For a few weeks the British parliament had remote electronic voting. It worked well. Now traditionalists in the Conservative Party including Mr. Jacob Rees Mogg have restored the voting in person. This led to a queue half a mile long that stretched all the way outside parliament and in Westminster Hall. It seems like good common sense -surely studies come later that masks can cut coronavirus cases by 40%- as Texas is learning the hard way. As coronavirus cases jump in Texas the governor makes wearing face coverings or masks mandatory in the state. Texas recorded over 8000 cases in a single day on July 3, 2020. "wearing a face covering will help us to keep Texas open for business." As a grim warning to Texans he said "we are now at a point where the virus is spreading so fast there is little margin for error." As the virus cases surged Mr. Abbott, the governor of Texas, ordered all bars shut and cut restaurant capacity by 75% last week and reversed step taken to open the economy. Another lesson learned the hard way when it seems like common sense- consider that on June 20 as reported in the WSJ a staggering 500,000 people went to bars in Los Angeles county the day after bars reopened. It is this type of activity that makes Dr. Fauci, say cases could reach 100,000 a day in the U.S. Infection rates are now increasing in 40 of 50 states with the southern states, western states doing badly.  A lot of it was plain common sense. A German study shows a 40% reduction of coronavirus cases when masks or face coverings are worn. For those arguing for the reopening so that economic hurt is mitigated there is even more reason to wear masks as it makes it possible to get back to work by following strict social distancing and mask guidelines. Everything in life is about adapting and making small changes for the larger good. Younger people have badly failed to show fellow feeling with lack of following social distancing guidelines on beaches and gatherings leading to the numbers now showing that people 18-34 are now equally at risk. ...

Landlords and real estate brokers say only 10% to 20% of workers will return to their office when New York offices reopen after 3 months on June 22. Most companies are taking a careful approach and letting employees work from home or opening offices at reduced occupancy. Gradually this will increase from 20%. Yet offices are not expected to have a more normal look till Labor Day with people returning to work, and that too if schools have reopened.

There are concerns about public transit how many people it can accomodate with social distancing guidelines, and if more drivers lead to traffic jams.

Venki Ramakrishnan, head of the Royal Society and a Nobel laureate in Chemistry says that even with all the growing evidence of the importance of mask use Britain is way behind in mask use. Only one fourth of Britain's use masks compared to two thirds in the U.S. On public transport many people do not use masks in Britain.

The Royal Society has put together a scientific advisory group to look at the evidence on pandemics.

Ramakrishnan says it was once normal to have a few drinks and drive home, and normal to not wear seatbelts. Today this is considered not socially acceptable behaviour, the same has to happen for use of masks. It is as important as handwashing and social distancing, says the Royal Society.

Google has come out firmly on the side of parents struggling to deal with schooling at home for children, with social distancing during the pandemic, as it supports working from home all the way till summer of 2021. For 200,000 full time and contract Google employees is sure to offer some relief.  Sundar Pichai the Alphabet CEO made the decision after a debate in Google Leads a small group of executives at the company.  Mr. Pichai told staff " I hope this will offer the flexibility you need to balance work with taking care of yourselves and your loved ones over the next 12 months."  He was especially concerned about parents trying to tackle schooling of children. The surge of the pandemic in California where Google is located is likely to have convinced Google executives that this was the right step, with no vaccine in sight, and the possibility of a second wave after this one. In a nationwide television address prime minister Modi of India calls for a Janata Curfew on Sunday March 22, for a test of people's self-discipline in fighting the coronavirus. He urged people to not go outside of their homes from 7am to 9 pm voluntarily in this curfew. The aim is to build public consciousness of the need for social distancing and staying away from crowds and people gathering, as an effective way to prevent spread of coronavirus. Only health care workers and people who provide essential services would be outside. He asked that at 5 pm on that day people ring bells to thank health workers and others who are doing what is necessary in the health crisis.

The prime minister said there should be no hoarding as the government will ensure that there are adequate supplies. He said an Economic Response Task Force has been setup under the leadership of the Finance Minister that will decide on an economic relief package.

