The Times correspondent in Beijing says he sees two Chinas one that is showing technological advancement in 5G, in space technologies, in information technologies, infrastructure building in big cities. The other China is in rural areas away from the big cities, in smaller towns in regions away from Beijing and Shanghai. These areas have suffered neglect and have changed little over decades, with the focus during industrialization on larger cities and the coastal areas. This is evident in the manner health services infrastructure and development of medical personnel and doctors were neglected in the rush towards building manufacturing and infrastructure of road and rail. There is a shortage of doctors and hospitals, health services are costly, and waiting lists for beds at hospitals is huge. Doctors are also not held in high esteem because the focus is on profit in the market economy that has taken over the health sector.  Education of citizenry on respecting the common welfare which is expected and normal in Europe and America has also suffered during the rush to industrialization. Efforts to ban and eliminate use of certain wild animal foods not being respected by fellow citizens can be seen in this context. This caused the SARS virus epidemic and the epidemic today from the coronavirus. Seen from this angle a slowdown in construction, infrastructure building, and a slowdown in the economy, can even be healthy, so that focus can be shifted to better health, better sanitation, and better medical infrastructure including medical human resource capabilities. Investment in public education on health and self enforcement of rules by citizenry for a better society is indispensable for progress. It is in these conditions that the challenge of the national and international emergency of the coronavirus can be seen today. It also provides an opportunity to reflect on progress so far and the needs of the future. These challenges are even tougher than repeating what one has done before such as building more and more infrastructure, as they involve building a better society through public discipline along with investment in health and education services. This provides a lesson for many Asian, Latin American and African countries in the rush to industrialization. Turning over the health sector to a market economy making services costly and distributed unevenly in the population has not worked in the U.S. for the betterment of all citizens and a healthy society. Carrying this over from U.S. to China and not learning from Europe in what Europe has done well in the less costly and broad distribution of health services for the people, is one of the poorer lessons learned. This is also true for India and South Asia, South East Asian countries in their rush towards industrialization.  ...

About 20% of people over 65 years work today compared to 15% 35 years ago, says Pew Research, and of these working 65 year olds two thirds are working full time compared to half in 1987. They are wealthier having median net worth of $410,000 today compared to $282,000 in 2010 much of the 45% gain made in the last 2 years from rising house prices and stock investments. Chile's new constitution was drafted after a vote on a constitutional convention in which many people not involved before and on the fringes of society including indigenous groups were elected. More than half are independents and from different groups of society not well represented before. As a result of this the lack of experience has led to enshrining every single right that one could think of instead of focusing on what the protests were about about - the pension system, unfairness in access to health housing and education in the way funds are allocated from the budget, and promoting fairness in government.  At this time there is no certainty that it will be passed. A 36 year old student protest leader Gabriel Boric is the president elected after two years of protests about the unfairness of the system that took shape under a constitution written by a military dictator Pinochet who ran the country from 1973 to 1990. Pinochet came to power in a coup that is common in the history of South American Republics such as Brazil, Argentina, Chile and other countries. During the period of the government of Marxist president Salvador Allende the country was polarized resulting in the military taking over.  If the constitution is rejected a new convention will be formed to write a new constitution. In following the Structure of the US, Canadian, or British constitution, or the Indian it is important to look at the document so that it will stand the test of time rather than simply enumerating the rights of man. It is also important to focus on how to make the basic rights such as food, housing and education be well funded and society to be run along lines of basic fairness in incomes, while protecting enterprise and industry that can create new wealth for the country. The large South American economies are mainly dependent on commodities for export and there is a need to fund new business sectors which are not in commodity products- copper in the case of Chile- so that the economy can develop in a way that funds progress in incomes. ...

Van Dam says its not that great being a worker in the U.S. because it is hard for the unemployed resulting from competing with workers in other countries with lower wages, and for those who are unemployed harder because worker collective bargaining is weakened over 3 decades. He cites a 296 page OECD report showing very little government support for unemployed and at risk American workers. It says this has contributed to higher income inequality and larger share of lower income people than almost any other advanced a nation. Only Spain and Greece are shown as having more households earning less than half the median income- showing large numbers of people are poor or close to being poor. In the U.S. an average of 1 in 5 lose their jobs each year, and 23% of workers 15 to 64 are in their job less than a year in 2016. The job churn hurts workers because of firing and layoffs being frequent, more than is healthy for a economy. The U.S. and Mexico are the only two countries not requiring advance notice before firings. And fewer than half of workers find a job within a year in the U.S. Two in three families with a displaced worker fall in poverty for some time. Unemployed workers with typically 26 weeks support get less support than any other country in the study. Only 12% of workers in U.S. are covered by collective bargaining. ...

