At a critical juncture in the global fight against the pandemic eight in ten U.S. counties are in lockdown. About 29% of the U.S. economy is offline on April 5, 2020, according to Moody's Analytics. U.S. daily output has fallen by 29% compared to March 2019. Moody's Analytics predicts a 30% annualized decline in the second quarter GDP as businesses gradually reopen in the summer. Higher unemployment and loss of household wealth are likely to cause demand side drops making the recovery very gradual in this scenario. It all depends on how long this lasts and how effective the fight against the pandemic is including the steps taken to cut the spread of the virus, the action taken for rapid testing and isolating of clusters as happened in South Korea and Taiwan, which remain models for effective action.

This WSJ report on critical analysis of coronavirus data has a very useful chart of Estimated Range of Symptomatic Cases Reported by Country. Complete coronavirus data for all symptomatic persons who have the coronavirus infection is lacking in most countries. Many people in large populations have symptoms and are positive but are not reflected in the official data collection. This is a big problem as the total number of cases are understated by a magnitude of twice to five times the numbers reported in official tally.   South Korea has done a good job of getting more of the symptomatic people with the infection in its data, as about 53% to 90% of such persons are reflected in official data. Next comes Germany at a range of 38% to 55%.  China comes third and has about 28% to about 38% of such persons reflected in its data, the U.S. currently on April 4 at about 14% to 19%, according to this chart in the WSJ. The source for this is Mathematical Modeling Center at the London School of Hygiene and Tropical Medicine. What this means is that the US. number of cases at 278,000  reported infected people with symptoms (April 4) is only 14% to 19% of the true number. Another way to say this is that the actual infected persons with symptoms is about 5 times what is reported, or over 1 million not the 278,000 reported.  As happens for China data collection agencies may never get the true number. To be comparable to the Chinese numbers, as the U.S. is a large country, the figure closer to the true numbers would be twice the 278,000 reported or over half a million symptomatic infections of coronavirus in the U.S. Why is this data important. With widespread testing as in South Korea one gets data that tells one how many people are infected (the size of the problem) and therefore the resources needed and the point of greatest impact. Also it tells one the typical transmission rate per person, and it helps hospitals in each area know what to expect and what resources are needed to prepare- not find people suddenly turn up in the E.R. in unpredictable numbers. The lack of widespread testing and better reporting in the data to get a grip on the pandemic is shown in this chart for countries hardest hit, less than 5-6% for Italy and Spain. As other countries imposed lockdowns including the U.S. and most of Europe, most of Asia, two countries Sweden and Japan continued to stall in restricting public gatherings and imposing lockdowns. Sweden's government is now looking at a U turn and imposing a lockdown. The government needs emergency powers to do this and it lack a consensus with the opposition parties to take this action quickly, says this report in DW.com. Japan is also looking at a lockdown to restrict the spread of the pandemic as cases mount.

Chancellor Merkel has shown exceptional leadership during the coronavirus health crisis in Germany, leading to increasing popularity for her government. Her approval ratings are the highest of any coalition government in the last 23 years. The government says this report in DW.com has appeared level headed in its actions, and Merkel has paid careful attention to advice from Germany's Robert Koch Institute, the German public health agency. Merkel also shared important facts for tackling coronavirus in an effective television address. By listening to the expertise of RKI public health agency the government has maintained calm and better handled the crisis. In the U.S. a similar process is taking place with the heads of the public health agencies, Dr. Fauci and Dr Birx playing a similar role, and president Trump paying careful attention to health experts. DW.com says doctors, nurses, police officers, and people working at supermarkets and pharmacies, are being applauded by the public, with people taking to the balconies to applaud. This is also happening in Spain. The hope expressed by DW.com is that going forward these professions will be better paid and public health become a top priority. ...

Coronavirus infections peak in Spain by April 4 and the number of deaths at 809 in one day is a drop from earlier in the week. Madrid is hit hard. The number of infections in one day dropped from 7472 to 7026 on Saturday April 4. Officials are looking for ways to get masks for all citizens as a way to prepare for an eventually ease restrictions. It is virtually impossible to get masks in Spain. Lockdown has been extended to April 25. 

Masks are mandatory in Austria, Czech Republic, Israel and Turkey.

Sir Keir Starmer is elected leader of the Labour Party in Britain with 56% of the vote. Starmer 57 years old, is a human rights lawyer who was director of public prosecutions from 2008 to 2013, and elected Labour leader just 5 years after being elected to parliament. Angela Rayner, shadow education secretary won the deputy leadership race with 52% of the vote. 