On one day alone 500,000 persons went to bars in Los Angeles County the day after after they reopened on June 20, according to some reports. The surge in California today is not something that just happened or fell suddenly from the sky.

In France after 72 cases were detected in the Quiberon peninsula the top regional official told about "the irresponsibility of young people vacationing or living here gathering in large numbers for festivities, ignoring the danger." Some of the people 18-25 years have with the risky behaviours increased the level of the dangers in this pandemic in many countries.

Prime minister Boris Johnson says he will proceed with "maximum caution" as he plans reopening of the economy. The reproduction R ratio for the virus is just below 1.0 and may have gone up recently, say public health officials. Britain faces a problem of deaths in care homes that is not under control.

The UK after a headstart in its vaccination drive has fallen behind other European countries and the US in the vaccination of children and teenagers. UK began vaccinating children in August far behind the US and Europe. On top of this UK under Boris Johnson decided to drop almost all public health restrictions during the summer. The change in Health Secretary happened on June 26, 2021 with Matt Hancock's resignation. The new Health Secretary Javid was to review the health restrictions in place till July 19. The sense of caution and preparedness that prevailed earlier as fallen short since July 2021 with the lack of coronavirus prevention measures such as masks, social distancing and vaccine mandates that were taken in Italy, France, Germany and other European countries, as well as in the US.

This editorial in The Guardian calls the new British Health Secretary Sajid Javid's actions after only 4 months on the job a bit of breathtaking cynicism, and a slap in the face of family physicians in the UK. In 2015 the Tory government promised to increase the number of family physicians by 5000, instead numbers have dropped today by 4.5% to 28096, even as demand has increased. The complete lack of sensitivity of the new Health Secretary to the level of burnout in family practice in the UK, and the difficulty attracting young doctors to family practice, has aroused a furious backlash in the UK. He has called for an end to social distancing and more face to face appointments when government policy had decreased face to face appointments following lockdowns. Britain has one of the lowest numbers of doctors per capita in Europe, says the Guardian, and that the government is tackling the wrong problem beggars belief.

Studies that show coronavirus cases are just as prevalent in the younger age groups 15-34 years as with older Germans. One such study uses data from the Robert Koch Institute and published in Eurosurveillance, a journal on infectious diseases. In this study scientists conclude that relative risk (RR) for coronavirus for residents of Germany 15-34 years, and particularly 20-24 years, was noticeably higher than in all other age groups. This study also determined that similar patterns emerged in South Korea where 20-29 years old age group had the highest number of detected cases.

This DW.com report attributes this higher coronavirus cases in younger age groups to work in high contact jobs in service industries, and to greater use of public transport. Laxity early on in following social distancing guidelines is also mentioned as a cause. Other causes could be greater attendance in sports stadiums and music venues.

A growing consensus among experts that it is not so much surface contamination as the air we breathe in that is the problem. The longer you spend time with people talking in face to face interactions, or inhaling air aerosolized in the air from small droplets, the easier it is to get the virus. This means social distancing matters in a big way. Masks are a big way to cut the risk. The worst you can do then is to be in a crowded event, ot have closeup face to face interactions for long periods. Being where there is loud talking or singing is a bad idea. This is why actions such as plexiglass installed as barrier, requiring masks in stores, using good ventilations systems and keeping windows has an enormous impact. This means that people can do a lot of work, exercize and other activities outside if one takes the basic precautions to heart and practices seriously. Contact tracing and testing, and other measures can be targeted to keep the spots that have a surge under control. ...

After meeting with Rishi Sunak, the Finance Minister and the Business Secretary Alok Sharma prime minister Johnson is keen on getting things to as normal as possible in the summer to save jobs. About 3 million jobs could be lost, some permanently if businesses cannot open by summer, especially in the hospitality industry. This could push unemployment up from 1.3 million  to 2 million. The Times reports that at one point in the meeting Johnson said "Christ!" In mid March after he had the virus and Britain saw a surge in cases Johnson made health a priority even quoting Cicero in Latin that the health of the nation was the priority. Now this is changing with the jobs situation becoming a major risk. Much of the loss of jobs is likely say experts in the red wall districts that Johnson took from the Labour party in the election, and the prime minister feels a special responsibility to these districts. Johnson also said at one point about youth unemployment 18 to 24 years that is bleak, that he wanted to have an apprenticeship for every young person. The critical R rate is higher in the northwest and the southwest of England, compared to London. In England overall it is between 0.7 and 0.9. It needs to be around 1 to avoid spreading the virus.  As part of the social distancing guidelines ministers are also discussing changing the distancing on public transport so that it is 1 metre instead of 2 metres, so that reopening to get back to close to normal can be done sooner.     ...