Liz Whitehurst is one of many young people who are giving up jobs in offices to take to farming. They are not from farm families and bring a new way and exciting way of looking at farming free of the pesticides and other practices common today. Only 2% of U.S. land is being used for growing fruits and vegetables, according to the Union of American Scientists cited in the Guardian newspaper, and this needs to at least double in acreage if American needs are to be met. Only 15% of Americans get the daily requirement for fruits and vegetables- so desperately needed is this  to lower the BMI of the 70% of overweight Americans with BMI over 50. In the light of this crisis the shift of young people to farming is an encouraging sign.  In 2015 Liz, 32 years, decided to buy a 3 acre farm in Upper Marboro, Md, giving up benefits and better pay at nonprofit jobs in Washington state.  Here she is shown picking up Aragula leaves in the November chill. She is not alone. She is joining a movement that is bringing highly educated, former urban first time farmers as the demand for better food, for local and sustainable food, especially fruits and vegetables grows in the U.S. Year on Year there is a 20% increase of farmers in states like California, Nebraska, South Dakota in the 25-34 age group. In the 2014 USDA Census this group is growing at 2-3% just when other groups are shrinking by double digits. These farmers are more likely to connect with the community supported agriculture (CSA) prorams and markets, to grow organically and limit pesticide and fertilizer use. They tend to have farms less than 50 acres. Liz leases the house and the fields from a neighboring couple in the 70's, growing organically certified peppers, cabbages, tomatoes and salad greens kale to aragula, rotating fields. On Tues, Thurs. and Fri. she and two friends are to be seen waking up in the early hours of darkness to kneel in mud and cut the greens. What motivates them is having a positive impact, to do that so it is immediate and you can see it making a difference, says Liz. Still young farmers face many hurdles, including student loan debt, and finding ways to meet the larger needs for online grocery service or the grocery chains. Yet a trend is taking shape for small and middle farms that provides some optimism as the number of farmers shrink significantly overall. Most alarmingly it is the lack of national and local policies to meet the health crisis of rising BMI's right at this level of local farms and community farms for local produce. Lack of any consciousness about this, even though good health in the U.S. as in other countries has always rested on what you are eating, long before processed foods became the norm this is the way the world met nutrition needs.    ...

A report made by a presidential commission led by Robert Kennedy Jr shows the dire situation for health of American children. The report assails a faulty medical system which invests too much in research on diseases and does little on understanding and working on prevention of diseases. The basic outlines of new health care legislation takes shape as Senators Dodd and Kenedy come up with a plan that scales down subsidies to low-income people to buy insurance. Attached to their revised outline is a budget office analysis thatprojects the plan costing $611 billion over 10 years and with expected changes from the Senate Finance Committee would cover 97 percent of all Americans. And earlier plan received much criticism because the Congressional Budget Office estimated its cost at $1 trillion over 10 years and left 37 million Americans uninsured. In addition there is the revised Medicaid expansions for aid to the poor that would add a couple of humndred billion dollars to the total tab. The administration's goal is to keep the cost down to $1 trillion over 10 years. The legislation as it stands includes the public option which is designed to control insurance costs. Mr. Obama said this week that "the public option would keep insurance companies honest." Employer mandated insurance is part of the Kennedy-Dodd legislation proposal. Employers with 25 or more workerswould have to provide coverage or pay the government an annual fee of $750 for each full-time worker and $375 for each part-time worker. The government pays the startup costs for the public insurance option as a loan to be repaid, and premiums would make the option self-sufficient....

