President Trump says the U.S. could permanently cut off funding to the WHO and revoke U.S. membership if the group does not make changes in the way it operates showing a lack of transparency in its operations and dependence on China. Mr. Trump says the WHO has shown "alarming lack of independence" from Beijing. In a direct letter to the Director-General Tedros Ghebreyesus -"it is clear the missteps by you and your organization in responding to the pandemic have been extremely costly to the world." Mr. Trump gave the WHO 30 days to make "major substantive improvements" or he would cut funding and reevaluate U.S. membership. Mr. Trump said in the letter that the WHO ignored early reports of the virus spreading in Wuhan, failed to share information with other countries. The U.S. which has the largest contribution by far to the WHO was unable to influence the organization. The U.S. has influence in finance at the IMF, the World Bank, and in the tech world, yet this did not extend to important matters of public health. It could be that public health had become an afterthought in the rush to prominence in tech and finance. The contributions of the U.S. exceed anything any other country has made. During the 2 years 2018 and 2019 the U.S. contributed $893 million, according to WHO records, cited in the WSJ. During this period the contribution of China was $86 million with an additional $50 million added recently. The $2 billion Mr. Xi said China will contribute is incorrectly reported as for the WHO, it is what China says it will use to support Africa and other countries in the world to fight the pandemic. ...

Dr. Nancy Krieger of the Harvard School of Public Health says data on health risk -including which counties have high rates of health conditions such as obesity and other diseases- should inform targeted interventions and public messaging. This is important for administrators and health authorites in each county and state as they deal with a surge in coronavirus in parts of the U.S. which have so far been only slightly affected such as rural southern and western states. More recently places such as North Dakoa which the virus missed in the early stages are now seeing a jump in cases. This report shows counties in states such as Tennessee, Georgia, Alabama, Mississippi, North Carolina, particularly susceptible because of high rates of obesity and other health conditions. In all almost half of the U.S. population has one of these health conditions- obesity, diabetes, high blood pressure, lung disease and heart disease.  The high prevalence of obesity which worsens all other health conditions is a result of the lack of healthy food and nutrition, exercize and healthy living habits, that were neglected in the closing decades of the twentieth and early twenty first century. The shift to processed foods is only one of many bad practices that happened in these years. The lack of attention to healthy eating and nutrition, and outdoor exercize has marked this period and created conditions in which the virus could damage health. ...

Encouraging early results for a vaccine from Moderna Inc. which tested on a small group of 8 healthy humans. A key requirement that the neutralizing antibodies created by the vaccine stop the coronavirus from replicating was met in the vaccine developed by Moderna. A larger human trial will now proceed for hundreds of healthy people. In July another trial will have thousands of healthy people. By the end of the year the company plans to make millions of vaccines.

This report in the Times says there are divisions in 10 Downing Street, between Mark Sedwill, the top civil servant, and the prime minister, Boris Johnson on implementation of a coronavirus plan, Mr. Sunak the finance minister and Mr. Johnson on reopening of the economy and the spending plans. There are also frustrations among Tory MP's, women, and ministers, who are left out of the centralized decision making where only a few people around Mr. Johnson- Gove, Hancock, Sedwill, Sunak, Cummings - know what is going on.  Mr. Johnson has not rested well says the Times and Mr. Sedwill also had the virus it is reported. Mr. Hancock is not trusted by Tory MP's to execute the plan correctly for testing, tracing and isolation of clusters, crucial to controlling the pandemic and restoring confidence in the economy. Mr. Johnson also face the prospect of an inquiry into nursing homes- he responded to Keir Starmer of the Labor party in parliament that he"bitterly regrets" what happened there. More money is being poured to control this. There is a sentiment of acceptance that Brexit talks are going nowhere and Britain simply walks out with no deal.   ...

Cygnus is the rehearsal for a full blown pandemic that the British government conducted in 2016. After preparations accelerated following the SARS crisis in 2009 and the H1N1 pandemic in 2015, the British government accelerated its preparations for H2N2 the next pandemic. The exercize took place in October 2016 for 3 days, for a worst case scenario flu pandemic affecting 50% of the population and causing 400,000 extra deaths. For 3 days people were told to imagine being in the 7th week of the pandemic, facing peaks in demand for health and hospital care. Cobra meetings were to be held. This report in the Guardian gives details on the Cygnus exercize. It showed a lack of UK readiness.  By July 2016 prime minister Cameron was replaced by Theresa May in the Conservative Party and a full blown crisis emerged for Brexit. Britain lost interest in Cygnus or the pandemic prevention effort as Brexit consumed Britain's energies. Soon it was forgotten by the time Boris Johnson became prime minister and won the Brexit election. This shows how even a sincere effort and preparation over years of planning can result in nothing. One insight was that while every agency acted there was no coordinated response with someone in the central authority guiding the entire effort step by step. France was exceptionally well prepared says France 24, citing a report in Le Monde, for the SARS crisis in 2002 and the H1N1 influenza in 2009. A billion masks were stockpiled by 2009. Following the H1N1 influenza not appearing in any significant way the media, political parties and the public shifted their attention away from public health crises preparation. For H1N1 the government spent 1 billion dollars some of it going to pharmaceutical labs. The eurozone financial crisis that followed the global financial crisis shifted policy to austerity measures. The entire preparation effort for influenza type health crises was abandoned as too costly.  The same pattern repeated in Britain which was also well prepared before 2010. Austerity budgets after 2010 had little room for public health investment.  One could say a similar pattern was seen in the U.S. Today the worst hit countries are U.S., Britain, France and other European countries. France which had 1 billion masks in 2009 to tackle a possible H1N1 epidemic finds itself with 150 million masks in March 2020 and scrambling to find masks. Some masks which were usable were even destroyed as expired, ministers and experts who had built up the prevention effort in 2009 were even demoted and forgotten, as was much of the preparation in these years. It wasn't just medical supplies pubic awareness had practically disappeared. In the U.S., in Europe, the same situation of a lack of public awareness so that experts, government, and the public could work together quickly, was clear to see. In countries such as Taiwan the preparation led to speedy response at all levels, making contact tracing, isolation of clusters effective. In the U.S. and Europe this early, early, period was lost leading to makeup mitigation measures and the growing sense of a loss of control over the virus. ...