Summer tourism is helping support a second wave of the pandemic. This report says Croatia is a case study on how the opening of tourism can trigger a second wave. Because Croatia depends on tourism and Croatia had controlled coronavirus cases in May, the government decided to open its coastline areas to tourists from Europe. These tourists returned home with the infection and spread the virus. Clubs and bars were allowed to reopen for the summer season after the lockdown in April along the Adriatic coast attracting visitors. With 500 miles of coastline and Mediterranean climate, ancient towns and affordable stay, Croatia is crowded with tourists. In 2019 21 million visitors came here according to the Croatia Tourist Board. On Italian visitor from Parma cited here says she found crowded parties and bustling bars and restaurants where hardly anybody kept social distancing and wore masks. People in shops and bars she says told people they did not need to wear masks. The governments in Europe were keen on making up for the economic costs of the pandemic and opened the internal borders of the European Union in June. The opening of resorts in the sunbelt of Europe in Spain and Portugal has led to the spike in cases in Madrid and other cities in Spain. The same is happening in France. But vigilance dropped especially in Croatia where little or no restrictions were visible. Not only were bars allowed to open but the social distancing rules and mask rules were never practiced. Some Croatians call it incomprehensible. It has led to the spike in Germany, Czech Republic and Austria. The Koch Institute says 12% of all new German cases are traceable to Croatia. It is now a fact that international travel is a way the coronavirus accelerates. Governments in France, Germany, and the UK which are not especially dependent on tourism have the option to encourage people to stay in their home countries and remove this cause of acceleration while keeping shops and offices open so that business and jobs are preserved. For people hurt by lack of employment in the hospitality industry and others with lost wages from being in an occupation that acts to accelerate the virus it is a better option to offer financial assistance than to end up closing offices and shops in another partial lockdown. Opening bars helped accelerate the pandemic in California after the lockdown with steeply rising numbers of new cases. Educating the public to the extent that it should be about the dangers is also missing.    ...

These simulations show the  importance of flattening the curve for coronavirus especially the steep jump in the curve when it grows exponentially as people mingle in crowded environments, on trains and subways, and in public gatherings of more than 10 people. This is shown here in four different simulations in the Washington Post. Social distancing and quarantine worked in China, Taiwan, Singapore and South Korea. Though the attempted quarantine simulation here does not cover the situations in China, Taiwan and Singapore where quarantine has worked and was the only way to tackle the coronavirus in time to do least damage. Additional simulations would show the way it was limited in Singapore through contact tracing and mandated staying at home for all who have come in contact with affected persons. And in South Korea a simulation could show how this worked through containment by testing and limiting spread, or China by an effective quarantine or lockdown of a city or province.  The basic idea is to limit contact and separate so that intermingling is restricted to as few places as possible for a limited period during which health authorites can achieve a controlled situation through systemwide organized efforts.  ...

German chancellor Merkel met with leaders of Germany's 16 federal states to come up with an exit plan for reopening the economy from the lockdown in phases. In this interview in The Hindu with Public Health Foundation of India president Srinath Reddy, the importance of safe behaviours including masks and social distancing, public discipline to tackle a potential third wave. This was also pointed out separately by the Director AIIMS.  Mr. Reddy says study by Public Health England shows just one dose of Astra Zeneca vaccine is not very effective against the Delta variant- about 30% effective. As a result he suggests the need to reduce the spacing intervals which are at 8-12 weeks to a shorter interval now that adequate supplies of vaccine will be available starting in July and August till the end of the year. The initial spacing was 4 weeks when Astra Zeneca vaccine was first introduced and at which time the variants had not emerged. The Indian federal government affidavit to the Supreme Court shows that 1880 million doses of vaccine will be available by the end of the year 2021 to vaccinate fully with 2 doses the population of India over 18 years using 5 locally manufactured Made in India vaccines. This does not include the mRNA vaccines that will be made available from Moderna and Pfizer for which cold storage facilities are being prepared by the federal government.   ...

