Using the design-build method under which architects and construction experts work together as a team, Orange Medical Centre, a new 374 bed hospital in Middletown, New Jersey, has reduced the construction costs by one third. Only 9% of the total square footage of hospitals built in the USA used this method according to Modern Healthcare an industry publication, so this is one more area in which health care savings can be found- and there may be many more areas like this- which goes to show that ingenuity, experimenting with new ways and asking questions about old ways of doing things, can generate large savings, especially when it becomes a necessity. As the old proverb says, necessity is the mother of invention. Design-build differs from traditional construction in that the architect and the construction company come from the same company. They understand each other so well that they can finish each other's sentences. This cuts out the waste and back and forth that goes on with changes that are made, as the changes are minimal. This is a big source of the savings. This is similiar to the integration of design and manufacturing experts into one team in the auto and other industries. HBE Corporation of St Louis is the company that handled this work. Its asmall outfit with 500 people and this is the largest hopital it has built, so there is potential for doing alot more in this field for health care savings....

In a Fox News Interview with Sean Hannity Venezuela's Opposition leader Maria Corina Machado says her stand-in Edmundo Gonzalez won by 70% of the vote in the 2024 election. In 2026 she says she would get 90% of the vote. She tells Hannity:  "I do want to say today, on behalf of the Venezuelan people, how grateful we are for his courageous mission.” She wants to share the Nobel Peace Prize with DJT.   Secretary of State Marco Rubio, points out that one reason the US is working through the interim leadership is that most of the Opposition has left Venezuela. DJT has said "it would be very tough" for Machado to become the leader of Venezuela because of the military, gangs and other militias that Chavez and Maduro have created that would disrupt the country's transition. DJT's view is that “We have to fix the country first. You can’t have an election. There’s no way the people could even vote...No, it’s going to take a period of time. We have — we have to nurse the country back to health." The key is maintaining the county's stability after Maduro and this is what the US president intends to do first. Secretary of State Marco Rubio pointed to this, that most of the Opposition is now outside Venezuela, sadly. Rubio told NBC Meet the Press - “We are dealing with the immediate reality. The immediate reality is that, unfortunately and sadly, but unfortunately the vast majority of the opposition is no longer present inside of Venezuela. In 2017 Facebook noticed a drop in user engagement- fewer comments, fewer posts, and less sharing. To address this Facebook made a change in its algorithm, which is a bunch of mathematical equations which determine what you see in the newsfeed. The result says this WSJ Facebook Investigation was to make Facebook an angrier place, a place where divisive comments were being posted, and sensational or exaggerated comments were being shared. This increased the level of divisiveness in the US during the early period of the Trump administration. As America looks back on this time- the issues related to migration across its southern border that are still alive today and on which there is now a consensus across Democrats and Republicans on returning migrants. The issues related to the urban-rural divide that many presidents preceding Trump and Biden had chosen to ignore, and which the Tech community showed little interest in. The divide also across educational lines with college educated splitting away from people lacking college education just as costs of college had soared. All these issues were out in the open and instead of having an educated debate these algorithms never intended for solving social problems actually made them worse.  It is now in the interest of both Republicans and Democrats to take a hard look at what went wrong and restore the civility and dialogue that marked American experience across all ages and income groups, and remove the overstated influence of such algorithm based apps. The WSJ Facebook Investigation is a way to restore the traditional media's true place in the national dialogue and push back against the insidious and dangerous influence of algorithm based news feeds such as this one.  Outrage Algortihms may be good for a few people and a few in tech  business in California and in capital markets in New York, yet they are bad for America and the American people as a whole, bad for the vast landscape of America and the vast majority of the American people. Mindless infatuation with pictures of young adults leads to a mindless and dangerous result in mental health, bad effects on women, illusions about what is right living, and increasing divisiveness in America.  ...

Each year malaria kills 500,000 people mostly in Africa, including 260,000 children. A vaccine jab for malaria has finally been developed. It took 34 years for Dr. Ripley Ballou, 70 years, to refine the technology while working at GSK in Britain to get this done. Ballou himself had malaria and could experience its debilitating effect. This made him resolve to find a solution. The vaccine jab is the first for a parasite and the first developed from scratch for African children. Its effectiveness wanes over months so that its use is intended for the rainy season. By giving 3 shots just before the rainy season when malaria is at its peak it can reach 70% effectiveness, say British experts. It is cost effective as other prevention measures as nets over beds- it will cost about $2 -$10. When combined with other anti-malarial medicines it is about 90% effective. Its safety is proven after having given 2.3 million jabs of the vaccine in African countries. Experts estimate it will prevent 5.4 million cases of malaria, from Mali to Kenya, and from India to Indonesia where malaria is still a danger. Malaria can repeat itself many times for the same person over a lifetime, increasing the health risks and damage to health. The vaccine was developed using technology that produces a protein that is also found on the outside of the malaria parasites in the early stages of its lifecycle. It exposes the immune system of a person to this protein to build up immune defenses. This British discovery will help African  Asian, and Latin American countries build confidence in their health systems ability to cope with dangerous diseases. In doing this it will improve the quality of life and combined with other health actions provide a better life in the poorest countries.   ...

Malaria in India is a notifiable disease in 33 states. It is part of India's vision to be malaria free by 2027 and to eliminate the disease by 2030. Health Minister Mandaviya says it is a public social and economic challenge in India. And that India witnessed 85% decline in malaria cases in 2020 compared to 2019, and 84% decline in deaths during 2015-2022.

Reform UK is now leading party with 34% of the vote to Labour's 25% and Liberals 11%. The report in the WSJ on Augu 28 shows how the Labour government did not live up to it's talk on immigration. It also shows how the Conservatives and Boris Johnson failed by opening up non EU immigration from Asia on the grounds that it would bring in the brightest and yet dropped the basic college degree requirement paradoxically or carelessly. Lobbying from health care home care hugely increased migration for this field under Conservatives and is only now being reversed by Labour. Labour has been too slow and the culture of Britain and Labour has not changed enough to grasp the problem. Their are vested interests in Britain such as universities and home care health care that have influenced the conduct of policy so that migration on non-eu has replaced eu migration after Brexit, but not attracted the most qualified immigrants. The 4% of the British population that entered Britain after Brexit as immigrants, millions arrived and now when Labour is trying to bring this down faces a large number of dependent applications. University students are now bringing in their dependents at rates that have skyrocketed. ...

The experience of Pintung county in Taiwan with a coronvirus outbreak is shown in The Guardian. Home quarantining of a couple from Peru leads to an outbreak. Community cooperation, a speedy response from local health authorities, testing and quarantining help to limit and control the outbreak. When it started this county on the coast, one of the poorest in the country, had limited health resources and no one was vaccinated. Most of the control response was through testing and quarantine, with vaccination assistance from the Taiwan government. Local people say strict quarantine is essential as the source of infections was from a couple who visited Peru and were allowed to home isolate. Taiwan government is now enforcing quarantine at hotels for all persons returning from overseas. WHO Emergencies program head, Dr Michael Ryan, says it is important to act quickly. He says "Be fast, have no regrets. You must be the first mover. The virus will always get you if you don't move quickly."  This happened in this Taiwan county as the local health authorites setup a soft lockdown of the two affected villages within 3 days of detecting the first case of Delta variant. This was announced over loudspeaker and food plus daily necessities were delivered to each household. A command centre was setup to take and coordinate the action daily. Taiwan authorites have enforced repeat testing and extended testing. ...

For cases to double Kerala has 72 days, compared to a national average of 8 days.