Britain staged a rehearsal for a pandemic called Cygnus in 2016. Till that time the preparations for a pandemic that started years earlier during the SARS crisis were conducted vigorously. Yet the by this time Britain was becoming immersed in the Brexit struggles in the ruling Conservative Party. Prime minister Cameron resigned on July 13, 2016  and was replaced by Theresa May. From that time on the struggles with pro Brexit factions led by Boris Johnson consumed the COnservative Party and sucked the life out of the pandemic planning that Britain had conducted for years before. The recommendations to correct deficiencies from the pandemic rehearsal exercize were ignored. The second failure happened as the crisis approached. Again the Brexit date of January 31 intervened and the months long struggle to get Brexit had taken so much energy and tired out most of the British public including new prime minister and leader of the Conservative Party Boris Johnson. Johnson did not attend the first Cobra meeting of the highest level ministers and military, convened on January 25, 2020, as reported in the Times. Such meeting are convened only for a national threat. Only 5 weeks later on March 2 did the prime minister attend a Cobra meeting. During this time the situation was grave in Italy with rising cases and infections. The entire process was conducted during this time by the Health minister Mr. Hancock who had assured the public that the situation was under control. Britain now has the highest number of infections in Europe exceeding that in Italy- at 240,000 on May 15, 2020. The prime minister and his adviser Mr. Cummings, were also infected by the virus, and Mr. Johnson spent time in ICU before recovering. Queen Elizabeth addressed the nation on Easter day, the first such address since 1940, to boost Briain's spirits. Never had Britain been less prepared as in 2020 when earlier preparations were ditched for austerity plans and events such as Brexit fatigue conspired to strip the nation of the crucial 5-6 weeks of preparation since the first January 25 Cobra meeting of the highest people in government.  Never had such preparation even for 6 weeks been more crucial than in February and March as the infectivity ratio was determined by infectious disease specialists at the best British universities and scientific institutions to be between 2.6 and 3.4 compared to the Spanish flu pandemic of 1918 which was somewhere between 2.0 and 3.0. This means every one person infects another 3 persons, compared to about 1 person in a regular flu season. This reproduction ratio and the nature of coronavirus remain a threat today as Britain, Europe, the U.S. and the world reopens.  As reported in the Times the infectivity ratio was also the reason for the mindset that refused to believe that the virus was real because at 3.0 infectivity the only way to tackle it was a "lockdown," and this was itself an "apocalypse" scenario for many in the pro-Brexit Conservative party that won the election, which badly wanted to get back to economic activity after Brexit. ...

President Trump says he is considering restoring funding to the WHO, but reduce the overall funding by 90%. He told Lou Dobbs of Fox News he would reduce the funding to 10% of what the U.S. was paying, to bring it to the level China was paying. China pays only 10% of what the U.S. pays and has a much larger influence on the WHO. In this crisis this has affected the normal WHO response to virus epidemics as much of the pre crisis development was missed by the WHO, and the WHO failed to ensure transparency from all its members so that international cooperation could happen very early to prevent the virus from spreading quickly.

The coronavirus is changing the office that people were used to and changing how many days they spend in the office. Hybrid scheduling is a new term that means workers can combine say 2 days working from home with three days in the office.

This will be an acceptable way for companies to operate. Companies are finding out that it is not necessary for employees to spend all their time 8 to 5 in offices to be productive. In fact studies have shown even in offices the distractions of meetings and other distractions on the phone cuts into productivity, so that some time or days need to be set aside to work without such distractions. 

Internet companies and software companies in particular are finding that it is not necessary to have employees work from offices. Another change is that smaller satellite offices could open up in less expensive locations and the workforce becomes less centralized.

New York's Brooklyn Hospital and other non profit hospitals in the state lack the funding of hospitals such as Northwell and Langone in Manhattan, and the funding of public hospitals in the state. Serving low income communities these hospitals face a severe funding crisis after elective surgeries were suspended during the worst period of the coronavirus. This will further compound the inequities in New York city and the state for access to health services.

More women tend to work in sectors such as retailing and personal care, and with the hardest hit sectors including fields in which we find more women such as education, leisure, hospitality, a lot more women will be affected. The unemployment rate for women and men started at 3.5% in February before the pandemic. In April the unemployment rate went up to 14.4%. Of this women unemployment was at 16.2% and men was at 13.5%. The women were adversely affected where their presence is highest - in food preparation, health care support and personal service.

The U.S. faces a critical gap in its coronavirus effort - the lack of one centralized source of reliable quality data. What we have today says this report in WSJ, are many disparate sources of information, without any uniform set of rules, different chronologies, and lacking consistency, all feeding into national or global databases run by individuals or private organizations that lack the resources needed. Not  the centralized government source for quality data that is being used in other countries. This is the second of articles in the WSJ on this problem. The first was on the John Hopkins database run by students and a professor lacking the funding or the resources for such a critical task, dependent on disparate and multiple sources of information without any set of rules. Other sources at the University of Washington or run by private institutions face similar problems. The data coming out of these databases is only as good as the data going in, say experts. As a substitute for quality data from a centralized U.S. government source these sources cannot give the decision makers in states the confidence they need, and the federal public health decision makers the confidence they need in their decisions for reopening in stages, says this report in the WSJ. ...
