Moving migrant crisis today was also seen in Surat, 1994, with the plague epidemic when migrant workers left the city. The government had to use paramilitary units in 1994 to quarantine the entire area. During these earlier epidemics the Indian Council of Medical Research and other medical organizations played a significant role. One of the lessons learned from the H1N1 epidemic that originated in Mexico was the need for effective surveillance and real time reporting so that the pathogen can be recognized in real time and tests done at labs to determine what it is, followed by effective response through isolation of region and patients. Dr. Pradeep Awate, Maharashtra's surveillance officer, says robust surveillance systems are important to understand the magnitude of the problem and strategically place resources. The strategies put in place for the Nipah virus in Kerala state by its Health Department in 2019 are the same ones now being used for cornonavirus - contact tracing and management of persons under quarantine. ...

In his monthly television address called "Mann Ki Baat" prime minister Modi apologizes to Indians for the complete lockdown of the country. Modi said "for a a country the size of India we need strong measures. Nobody likes to take strong measures, but it is the only option, seeing other countries and learning that the disease must be dealt with at the beginning before it becomes incurable."  He cautioned that social distancing did not mean emotional or human distancing. Sympathy and cooperation are needed. He called frontline healthcare and sanitation workers "warriors" and "soldiers," and promised Rs 50 lakh (5 million) insurance for 20 lakh (2 million) such workers. He also commended the work of shopkeepers, drivers, bank workers and others to keep essential services running. Modi suggested now is time "to not go out, but to look within yourself. Mr. Peter Navarro, 70, a key advisor to president Trump on trade and economic policy, is now the DPA policy coordinator.  Mr. Navarro takes on the presidential task force role of coordinating government-business cooperation with wartime powers under the Defense Production Act. Mr. Navarro says in an interview with the WSJ that he will take tough measures if necessary. Saying this is no time for delay, to hoard or price gouge, companies must move quickly or the government will clear the obstacles. "This is war. When there are bumps in the road, President Trump is going to knock them down immediately and unmercifully."  A California Economics professor Mr. Navarro warned about the virus and epidemics of this kind in a 2006 book. He now assumes this role after president Trump said about GM "always a mess," after long negotiations with GM leading to a shrinking supply of ventilators and government frustration. President Trump then used DPA powers to order GM to make ventilators and lined up 5 other manufacturers in 2 days. Navarro says he is following Trump time which he has in his office. This meaning he and the president will use the full arsenal of federal powers to get things done in record time.  ...

U.S. president Trump admonished GM for delaying the delivery of ventilators with negotiations, and called it  "always a mess." GM offered varying numbers, initially says the president saying it could supply 40,000 ventilators but later saying it could supply 6000 by late April. As a result the president used the Defense Production Act to order GM to make ventilators for the government. The Trump administration then pulled together other companies that could make ventilators for the government. The president has publicly criticized GM in the past for closing factories and laying off workers in midwestern states.   The president said yesterday that the administration was working to sign contracts with other companies including, General Electric, Philips, Hamilton, and Medtronic. In all the administration wants to get 100,000 ventilators in a short time frame to meet the needs of hospitals in states with need. Any surplus ventilators could be sent to UK, France, Italy, and other friendly countries that cannot manufacture on their own. At his daily press briefing Friday Mr. Trump said he called Boris Johnson of the UK and the first thing Boris told him was "we need more ventilators." ...

The U.S. Senate bill adds $454 billion for Treasury that can support further Fed action if needed. This has also resulted in a recovery in the stock markets. The editors of WSJ caution Treasury from intervening too far up the risk curve to help companies that had overleveraged themselves with risk before coronavirus hit. It makes clear that the U.S. central bank the Fed should only offer liquidity against good collateral to companies that were healthy before the shock. As president  Trump never tires of telling listeners to his daily briefings from the Brady room in the White House- Boeing and the airlines were healthy before coronavirus hit. It was not their fault that coronavirus hit so suddenly. These companies deserve government help, says the president. By making the distinction between otherwise healthy companies and companies that overleveraged themselves on their own, the Fed, Treasury, and the U.S. government can get more bang for the buck. The WSJ editorial also says there is a bit of good news in the behaviour of politicians, media and the public in the way they are ignoring the trivial politics and self-centred behaviours, including indiscriminately being critical of the president, and focusing on the important matters that affect all our lives.  ...

Reflecting he says he did his best with an expanded level of membership for Labour party, and shifting the party to an interventionist economic policy that was anti-austerity investment led economy. He made his share of mistakes says Corbyn, as he was just human. And urged new Labour party leaders to spend time listening to people in all parts of the country, and recognize the strengths and good in the people.  ...

