US health care is not working. In UK and Canada the sytem is regulated in a way doctors and hospitals work togerhter to keep costs down. This is not happening in the US. insurers and doctors are pitted against each other and patients are in the middle. Insurers are also running about 70% of self funded employer plans that pay insurers a fee and are the responsibility of employers. Insurers then cut costs and do this for employers. Sel funded means employers pay cost of claims insurers merely get a fee. Patients are caught in the middle between Insurers and doctors/hospitals. The insurers cutting costs overbilling Medicare and skimping on payments, doctors trying to overcharge. Christopher Whaley, an associate professor at Brown University’s School of Public Health says- “We have a system of whack-a-mole, with providers trying to get paid and insurers trying to constrain costs—patients are caught in the middle,”  Obama exchanges to get people covered never thought through to come up with a better system and is a Band Aid. At some point the whole system ought to be scrapped and one built from scratch and designed to work so that hospitals and doctors provide best service at lower costs.   ...

The social media site Twitter frequently loses money and took on $13 billion in debt says this report in the NYT. Musk faces financial challenges a he takes over the company, and this report says the company would be unlikely to invest and simply be paying the bills. Twitter it says, has not made a profit for 8 of its past 10 years. 

Twitter's interest expense alone would grow from $50 million to $1 billion with the new debt. Company operations generated about $630 million in cash flow to fund operations. Twitter had $6 billion in cash before the buyout and a large part of this would have gone to close the acquisition. All the financial resources of the company will now go to service the debt. About $1.2 billion in marketing expenses mostly salaries are likely to be cut. And layoffs of a significant fraction of the total employees may be a way to conserve capital to service the added debt, this report shows.

Safety checks at Norfolk Southern are the subject of this report in the WSJ. The derailment of a freight train leading to spill of toxic chemicals in Ohio on Feb 3, 2023, led to concern about the safety practices at American railroads in the efforts to cut costs. The National Transportation Safety Board has opened a probe into the safety culture and practices of Norfolk Southern. One practice called PSR or precison scheduled railroading has been used by railroads in America to cut costs. This means fewer employees, longer freight trains as long as 3 miles, and fewer locomotives. The result is that employees count went down from 159,000 in 2011 to 115,000 in 2021 a drop of 28% even when freight dropped by 11%. This increased profits resulting in higher dividends and stock buybacks, with a heavier workload and less time off  even for sick days until a threatened strike. President Biden intervened to set a new pay and work deal and an element of fairness for workers. ...

Women voters made up 53% of the voters in the 2012 election. The 2016 U.S. presidential election in unique because of the first women candidate for president with advantage in the women's vote. Also unique is the split between the college educated and the non college educated voter, something that has into happened in an election in this way for a long time. A pro business party such as Republicans finds itself with a significant part of the non college educated vote.  Another difference is the huge turnout of Hispanic voters, and questions raised about how their loyalty may have shifted for decades with Republican positions on immigration and deportation. The demographics have also shifted with college educated voters in suburbs of southern states such as Georgia, Colorado and North Carolina. States that once dominated the national election such as Ohio with a larger number of non college educated voters are now not as significant as in the past. If infection rates are not down from the over 1 R rate and the transmission increases a national lockdown is considered as the next step. Ajami makes the point that opinions and attitudes -after the Obama efforts to improve America's standing in the Muslim world - havent changed much since the Bush days. He cites the Pew Global Attitudes Survey for 2009- In Turkey after the Obama Anakra visit favorable rating is only 14%, 69% are unreconciled. In Egypt 27% have favorable view 70% do not, in Pakistan unfavorables actally went up from 63% to 68%. He also points to the situation in Iran where the protester for the fraud in the election of Ahmadinejad did not receive much supprt from Obama, as the Obama administration decided to engage with Ahmadinejad to achieve nuclear settlement. In effect the rhetoric from Obama has not been matched with courage of convicitions , and lacks the courage to turn a new chapter by breaking from the past not just with talk but in real policy changes. And says Ajami the Arabs havve stopped listening to the rhetoric as little has been accomplished by way of change. At the same time false expectations may have been aroused because the Cairo speech was made at the University with the aging Mubarak at Obama's side, and beyond addressing these students the feeling clearly must be that the US would simply continue its policies of supporting old regimes that tolerate no dissent of any kind such as Mubarak's. ...

