As the coronavirus surges in India with over 300,000 cases a day on April 26, a clear picture on the vaccination drive in the country is critical. The following is the picture of the vaccination progress from Union Health Ministry in India as shown in The Hindustan Times. India has vaccinated 140 million people with at least one dose says this report in The Hindustan Times.  On Saturday 24th April 2.4 million doses were given for that day at 8 pm. This was done over 99 days. This means about 12% of the population of 1.2 billion has been vaccinated.  This compares with the vaccination in Germany for about 21% of people vaccinated with over 18 million getting the first dose in Germany by around April 25. Both Germany and India have suffered from vaccine shortages, some skepticism about vaccinations. Gradually sentiment is shifting in both countries so that once skeptical Germany now has about 75% of people willing to take vaccine on April 25, 2021. In India about 6 million healthcare workers have 2 doses of vaccine, and about 9 million have 1 dose. About 6 million frontline workers have 2 doses and 12 million frontline workers have 1 dose of vaccine.  There is a shortage of vaccine supplies and a bold decision was made by the Indian government on April 25th 2021, after the surge of cases to a world wide maximum of over 300,000 cases a day. The decision was to give immediate regulatory approval for the three major vaccines in the US to be brought and used in India. And delivery will be speeded up - no customs duties and fast processing of supplies access to speedy logistical supply routes. This is a huge step forward for the vaccination drive as this means Pfizer, Moderna and J&J vaccines can now be used in India. The government is also urging the companies to make in India or export to India with prices that provide flexibility in pricing for the private market. The locally produced Covishield Astra Zeneca based vaccine produced by Serum Institute will be allowed to be sold to the private market at 600 rupees or close to about $10. Pfizer and Moderna, J&J can price in a way that would be somewhere around this price range. The access to more vaccines and the ability of the companies to make a reasonable profit in the Indian private market means that vaccine supplies should open up in May and June.  This could give a huge boost to vaccination numbers so that India's vaccination percentage of population vaccinated should keep up with that in countries like Germany and France that were slower to get started in Europe but are now catching up quickly. Steve Bannon is described in this indepth report by Scott Shane as a workaholic, born to working class family with his father a telephone line operator, who went to Virginia Tech and joined the Navy in the hope of advancing a career in politics. At Virginia Tech he won a leadership position of the student organization. He was described by another student who knew him well as passionate but not likely to get much done. The period at Virginia Tech and in the Navy were the Carter years followed by election of Ronald Reagan. The election of Reagan had a huge influence on Bannon- the same overtones of that campaign of Reagan are seen today in the forgotten men and women, white working class families that Conservatives then and Tea Party Conservatives in the Obama years felt ignored. The downward drift of the lower middle class families that saw incomes drop as manufacturing hollowed out in the U.S. with foreign competition, the failure of establishment politicians of both parties to protect American manufacturing and working class families, added to the sense of angst for Bannon. Bannon just like politicians in the Obama camp such as Emmanuel, found the way to politics through finance and gains made as the banking sector and financial institutions made huge financial gains by 2008. This was a stepping stone for their political ambitions. Emmanuel who is also a workaholic and passionate about his views worked to elect a black president, Bannon choosing to do the opposite and push for bringing back the Reagan era. Most on the liberal side see him as part of a racist movement. Reagan was none of those things. How does one reconcile the two? It is possible that seeing the fight against the established politics as an impossible task, Bannon in his passionate temperament did not object to the support of right wing extremists, in the same way that Trump did. As both Trump and Bannon have people of Jewish origin and black people in their circle of friends or family. What incensed Bannon as described here by Scott Shane of the NYT, was that after the financial crisis of 2008, hardly any bank executives who had committed wrongdoing went to jail, his father's line operator retirement savings were devastated by the financial crisis, and working class families struggled harder than ever, that his daughter at West Point was with mostly children of working class families who were the ones fighting America's wars. Many ironies abound in the story. Bannon got his business start in the same financial institutions that were involved in the financial crisis of 2008, Bannon & Co was acquired by Societe Generale. He is from an Irish Catholic working class family in Richmond and attended Benedictine High School, with a mother Doris that worked on the campaign to elect Douglas Wilder, a Democrat, as the first African American governor of Virgina.  The other ironies are in that Bannon sees Trump as "an imperfect vessel" but still good enough, and Trump sees himself as "making all the decisions" when asked about Bannon, as a range of interests struggle to form a coherent movement on the right in American politics- an unlikely combination of a telephone operator's son and real estate magnate's son who built his own real estate business in luxury real estate towers far removed from ordinary men and women they represent. ...