States which have not been affected much and fewer cases in the western part of the U.S. such as Idaho, North Dakota, Iowa could open earlier. Texas could start in May. California would have to do more testing before it starts Phase 1. New York, New Jersey, Michigan and Massachusetts, would come later because of the severity of the crisis. India crosses the 10 million mark in coronavirus cases for the first time on December 18, 2020. The number of deaths is at 146,000. The U.S. is the only other country with over 10 million cases. U.S. has 17 million cases and 318,000 deaths on December 18. The daily cases in India peaked in September at about 94,000 and have declined to 27,000 in December. The U.S. meanwhile is hit by a second wave that is much worse than the first. Daily cases now are close to 250,000 on December 18, 2020. The daily cases in the second wave are much more severe than the first. They have increased by a factor of 5 to 10 times. Places which got through the first wave without severe damage are hit hard in December. This includes Germany, and California. In California daily cases exceed 50,000 and in Germany 30,000. In California, France, Germany, and UK, Spain, the daily cases far exceed earlier cases in the first wave by a factor of 5 to 10 times. For this reason India needs to be wary of a resurgence in the pandemic in a second wave. With its large population, need for economic recovery, and opportunity to benefit from the vaccine developments and its strict protocols for testing, isolation and social distancing, mask wearing, India can carefully and vigilantly prepare for the second wave. ...

This ratio measures the average number of people infected by a carrier A. The people A has infected B+C+D are the ones now not infected by A with the lockdowns such as in New York, Italy, Germany, UK and France. It is determined by global health experts that the number of B+C+D is about an average of 4 persons infected by 1 person A with coronavirus, though it may be much higher in practice in some areas. The natural rate of RO or reproduction ratio is considered by the European Centre for Disease Prevention and Control to be 3.86 or about 4, if no lockdown or social distancing or other prevention is practiced. This in a situation where people behaved as before unaware that the virus was around them. Governments such as New York and France, UK, Germany are including this key variable in their determination of how long a lockdown lasts, and for determining if the reopening is not going the right way or failing. In such situations the lockdown would be reinstated, or if it is a phased reopening such as in the U.S. and other countries go back to the previous phase. In Italy and Germany the RO reproduction ratio for coronavirus is estimated by official experts at 0.8. Germany's RO estimated by the Robert Koch Institute and Italy's by Franco Locatelli, scientific advisor to the government. In New York the margin is thin- with RO of 0.9, estimate from the state's governor. In France which has one of the tightest lockdowns of all with a document required to go outside it is at 0.6, the figure coming from the prime minister Mr. Philippe. In the UK it is below 1.0 but no accurate figure is reported. As Dr. Birx- leading the coordinated response in the U.S. - emphasizes over and over again this is a very contagious virus, about which not much is known. Social distancing, wearing masks, basic prevention measures such as frequent handwashing, and not gathering in large numbers of people, is essential for defeating this virus. Useful tips as people go back to the office, from a number of experts in the WSJ. Walking, biking to work or driving to work is safest. Use of public transit requires masks, sanitizer, frequent hand washing afterwards and caution. Surfaces such as elevator buttons, doorknobs and printer or device buttons should be disinfected regularly- use a tissue paper or paper towel to handle them and then use hand sanitizer. Meetings should be done with social distancing of six feet distance, and reduced number of participants. All should wear masks. Taking meetings outside where airflow is more is a good idea. Reduce number of meetings, as the meetings have not been very productive ways to get work done and many workplaces already have too many meetings. Avoid lunch dates outside, even in the open, with colleagues. A better way is to eat on your own and then take a masked or socially distanced walk with a colleague afterwards, say experts. High touch communal items should be replaced in offices such as coffeepots water coolers, and bulk snacks and this should be done by the companies as they prepare the new layout and arrangements.   ...