As Trump tries to gain the support of black voters with his visit to Detroit, the questions remain say people in Detroit.  His alienation of minorities from the primaries is not forgotten, and the lack of underlying empathy is why some experts say this is not resonating in the last 50 days before the election. Another problem is that presenting blacks in a bleak state is not seen as showing respect because Trump was absent during the struggles Detroit went through since 2008 down to street lighting and schools, foreclosures, and is only here now that the Michigan and Detroit economy has recovered to a considerable degree. Here Vanessa Williams of WP says there is a near universal condemnation of this kind of talk such as "what do you have to lose," as seeing blacks lacking the ability to think about where they were and where they are now, and the path ahead in clear terms as whites or Asian Americans are able to do. Democrats point to the gains for blacks in declining unemployment, some of the issues of inner cities not responding to either party's policies, improvement in health insurance, and access to voting rolls, and in the Michigan economy the rising tide lifting all boats with a booming auto industry. Who is Ed Whitacre? What is he like and where is he from? Ed Whitacre headed Southwestern Bell or SBC, which he merged with AT&T. Bored as a retiree in San Antonio after leaving AT&T, he took the job at GM. He golfs, wishes and hunts with his chocolate Labrador retriever at a ranch near his house in San Antonio. He is impatient by nature and likes to see things done. Managers who worked with him at Southwestern Bell say while they were working on day to day business, Whitacre would be the one thinking ahead, trying to figure out how to compete in the future, and the things that were likely to happen in the changing environment. For a smaller Bell he saw that it was simply whether his Bell would be acquired or whether he would acquire other Bell companies. He is a hands-on guy who like to do things himself, like running a bulldozer around his ranch, one of the things Whitacre likes to do. His beginnings are in small town Texas. The place is a sleepy railroad town called Ennis, Texas, where for 50 years his father was a locomotive engineer. Whitacre says his father had never finished high school, and he did not want Whitacre working for the railroad. Both his parents insisted that he get acollege degree. Whitacre went to Texas Tech in Lubbock, Texas, because the tution was only $75, and landed a job at Southwestern Bell in 1963 as a facility engineer. And he stayed with the company all the way- with 19 moves living in Texas, Arkansas, Missouri and Kansas- till it became the new AT&T. Frost, a retired San Antonio banker and a member of Southwestern Bell's Board in 1990 when Whitacre became CEO, says Whitacre started from the bottom, and literally, even climbing telephone poles. So it isn't surprising that this guy walks around the GM Renaissance Center, talks to GM employees, tries out a Taco at the Food Court at the Renn center (says its OK but not like Texas tacos), and uses all elevators like everybody else, unlike GM executives who equiped elevators so they could bypass floors. And he isn't hesitant to wear jeans and a sweat shirt while visiting a factory, which he says is all the clean clothing he had at the hotel. Now he has an apartment. Works 14 hours a day, 5-6 days a week, and has his phone ringing just when he hopes to leave town to escape for a weekend. ...

Do you ever wonder how Japan keeps it streets so clean. There are no litter bins or street sweepers to be seen. This report in the BBC News looks at Japan's cleanliness ethic starting from school days for children growing up with the idea that clean is what you make happen with your own two hands and a broom. That is all it takes and a sense of personal responsibility infused into the culture from school days as children.  For 12 years of school life from elementary school to high school cleaning the school is part of the school routine for students. The social consciousness was developed in this way and children as they grow up learn to take pride in their cleanliness and in the cleanliness of their surroundings. This carries over to cleaning up the neighborhood.  In India's Swachh Clean India campaign their are street clean sweepers in addition to people themselves taking on the job of cleaning. This is ok for public facilities like railway stations underbridges and other public facilities, but for neighborhoods and schools making cleaning a part of the routine in schools is a good idea that needs to be universally adopted as part of Swachh India, Clean India campaign. This also holds true for all Asian and Latin American, African nations which could learn from the keeping the country clean efforts of Japan and more recently India. As India shows not having done this well before is no reason for discouragement, getting started and keeping it going, building public awareness and support is the key. This is particularly true for developing countries because it is easier to prevent illness and disease by increasing levels of hygiene and sanitation, saving hundreds of millions of dollars for large countries like India and Brazil, Mexico, Nigeria, for days and productivity lost. It also pushes countries to the next stage of development faster through infrastructure development and quality of public services, quality of life.    ...