The US spends astonishingly only $500 per toddler, about 4% of what advanced countries in Europe spend.The Biden plan is to change that. The impact has been a catastrophe for women leading to increase in mental health problems as culture and other reasons lead to women taking on 60-70% of child care and household chores. German chancellor Angela Merkel arrived for a meeting of eurozone leaders in Brussels on October 23, 2011. She said: "I believe that now we have reached a more realistic view of the situation in Greece and that we will provide the necessary means to be able to protect the euro." Germany has insisted that bondholders take writeoffs of between 50-60% of Greek debt so that Greece would have sustainable debt. A review of Greece's debt by the European Commission in coordination with the ECB and the IMF shows that Greece's debt situation is totally unsustainable and will require a bondholder writeoff of around 60%. according to that report a 60% writeoff for bondholders would be required to bring Greece's debt below 110 percent of GDP by 2020. This has supported the German "realistic" view and Jean-Claude Juncker of Luxembourg, who heads the euro group of finance ministers stated that "we agreed yesterday (Friday, Oct. 21) that we have to have a significant increase in the banks' contribution." France also backed away from the plan it was supporting for the European Financial Stability Facility (the fund established to lend to troubled countries) to borrow from the European Central Bank, something Germany opposes. French finance minister Francois Baroin, said the issue was "not a definitive point of discussion for us,... what matters is what works." The Dutch support the Germans on these issues and Dutch finance minister, Jan Kees de Jager, said the use of the European central bank was "no longer an option." Options being considered are for the European Financial Stability Facility to offer insurance against a portion of losses on Italian and Spanish bonds....

U.S. president Obama's passive response in the handling of the NSA spying on the phones of world leaders including the president of Brazil and the chancellor of Germany comes under criticism in the U.S. The failure to provide adequate assurance- and take immediate action since the summer of 2013 when the first revelations of NSA spying were out- to regain trust of European and other leaders is seen as a weakness in leadership. With German presidential elections approaching German chancellor Merkel actually tried to tone down the initial uproar over NSA spying revelations in the summer of 2013. It was only after it was revealed in October 2013 that NSA had monitored Merkel's mobile phone did the chancellor make an issue of this and Obama could not respond to why no action had been taken since the summer and a complete review of NSA spy activities made by the President and advisors. Because world leaders are involved, and not just of allies but large emerging market nations such as Brazil, this becomes the personal responsibility of the U.S. president. Obama also comes under criticism for not responding to the failure of the healthcare website. This matter is of a different nature and could be handled by the President's Health and Human Services Secretary, Ms. Sibelius....

This editorial in the Wall Street Journal says faster approvals at the Food and Drug Administration have helped bring more generic drugs to market lowering prices for the public. In 20 months the Trump administration approved 1617 generic drugs, 81 a month on average or a 17% increase over the preceding 20 months. Council of Economic Advisers in October said this was a saving of $26 billion for the public.

President Trump is looking at price controls as a way to bring down drug prices. With increasing outcry about high drug prices in the U.S. the Trump administration and Democrats in Congress are looking for new approaches to bring down prices.

France has a large public sector which gives it protection in this crisis from mounting job losses. According to the OECD France had91 public sector jobs for every 1000 peopple compared to 49 for Germany. Public spending is 52.4% of GDP in 2007, compared with 44.4% for the UK, and 37.4% for the USA, according to the OECD. Still the unemployment rate is rising. The unemployment rate was 7.9% in November 2009.

Comparative effectiveness research will be conducted to evaluate what is the best treatment for any ailment or disease or health problem under the $1.1 billion allocated for this in the Stimulus Plan. What are the alternative methods of treatment, what is the effectiveness of each treatment, what are the comparative costs and so on. Is it better to treat neck pain with surgery or acombination ofphysical therapy, exercize and medications? If there is ablockage of arteries in the lower leg and leg pain, how does drugs and watchful waiting compare with surgery? For chronic heart failure how does home monitoring of blood pressure and weight and exercise in addition to medications provide an alternative route as opposed to just medications. Dr Fisher of Dartmouth Medical School cites these as examples of questions that can be asked in comparitive effectiveness research. The money will be available to the Health and Human Services Department and will be used over several years. About 15 federal employees will form a council to coordinate the research and advise President Obama and Congress on how to use the money. In 2007 the US spent $2.2 trillion, or 16% of GDP, on health care, and the Congressional Budget Office says it will grow to 25% of GDP at the rate its growing by 2025 if left to its own devices. ...

