, Spain, France and Italy, in the failure of austerity measures alone creating a return to economic growth and lower deficits. Sharply lower consumer spending is hurting Apple sales in China. Apple cut sales and issued a sales warning in January 2019. This follows Apple's sharp slowdown in India with its uncompetitive pricing.

Retail sales growth in China- which bounced back in previous downturns- dropped to the lowest level in 15 years in November 2018. Auto sales are down with the sharpest drop in 7 years- the first annual drop in sales since 1990. Fears of a housing bubble have led to restrictions on home purchases for speculation which have not been lifted. Income tax reduction has not increased spending. GDP growth for the fourth quarter dropped to 6.4%.

Further signs of a sharp pullback are seen in the drop in consumption tax revenue falling by 61% in October and 71% in November 2018. The consumption tax is placed on cars, gasoline and luxury goods, and is paid by the companies making the products.