In buses or transport no one in front or back of you. In conference rooms a similar distance. In parks outside 2 benches long distance. Be sure to see this video as it is critical in the fight against coronavirus. Most people don't get it that a large number of people have it and are asymptomatic, (its not like the regular flu where everyone has symptoms) yet they are carriers of the virus and spreaders of it. That is how the problem got worse in Europe and America in congested areas where restrictions were relaxed for too long. This is why the 2 metres rule (6 feet) is a safety rule, just like not crossing a red traffic light.

This report shows the level of coronavirus testing as of March 27, 2020 in different states in the U.S. The U.S. testing program and supplies of tests are being ramped up quickly. About 65,000 tests are being done on Americans each day. The need is for 150,000 tests a day say public health officials. Testing is slow in California compared to aggressive testing in New York which has 60% of all coronavirus cases. California with twice the population of New York has done 77,000 tests compared to 122,000 in New York. Texas has done only 21,000 states by comparison. Every week new testing technology is being developed in different labs, medical companies, and universities. Including tests that can be conducted quickly and not requiring health workers or health workers with protective gear. Speed of processing test, least point of testing contact, and the protocol for quarantine, are all part of the testing and isolating of clusters mechanism to tackle the virus. This is critical in the coronavirus action plan being developed by Dr Brx of the U.S. White House team. ...

The contrast between lack of effective measures taken in the Lombardy region with the aggressive action in Veneto that has proved effective. Veneto followed the method of quarantining, mass testing for clusters and isolating the affected people.  The Italian government took the first steps to close off northern Italy Feb 22, and it was not till March 10 that a nationwide lockdown was done. The action taken in the Veneto region is shown here in this WSJ report with the town of Vo as an example of steps taken that worked. A microbiology professor and infectious diseases expert at the University of Padua, Dr Crisanti, developed a test for the coronavirus as early as mid-January using the information made public by Chinese doctors. Dr. Crisanti oversaw the testing of 95% of residents of Vo, a town of 3400 people in Veneto region. He found 3% of the population was infected, with half testing positive asymptomatic. Following the aggressive lockdown the tests were done two weeks later and the rate of infection had fallen to 0.1% with only 8 new infections. "The main lesson from VO is that when you have a cluster of infected people, you should do a very aggressive lockdown and then test as many people as possible," Dr Crisanti says. The results from Vo led to Veneto increasing testing in the rest of the region carrying out 80,000 tests, compared to 88,000 in Lombardy, with double the population and 5 times more infections. Lombardy followed government directives to test only those with symptoms. When it spreads it is harder to do the test isolate clusters, test isolate clusters, in a continual loop, yet this remains the method cited by Dr. Brx in the U.S. today as the right way to target clusters in a laser approach. In yesterday's briefing at the White House Dr Brx said this is a method the U.S. is familiar with and has used in Africa to tackle HIV, Ebola Virus. It is possible using GPS to target down to a specific clinic in a specific place, which is how it was successfully done in Africa. ...

The mistakes  and the right action done in Italy that the world can learn from as Italy tackles the coronavirus. The coronavirus is a dangerous pandemic yet there is one part of it that can be used to take the right action. The timeline of countries affected early in January and February and early March with information from these countries on what worked very effectively and what did not work with bad results is available. The mistakes were made in Bergamo, a town in Lombardy region of northern Italy with the highest number of infections and deaths in Italy. Bergamo had limited testing, no rigorous attitude for quarantining those who had come in contact with people testing positive, and lack of contact tracing. In Vo another town in northern Italy the situation is a complete contrast with resort to mass testing and isolation of clusters which has reduced infections to zero and made it a safe place. Vo is a small rural town 85 miles east of Bergamo in the Veneto region. This was the method used in South Korea, China, Taiwan and other Asian countries that have overcome the virus. Bergamo is an example of what failed in Italy with the worst number of fatalities. The health crisis worldwide has shown this  method of first general quarantine to buy time to build capabilities for testing  and preventing things spiralling out of control,  then mass testing, contact tracing and isolating the people who test positive, and repeating this process again and again till infections are way down,  is the only way to control this crisis. In the early days massive quarantine or stay at home strictly enforced is the best solution till production of tests accelerates to permit mass testing and isolating the clusters of infections. This mass quarantine buys time for accelerated production of tests and building up the capabilities of labs to process these tests, including use of a central national lab centre with national data on computers for microbiologists to monitor the entire country. This was done in South Korea reports in WSJ show. This is vital for everyone involved in the effort to control the virus to understand based on the experience of  countries that have successfully overcome coronavirus. It is the experience in South Korea and Italy that the U.S. White House response coordinator Dr. Brx is looking at and learning from as she and the White House team in the U.S., governors of all 51 states, health officials including CDC, are looking at as they execute their action plan in phases.  ...