US law now supports turning back migrants at the border. Under public health policy Title 42 migrants are denied asylum, a federal appeals court ruling supporting this. It is also now supported by the Biden administration. It is a key tool used by both the Trump and Biden administrations to combat illegal border crossings. With the rushed approach adopted by the Trump administration not enough consideration was given to winning support in the House from 25 conservatives in the Freedom Caucus. Without their support the bill cannot be passed in the House of Representatives. The fight also includes one over what are essential health benefits including whether  maternity care would be included. As a result some moderate Republicans are also expressing opposition on the grounds that less people will be covered and fewer benefits will be provided under the Republican House plan called AHCA. President Trump has not involved himself in the details, and the bill comes very early in the first 100 days, leading to the perception that health care has become a partisan conflict without really grappling with the problems of high cost of health care and creating a solution that all can support. Democrats are seen as having made the same error early in Obama administration's first term. In 2017 Facebook noticed a drop in user engagement- fewer comments, fewer posts, and less sharing. To address this Facebook made a change in its algorithm, which is a bunch of mathematical equations which determine what you see in the newsfeed. The result says this WSJ Facebook Investigation was to make Facebook an angrier place, a place where divisive comments were being posted, and sensational or exaggerated comments were being shared. This increased the level of divisiveness in the US during the early period of the Trump administration. As America looks back on this time- the issues related to migration across its southern border that are still alive today and on which there is now a consensus across Democrats and Republicans on returning migrants. The issues related to the urban-rural divide that many presidents preceding Trump and Biden had chosen to ignore, and which the Tech community showed little interest in. The divide also across educational lines with college educated splitting away from people lacking college education just as costs of college had soared. All these issues were out in the open and instead of having an educated debate these algorithms never intended for solving social problems actually made them worse.  It is now in the interest of both Republicans and Democrats to take a hard look at what went wrong and restore the civility and dialogue that marked American experience across all ages and income groups, and remove the overstated influence of such algorithm based apps. The WSJ Facebook Investigation is a way to restore the traditional media's true place in the national dialogue and push back against the insidious and dangerous influence of algorithm based news feeds such as this one.  Outrage Algortihms may be good for a few people and a few in tech  business in California and in capital markets in New York, yet they are bad for America and the American people as a whole, bad for the vast landscape of America and the vast majority of the American people. Mindless infatuation with pictures of young adults leads to a mindless and dangerous result in mental health, bad effects on women, illusions about what is right living, and increasing divisiveness in America.  ...

The contrast between modernizing, developing East and South Asia ( from Mumbai to Shanghai) with war torn desolate West Asia (from Tehran and Baghdad to Kabul and Islamabad) is so striking today that it is something to reflect upon for wisdom and understanding. UAE support for Sudan's RSF Rapid Strike Force and Saudi support for the military - fracturing of Sudan, errors piled on errors led to the civil war in Sudan. A civil war in a country neighboring Saudi Arabia just across the Red Sea. Saudis and UAE were on opposite sides briefly after UAE pulled out of Sudan, UAE acting in this way to object against Saudis requesting US sanctions on UAE.  Once close partners have moved apart as they spread their influence in different conflicts in the Middle East.  This has not created a region that can grow economically without the disruptions of conflict in the way other parts of Asia have emerged to modernize the countries as in Taiwan, Korea, China and India. In neighboring Pakistan another conflict has emerged as partners split, with looming conflict between Afghanistan and Pakistan. Yemeni Houthis are in conflict with the US and affect the Persian Gulf shipping lanes.  Iran with it's pursuit of weapons programs and nuclear weapons is using capital that is badly needed to improve the economic situation on arms buildup for the regime and for allies in Lebanon and Yemen, leading to protests and crisis. In this way the Middle East has failed to use oil wealth to modernize the entire region. Much of it was wasted in Iraq and now in Iran by policies that led to war and regional conflicts not modernization and technological transformation that has happened in Asia. The US has inadvertently becoming a partner to this as when the Obama administration helped fund Iran's economic rebuilding which was instead used to fund the military, and before that the Reagan administration support for Iraqi socialist ideology regime. The challenge for China was how to modernize after the Japanese invasion and civil war. In Korea it was how to modernize after the civil war. In India it is how to modernize with a smaller neighboring country Pakistan promoting terrorism and wars now with China's support. In Asia all these challenges were and are being met to steadily and persistently modernize to European standards with a singleminded focus and determination to meet the aspirations of the people with the US business working alongside Taiwanese, Korean, Chinese, and Indian governments and private industry. In West Asia various ideological (Iraq), military (Pakistan), religious Shiite (Iran), religious + modernizing (Saudi +UAE) with erratic leaders and little representation of the people, has destroyed the tranquillity of the region and destroyed democratic forms of government, destroyed bottom up education and health of the population except for priviliged groups in countries in the region of West Asia. Involvement of US and Europe or Russia in West Asia has led to distintegration of Soviet Union (Boris Yeltsin) and deindustrialization of US and Europe (Reagan, Bush, Clinton, Bush, Obama administrations) with business shipping out manufacturing to China while wars engaged the attention of American and European elites in Iraq, Iran, Pakistan, Afghanistan. The entire west Asian scene for 1950-2030 has been a disaster, one massive disaster for all involved. The contrast with East Asia and South Asia reminds one of the words from Robert Frost of New England in Mowing- that reflects on the enduring value of honest labour. "My long scythe whispered to the ground. What was it it whispered? It was no dream of the gift of idle hours, or easy gold at the hand of fay or elf: anything less would have seemed too weak to the earnest love that laid the swale in rows. Isaacson's new book on Musk says Musk's story is a cautionary tale. The compulsion to be drama magnet or mean are not prized traits, say others.  More sinister (the hell) is celebrating a culture that does not respect the need for worklife balance, of respect for nutrition and exercize for good health. Musk's methods which he calls "algorithm", a word known more for obscurity than meaning, are nothing new. For years as the US, Japan and China, now India have innovated there is a focus on simplifying things, and to do this questioning the existing way of doing things by breaking down the existing method into pieces and reorganizing the pieces of the puzzle leaving out unneeded pieces. What is the key to his success as it was for Jobs at Apple is creating a culture in which people would invest in and take risks for innovation at the high end of the price spectrum. Jobs used design and new stuff like the iPod and iPad, iPhone to do this. Musk does this through playing the role of a social media icon but a dangerous one that does not respect worklife balance and good health habits of nutrition, exercize and mindfulness. In processes this can give you a process that takes less time and money- how India's moon mission and rover Chandrayan 3 was done for $78 million showing these work practices of Musk are nothing new, and universally adopted by successful companies and nations. Newer ask your employees to do what you would not do, is also adopted by the best managers. By turning it into a mantra it obscures the fact that America today is a country of massive inequalities where two thirds of 4th graders cannot pass ACT reading test and half of retirees have zero savings, working people and families face a cost of living and health crisis and are badly neglected. How does it help to role model as an icon and popularize a culture that tolerates and accepts such conditions that would leave men deeply troubled, including America's leaders Washington, Lincoln and FDR if they were alive today. ...