The new shadow chancellor is the MP for Oxford East since 2017 Ms. Anneliese Dodds. She is a former academic and member of the European parliament. Jo Stevens the MP from Cardiff East is the new shadow foreign secretary. He resigned from the Corbyn team in 2017 to oppose Brexit.

Corbyn sceptics swept the elections to the National Executive Committee. Starmer supports EU freedom of movement to continue, public ownership of services such as post, rail and energy, and raising income tax on the top 5% of earners.

The people in the U.S. are shifting to widespread use of masks. There was some cultural resistance in Europe and the U.S. to use of masks, compared to Japan, South Korea and China where the use of masks in epidemics was common in earlier health crises. Europe changed first and now the U.S. is adopting masks as a way to avoid th spread of coronavirus. Health authorites in the U.S. now recommend use of masks to prevent asymptomatic people with infection from spreading the infection. Health experts say the widespread use of masks in Asia is one reason in addition to quarantines, contact tracing and isolation of clusters, is how China, South Korea, Taiwan and Singapore have controlled coronavirus to the point where it is no longer a serious danger.

The NHS Nightingale hospital was built in 9 days by converting the ExCel convention center in east London. This is the largest hospital of this kind ever built with 4000 beds. Prince Charles opening it by video link from his home in Scotland said it was " a spectacular and almost unbelievable feat of work." The hospital is named after the founder of modern nursing Florence Nightingale. The British government drafted hundreds of army engineers to help contractors build the hospital in record time. It will need 16,000 staff and hundreds of volunteers.

Having an adequate supply of N95 masks is critical for each hospital tackling the coronavirus pandemic. The lack of enough masks leaves health care personnel without the basic protection and is a grave emergency. Hospitals are resorting to reuse of the masks in this crisis and this is not a good practice as it increases the chances of infection. President Trump has invoked the Defense Production Act on April 2 against 3M. This gives the federal government more control over 3M's operations to ensure that it goes all out to make the healthcare N95 masks that the hospitals need in this grave emergency. This report in the WSJ covers the situation as of April 3 on the supply of M95 masks for health workers and others. N95 masks block 95% of very small particles. Supply in the U.S. is for 50 million N95 masks. Demand in the U.S. is for 300 million N95 masks as estimated by the Department of Health and Human Services. in March- this is how many are needed by health care workers to fight this pandemic in the U.S. The principal manufacturer is 3M. 3M company has doubled its production since January 2020. The trend before this pandemic was to send production over to China and other countries. This is changing now with the pandemic and the U.S. policy shifting to be self sufficient in medical supplies in the event of an emergency. A policy Peter Navarro, who heads the agency in charge of getting medical supplies, says President Trump is insisting be implemented. Hospital buyers supported the earlier trend to keep costs down, but this appears to be a costly mistake, putting health care workers in hospitals across the U.S. without the basic protection they need. Minnesota based 3M invented the first modern disposable masks in the 1960's. Interestingly 3M continued to make millions of masks in the U.S. even though competitors moved manufacturing overseas. The 50 million disposable masks 3M made globally went to workers in industries where it provided extra safety from metal shavings or other substances, and medical workers. Now 90% of masks go to medical workers. 3M ramped up production globally since January 11 when the pandemic first hit to 100 million masks a month globally, and 35 million a month in the U.S. at plants in South Dakota and Nebraska. 3M says that it will import 10 million masks from its factory in China, which earlier this year was restricted from shipping it outside China as China needed masks for the pandemic. About 10 million more masks are made by two other manufacturers Alpha Pro and Louis Gerson Co.  U.S. Department of Health and Human Services ordered 600 million N95 masks from 5 companies to distribute to hospitals and build up the national medical supply stockpile. 190 million each of this is from 3M and Honeywell and 130 million Owens & Minor Inc.  3M says it will make 50 million a month in the U.S. by June. Honeywell which had moved production overseas, plans to bring back production to the U.S. by making 10 million masks by May at its Rhode Island and Phoenix plants. There is a company in Singapore that makes one million masks a day in China and other Asian countries, Pasture Pharma Pte, but most of it is committed to government agencies in China.   ...

Florida has counties in the list and the state only imposed a lockdown in the state as a whole on March 25. People in Jacksonville and Tampa continued to travel at a higher rate than people in other parts of the country says this report in NYT. In Louisiana not till March 20 weeks after after Mardi Gras celebrations in New Orleans was travel reduced significantly. 

It takes 2-3 weeks of this higher travel to show up in data about infected people. Dr Fauci, the leading health expert on the crisis, says the impact of following the stay at home lockdown shows up in the reduced level of new cases in many areas, making this change in behaviour very critical.