Full time workers in Sweden have the right to take leave for 6 months to do something they want to do, including starting a new business. This includes taking care of a relative or study leave. Workers can come back after 6 months to resume their old job. Employers cannot say no except for special reasons.  This leave has the support of employers, unions and the government. In Sweden's unique culture which emphasizes work-life balance workers taking 6 month leave of absence can get back to their old jobs smoothly.  In Sweden it is very hard to fire an employee who is permanent. So that once an employee has permanent status there is very little incentive to leave to do something else. The 6 month leave option lets workers try out something different or start a business without incurring career risk in addition to financial risk. Workers are more comfortable venturing out when they know they can come back to their old job and are not risking their careers. This BBC report shows profiles several new business owners who took 6 month leave to start a new venture they were passionate about. The way this happens is that an employee first tries out an idea in his spare time while he works full time. When he is comfortable making the move he can take the 6 month leave to devote time to it full time. Experts say Sweden is the only country in the world to give this right to leave to start a new business. In 2017 175,000 people took this leave compared to 163,000 in 2007, according to Statistics Sweden. During this period registration of new companies shot up from 27,000 to 48,000. ...

Egypt plans to tackle the financial crisis after the pandemic and the war in Ukraine by increasing natural gas exports by one third. It has the LNG terminals to do this which are underutilized. The LNG could be exported to Asia or Europe at ten times the price buyers in Egypt pay for it. The way this additional natural gas is to be exported is to impose 15% cut in use of natural gas in Egypt similar to what the European Union has done with its 15% mandated reduction. This will then be diverted to LNG terminals. The max temperature for air conditioning is 25 degrees under the new plan and lights are dimmed or shut off after 11 pm in streets, shops and malls.  The war in Ukraine has doubled the price of wheat and other basic food necessities imported from Ukraine and Russia. This put a heavy burden on state finances in Egypt with subsidies on bread and other food for 70 million people out of 102 million people. Investment needs are also affected. Saudi Arabia has stepped in with help as no IMF program has been set. A 14% devaluation of the currency took place in 2022 and another devaluation of the currency is expected. ...

This is what is left after decades of civil war. The intervention of local and foreign powers in the Arab world has led to huge breakdowns in society, and massive destruction in countries facing civil war such as Syria, Iraq, Yemen, Libya. This is the kind of situation that happened in China's northern and southern provinces in the 1930's, and 1940's. A new pathway for peaceful development outside of ideologies and socialist movements, pan arabist or religious movements need to be found for the region, the Arab world in North Africa to the Middle East. China, Korea, and Vietnam found a way out after civil wars over 1940-1975. India is putting out a map for a course of action with Vikshit Bharat  or Developed India after two centuries of stagnation under the British Empire 1756-1947, and 5 decades of stagnation under the Congress Raj 1947-2000. The spirit of development and desire for modernization, for modern science and technology, good governance and institutions, must come from within.   ...

The U.S. is moving quickly to gain access to an effective vaccine by September or October 2020. It is doing this by providing the money for companies to conduct trials and ramp up manufacturing in a big way. The U.S. government has agreed to give Astra Zeneca upto $1.2 billion to secure supply of a Oxford University developed vaccine which could be ready by October. Astra Zeneca has agreed to make the vaccine under a licensing deal with Oxford University's Jenner Institute and promised it will not make a profit on this. U.S. has also given $483 million to Moderna in Cambridge, Massachusetts, for making the vaccine. Both Oxford and Moderna are testing the vaccine on humans. Oxford uses a tested older technology, Moderna a new technology. UK has given Astra Zeneca $79 million to secure 100 million doses of the vaccine, with 30 million ready by September. Oxford is also in negotiations with Gavi the international vaccine alliance, and Coalition for Epidemic Preparedness Innovations on further deals to boost production. Oxford began a 1100 person study in April, and is  doing a 5000 person trial in late May.  ...