US president Biden announces a plan that makes it mandatory for all employers with more than 100 employees to require their workers to be vaccinated. The Labor Department's Occupational Safety and Health Administration will issue an emergency temporary standard for implementing the new rule. This will cover 80 million private sector employees. Businesses that fail to meet the standard will face fines of up to $14,000 per violation. Employers must also give workers paid time off to get vaccinated or recover from any side effects.

Federal government workers and contractors also must meet the new requirement or face regular testing. In all 100 million workers or two thirds of American workers will be covered. 

To meet testing needs the Biden administration is procuring $2 billion in rapid point of care and over the counter at home Covid tests, using the Defense Production Act for accelerated production.

It must be of the highest importance that all people in America in every region of the country be treated in the same way, not leaving chemicals to be dumped in areas that are poor or neglected- as more cases are seen this report says in the cities and towns along the Mississippi river, in southeastern states, and it shows the impact of trace chemicals of nickel, arsenic, and chromium from industrial plants, chemical plants, and coal production. Forever chemicals can be found as some reports show also in peaceful looking landscapes in America- such are the dangers unless the people of America insist that their leaders fight these wars before culture wars and over other issues not central to the welfare of all Americans in every part of this Nation. ...

Millets are small seeded grass grown since ancient times in India and Africa that have the advantage during climate change of being resilient to drought, adverse weather patterns, require less water, and provide high nutritional value. Einer Elhauge, professsor of law and founding director of the Petrie-Flom Center in Health Law Policy at Harvard University, says there are limits to Congressional powers under the Commerce clause such as enacting a law requiring Americans to eat broccoli. If the health care law required all Americans to subject themselves to health care this would be the case. But the law only requires all to buy insurance so that insurance can cover the costs of healthcare for all. He points to the "necessary and proper clause" as also authorizing the health care mandate. That clause gives Congress the power to pass laws that are "rationally related" to execution of constitutional powers, such as criminalizing the interference with mail, on the basis of the constitutional power to setup post offices. In this case the health care law mandate is related to the constitutional power to regulate premiums and prohibit rejecting the sick, says Elhauge.

Governor Newsom of California says he will seek federal authority to setup a state single payer plan for health insurance. An expert at Georgetown University says there is a new burst of energy for broader coverage or universal health coverage. States with new governors or seeing this momentum are California, Washington, New Mexico, Maine, Nevada, Delaware, Oregon, Connecticut, Wisconsin, Michigan, Illinois. Polls show a large majority of Americans now support Medicare for All as a form of universal health coverage for all Americans, a system that prevalent in Canada for the last 50 years. Two hearings will be held in Congress on Medicare for All in 2019.