Of the people asked the question "what issue do you see as the most important for health care" 44% in a NBC/WSJ poll said they considered Cost as the biggest issue, and 37% said that they considered the Uninsured as the biggest issue, 11% said quality of care was the biggest issue. The President is focussing on cost as he develops the health care plan. He is also saying that to provide health care for all and cover the uninsured without controlling costs would break the bank. Another thing that the White House shows it has learned from previous failed attempts, is that it is doing this with an open Health Care Forum with participation of different sides in the health care discussion. Clinton's effort in 1993 ran into problems because it was developed secretively, and without much discussion, and little effort to get different people's views. The previous attempt also waited too long after the election, and this time the President is moving quickly when he enjoys large popular support. Its also true now that there is serious concern especially in this downturn of what a threat spiralling costs of health care are becoming to the future prosperity of the American people. This time the leaders in Congress are setting an aggressive schedule to get health care palns legislation on the floor by June and get a floor vote in August 2009....

Is time slipping away for Russia to restore what it sees as its special relationship with Ukraine, as Ukraine finds its own identity through its language and independent Orthodox Christian Church since 2019. This WSJ podcast report is by James Marson who lived in Kiev from 2007 to 2012, and Ryan Knutson, with the Archbishop of St Michael's cathedral in Kiev, and the editor of Elle magazine edition in Ukraine joining in.  To understand Ukraine one has to know that Russian is the language of the cities, which means people in Kiev speak Russian. People in the countryside Ukrainian. This is very unusual for a nation and it shows the condition of the country for centuries where intellectuals in cities dominated cultural and political life distant from the people in the countryside. For centuries Ukraine was dominated alternately by either Poland and Lithuania or Russia other than a period of 200 years around 1250-1400 when the Mongols were dominant. The peasants and countryside suffered greatly as in India and other parts of central Europe in the long history till the modern period in 1900.  Russians see their origins in the Kyivan Rus, a state bringing together the different ethnicities Ukrainian and Russian in the period 1000-1240 under the Byzantine Church in Constantinople. Kyiv, the modern capital of Ukraine called Kiev today being the capital of this state. This is the cultural connection that president Putin and Russians see as one they do not want to see drift away. After the Russian state drove out the Mongols in 1240 the northern provinces and Kiev became part of the Polish-Lithuanian Commonwealth, and the rest became part of a new Russian state. After 1650 Ukraine became part of the Russian Empire and by 1800 with the partition of Poland was fully made part of the Russian Empire. Russian is now after 1800 the language of the intellectual class in Kiev and the cities, and Ukrainian language persists in the countryside. In 1804 Ukrainian is banned as a language and subject of instruction in schools. The end of the Russian Empire under the Tsars in 1917 ended the ban on the Ukrainian language and a period of respect of the cultures of the different soviet republics including Ukraine ensued. Putin has strong feelings on Kyiv, or modern Kiev, as the place where Russia as a country began. He wrote a 7000 word essay says this report in WSJ in 2010 on this relationship as he sees it.  Yet the period of protests in Kiev since 2010 has resulted in Ukraine building  its own identity as a nation. Magazines in the country are required to use Ukrainian for 50% of their circulation. People in Kiev now use Ukrainian instead of Russian as the sense of national identity is being revived. During 1917-1921 Ukraine fought a war with the Bolsheviks after the Russian Empire collapsed. This history is why Russia is acting now to push for Ukraine not drift completely away. It is also what makes Ukraine different from Poland which has cultural ties to Western Europe. It is why the US or Germany is not willing to go to war with Russia over Ukraine, as it would over Poland. It is also why Russia may not see war as the best option as about one third of Ukrainians say they will fight to defend their country, according to this report. The situation is complex and this is why both sides want to negotiate some way out in which Russia wants the US and NATO respecting its sense of connection with Ukraine in its history with Kyiv as the place Russian state started, and Russia not going further. Russia's tangible proposal is for no to Ukraine joining NATO or the European Union. The US and Germany want something else- the right of Eastern European nations that suffered from Tsarist or Soviet domination or German Hapsburg domination to finally be able to assert their own right of self-determination as democratic countries. This would include Finland. And also Sweden. Ukraine is not another small Eastern European country. Population is 44 million and it is the second largest by area in Europe after Russia.  Russia may also see the move to bring this up at this time as a way to unify the country against what it sees as threat from NATO. As Brendan Simms of Cambridge notes in his recent book -Europe, France went through a period after 1600 when it needed external danger as a way to unify the country, as much as unity of the country to fight external danger. The economic costs after building Nordstream II pipeline are to0 great for both Russia and Germany, and for the US and Russia during the pandemic, which means there is a real need to find a way out for all sides.     ...