Changes Novartis CEO Daniel Vasella is making in the pharmaceutical business. He has hired Joe Jimenez, who is running Novartis's consumer health care business to be the new pharmaceutical division chief. Jimenez previously worked at packaged goods companies including H.J. Heinz Company. Jimenez is cutting 25% of the jobs at pharmaceutical division's headquarters in Basel to reduce bureaucracy and costs. In March he promoted Trevor Mundel and Andrin Oswald, 2 young executives, to head the drug developmet group which puts drugs through human testing and submits them for regulatory approval. This group had become too bureaucratic and slow to move and take initiative. To improve its functioning Jimenez is organizing it into small teams with each team assigned an experimental drug in Novartis's pipeline. Each team of 8 people including physicians, experts in regulatory affairs and marketing and toxicologists work together to spot potential safety issues early and discuss them with regulators to determine whether to put the drug through expensive clinical trials. Each team takes the responsibility to take its drug to the market. The pharmaceutical unit is also being organized to be more nimble. It solicits health systems early on whether its willing to pay for drugs. And Jimenez has startd 4 pilot projects in tough markets to improve relationships with payers, including the Pacific Northwest where Novartis has offered to train an HMO's nurses in aspects of heart disease. Vasella supports the generics division of Sandoz because the growth is in generics, with generics commanding 60% of the prescription volume in Germany and USA, and sales for generics up 25% this year in the generics division. And Novartis paid $39 billion for Alcon, a eye care company. Its also working aggressively in the vaccines business, which like generics enjoys double digit growth. ...

Three tankers held off the coast of Mumbai by India's Cost Guard have been sanctioned for carrying Iranian oil. India stopped buying Iranian oil in DJT's first term 2016-2020. In his second term DJT wanted India to stop buying sanctioned Russian oil as a way to reduce funding for Russia's invasion of Ukraine now in its fourth year. India has stopped buying Russian oil as part of the goodwill effort to reach trade agreements with the US, EU, and Germany. The seizing of the oil tankers is part of a new effort by India to support bringing Russia to the negotiating table to end the Ukraine war. Russia has demanded Ukraine turn over Donetsk region to end the war, which is a major stumbling block as Ukraine says there are Ukrainians living in Donetsk region. Germany's increase in its defense budget and investment in its armed forces has led to Germany+ (Germany plus UK and France) acting as the chief supporter of Ukraine, after the US has taken more of a neutral stand. The US basically wanting to end the war in 2026 so that the US can address the situation in the western hemisphere with drug and migrant trafficking gangs in Mexico, Venezuela and Columbia, and rebuild its economy to bring back manufacturing from China. For India the guiding principle of its foreign policy is Gandhiji's thinking and advice for fairness and peaceful coexistence - it does not believe in a British inspired NATO expanding on the borders of Russia, and at the same time does not see how a war on a neighboring Russian speaking region is in Russia's continued interest for a fourth year with bombing of energy infrastructure to leave Kviv in darkness. Non -alignment was Nehru's not Gandhiji's idea- the ideas of respect and fairness are basic to Gandhiji's thinking and India will remain true to his ideas in world relations. One aspect of this change in world affairs is missed by all and the media, that is that with the EU and US+ Japan, and India+ Indonesia there is a population of 1 billion of western peoples, and about 2 billion of Asian peoples, for a total of 3 billion people. This is a region three times the size of China, which with its access to capital and technology, labour and good governance is in a position to industrialize and reindustrialize, and bring manufacturing/science and technology to the core of this economic region by 2035. An industrialized India with 2X-3X the size of its current GDP will still be governed on Gandhiji's ideas for world relations in 2047. ...

Yet the pretensions to goodwill for the public cause is not supported by facts, facts that the AI companies have nothing to show for the dismal situation for Global Literacy that is the case today. Literacy in the US that is dismal with about two thirds of 4th graders not able to read and comprehend the English language at a level of proficiency in American schools. These AI purveyors care only for the money they can make using vast amounts of electricity for these servers, and pretensions for public purpose are intended to smooth their access to public resources not some genuine interest in whether kids can read, which requires the hard work of the teachers in the public and private schools of this Nation and others in Europe, Asia, Latin America and Africa. The Movement for Global Literacy is Lyrarc's effort to support reading and learning and Lyrarc serves this purpose without such massive funding and without charging for the public service to the Nation and to other Nations in the world community. Anthropic settlement of $1.5 billion at $3000 a book for its AI bots use of copyrighted books, can lead to future litigation for OpenAI model that consumes vast amounts of data. Anthropic was founded by siblings Daniela and Dario Amodei after leaving OpenAI in 2021 in San Francisco. It hired Google Books Turvey to scan books for its large language models on a massive scale to train Claude its version of OpenAI's ChatGPT.  An investment of $4 billion by Amazon and additional $2 billion by Google provided funding. In this way it is a competitor to Microsoft funded OpenAI which made early advances in AI.  This article in WSJ says by making the settlement for $1.5 billion Anthropic is trying to make it harder for Open AI to scan material easily without paying for the access and thus blocking it's rival.   ...

