Renault SA sales increased by 3.2% to 2.7 million cars in 2014. Renault does not sell vehicles in the U.S., and it has only a small operation in China. Sales in emerging markets outside of Europe declined from 50% of sales to 46%. Sales in Brazil were up 0.3% and sales in Argentina declined by 40%. Sales in Russia declined. About one thousand clincs are operating in the US at drugstores, supermarkets and big stores since the idea took root 4 years ago. Wal-Mart has partnered with hopitals like the Christus Medical Group in Texas, Aurora Health System in Wisconsin and COxHealth in Missouri to setup clinics. Mayo did so after employees and patients said they wanted more convenient treatment for minor medical problems, so there may be a need here that as not been met. The lower costs at these centers compared to primary care doctors offices or emergency rooms make it possible for them to price lower and meet the needs of the 45 million or so uninsured people in the US, numbers growing as jobless rate increases. They are typically staffed by nurse practitioers or physicians asistants. Dr Herman at Mayo Clinic, who supervises its retail store clinics, says rather than fight this trend primary care doctors should learn from it, and work with hospitals around the country to offer more convenient locations and consumer friendly office hours, including periods of walk-in care with no appointment....

Doctor are suffering from a "demoralization syndrome" says Eric Reinhart, physician at Northwestern University. He sites one study that shows universal access to health care in America could have prevented at least 338,000 deaths during the pandemic. Overwork comes from physicians retiring from the healthcare workforce in large numbers. In 2021 117,000 physicians retired and only 40,000 physicians joined the workforce. Another 20% will retire in 5 years. Burnout itself comes from a realization that the system is failing its people and you are working within it without the social and cultural narratives that supported previous generations of physicians. It is these narratives of honesty and service that are the essential parts of the human condition that are lacking without access to good health care for all the people and the education to promote good health.

Arizona, Utah and South Carolina are 3 states that are cited by the US Labor Department for not adopting any portion of the Occupational and Health Safety Emergency Standard for health care workers. The Labor Department says that by not adopting these standards for social distancing, mask use and paid time off for vaccination, these states are risking the health and safety of health care workers. OSHA sent letters to these states revoking the states abilities to run their own occupational health safety programs.

Biden calls ending the war in Afghnistan a "wise decision" for the American people. He says in his foreign policy speech that "it is about ending an era of major military operations to remake other countries." A Pew Research poll shows 54% of American adults support the decision.  In a sense the decision had already been made. Biden cited the Doha agreement president Trump signed a year ago with Taliban that called for the release of 5000 Taliban prisoners which included most of the top commanders, and no agreement on the future of Afghanistan. The decision had come much earlier than that when the wars in Iraq and Afghanistan from the period of George Bush were rejected by the American people for the cost and lack of purpose during the presidential election of 2016. That period marked the rejection of policies set under Reagan, Bush and Obama for starting American involvement in the Iraq-Iran conflict first on one side and then on the other side. All the time precious resources that were needed for infrastructure and services in education and healthcare were diverted to these wars, impoverishing America and also Europe. Looking beyond the words thrown around for political advantage both Trump and Biden and the American people, had decided to put these wars behind them 5-10 years earlier. Biden said assertively that America had made a tragic wrong turn, that was all he could say about Reagan, Bush, Obama policy. In the meantime he stated something else was happening- the US was losing its position in the world by wasting its resources in these wars that do not serve the interests of America. "There is nothing China and Russia would want more in this competition than the US to be bogged down for another ten years in these wars."  Biden was saying that he had the courage and tenacity to make a decision that was the right one and a wise one for America against all the transient opinion of people who lacked a grasp of what was happening to the American people- the increasing impoverishing of America in both rural and urban areas. Problems with China's health care system stem form years of underspending. About 1% of GDP went to healthcare in 2006,according to the WHO, ranking China at No. 156 of 196 nations. This underinvestment has caused great hardship to the rural poor who have postponed or been denied access to healthcare because of exorbitant expenses. It shows up in the number of trained medical workers- only 17% of China's medical workers are university graduates. In village and township clinics the number falls to 2%. The government has dedicated $121 billion for health care through 2011. But this may not be enough. Aobut 300 million in the rural areas have no coverage at all. The government's plan is to get farmers insured through county level rural- cooperative insurance plans. These programs begun in 2003, offer only scanty coverage. Outpatient services and medications are not covered and coverage varies from county to county for hospitalization bills. The goal of th health ministry is to get the coverage for hospitalization bills up to 50%....

Brazilians turn to pawn shops lending by government owned bank Caixa, in a regulated portion of the lending industry, as credit card rates increase. Brazil experienced a huge surge in credit card debt in the years when consumer loans were freely made in the last decade. Between 2004 and 2014, consumer credit in Brazil increased 658% to $297 bilion, according to the National Association of Executives in Finance, Administration and Accounting. Central bank figures show 6.7% of personal bank loans and 26.3% of credit card accounts being in default. As in Turkey much of the country's growth was fueled by increased spending and consumer credit. The credit binge and the lower revenues from a decline in commodity prices is leading to slow growth and a stagnant economy.