In this essay in Der Spiegel, Charles Hawley says that the Trump movement has become a movement of patriotic downtrodden whites, with a whole range of interests-of extreme right talk show hosts, Tea Party politicians, white power supremacists, those left out by globalization in the working class especially in the midwestern states. The danger he says is that this movement of which Trump has become a part, rejects the narrative on which America is based of the Constitution and the Founding Fathers establishing a country based on principles of "the inalienable rights of man," that have evolved through the years to include black people, women, and minorities.  To put this in perspective, president Obama writing for The Economist magazine in October 2016, puts this movement in a different context- that of the Alien and Sedition Acts of 1798, Know Nothing Movement of the 1800's, the anti-Asian sentiment in the late nineteenth and early twentieth century, periods when anti-immigrant or anti-foreign sentiment gained prominence. Obama's view is that it is not fundamentally economic. In this he is right in that some of the forces on the far right do not stem from globalization. Yet he would be missing a great deal if he did not address the economic problems for the middle and working class that have given such views the support of a broad segment of the population, especially in some midwestern and older industrial states compared to say the economy of California or New York. Obama is aware of the problems in his essay as he points to the problems of workers trying to get a decent wage, of job losses through globalization, and the aggravation of these problems by the financial crisis of 2008 when some of the potential physicists and engineers as he calls them went into the financial sector to create faulty mortgages. Yet he goes back to the free trade and global networks of supply chains as having reduced global poverty, without showing a keen awareness of how it has through a combination of events and decades of policy indifference to manufacturing communities in the U.S.- as documented by experts and shown in Lyrarc, with David Autor and Gordon Hansen in the WSJ, 2016- 08-16. A Gallup Study, WSJ, 2016-05-16, supports Obama's assertion by showing that many of Trump supporters are actually self-employed and not in economic distress. Yet the movement would not have taken its proportions without the merging of different groups particularly largely disadvantaged working class voters, and fortunately Hillary Clinton, Bernie Sanders, have a better sense of this than the president. It is by their efforts that income and wealth disparities can be tackled in a way that restores the social fusion of all parts of society- in Hillary Clinton's emphatic words in the final debate by "growing the middle," growing the middle class. The $17 discount for Russian oil to Brent crude is a result of the president's efforts with sanctions plus tariffs on China and India to cut oil purchases from Russia. This puts a strain on Russia in financing the war with Ukraine. Bothe China and India have cut purchases of Russian oil in recent weeks and the Indian refinery at Jamnagar no longer gets Russian oil, according to recent reports. DJT was criticized for his stance on Indian tariffs as inconsistent with the agreement with China on rare earths. It now appears that China and India have both agreed to stop financing the Russian war effort with big oil purchases and are shifting it to other places such as Brazil, Guyana and Canada. India plans large oil purchases and arms purchases from the US and this is part of the trade agreement being negotiated with India. About one third of the additional 240 million barrels of oil on the seas in tankers is Russian oil being stored for lack of buyers with total oil on waters at 1.4 billion barrels. This has led to a 48% increase in tanker costs to $125,000 a day. All this makes it harder for the Russian economy to sustain the war effort as the US pushes both sides to settle the Ukraine conflict in the 28 Points Peace Plan negotiated with important Russian negotiators in Washington DC over 3 days last week. ...

Paul Volcker sets up the Volcker Alliance foundation to support improvement in how government works at the federal, state and local levels. Volcker sees it as a catalyst for improving government and for renewing the spirit of public administration that pervaded America as it recovered from the Great Depression and World War II. Volcker's concerns for the situation in America in 2013 are: the lack of effectiveness of federal regulatory agencies, and the lack of the spirit of public service, the missing enthusiasm for work in public administration. The Volcker Alliance will be independent of academic institutions, and act as a catalyst, with senior people working alongside junior people and nonresident fellows. The initial budget is $5 million, some of it from Volcker's own account, and based at his offices in New York.

Supreme Court Justice Sandra Day O'Connor from Arizona was a pioneer of women's rights in the workplace, and had her own struggles before being the first woman appointed to the US Supreme Court. Born on a vast 200,000 acre cattle ranch in El Paso she was much of a cowgirl. She is considered to have blazed a trail for other women through her life and work.
