This maybe of some comfort to countries without the internet connections prevailing in so called advanced countries. Even there the improvements are coming with a cost such as the proliferation of watching Tik Tok from China in India and China with poor quality content that is likely to fragment peoples attention making it harder to lead healthy lives that we once took for granted before the advent of Mr. Job's screen enabled phone. ...

Eri Machii, founded AfriMedico, a nonprofit organization designed to deliver medicine to remote regions in countries in Africa. She worked in Niger under a Japanese Overseas Cooperation Volunteers program after two years working as a pharmacist. There she learned about the dangers in Africa in remote villages where travel and delivery expenses were high and infrastructure lacking. She devised a method of Okigusuri for Africa where as in traditional Japan medicines were left in a kit in villages and people paid only for what they used. Payment is done by cell phone using the M-Pesa money transfer system. Maichii learned about okigusiri as a system used in Japan throughout its history in places where infrastructure was lacking, lack of universal health insurance, and large families living together. She found that this was true for distant villages in countries like Tanzania where she implemented the system under AfriMedico. 20 volunteers helped found the organization and Tanzania pharmacist network helped guide them in setting it up. Large amounts of medicine taken at one time reduce the cost of transport. Use of the system of medikits in urban areas helped subsidize the village use. This is a system that has great potential for medicine delivery in many parts of Africa and Asia that have the same problem of access to basic medicine kits- so that treatment can be done earlier in the process for quicker less costly recovery, improving general health conditions. One can think of Indonesia, Philippines, India, Pakistan, Bangladesh, West and East African countries as having potential for wider use of this system. As people pay for only the medicines used using cell phones the system has wide applicability from cost and access point if supported by private and governmental agencies in these countries.   ...

In an Op-ed in the Washington Post Mehmet Oz, Head of Medicaid and Medicare says while banning use of Medicaid and Medicare funds for transgender medical procedures- "America’s children aren’t lab mice. They deserve quality care backed by sound evidence and should not be conscripted as test subjects in risky experiments that cause irreversible harm. Federal government is banning the use of Medicaid and Children’s Health Insurance Program funds to subsidize sex-rejecting medical interventions for minors and prohibiting hospitals participating in Medicare and Medicaid from performing them. Dr Oz says "extraordinary interventions such as cross-sex hormones and double mastectomies require extraordinary evidence, especially when children are involved." He says the evidence is lacking and cites information from many countries. ...

Krugman points out that about 13 million Americans without insurance gained health insurance under the Obama plan. He says if it is turned back 8 million whites without a college degree in that 13 million will lose health insurance. Of these eight million about two out of three voted for Trump, so that 5 million Trump supporters could now lose health insurance even though they are older and have more health conditions. Krugman says this aspect of the election campaign was not covered well in the misinformation and social media information of the 2016 campaign, and the lack of media focus on the important issues in the election. The Republican House health care bill AHCA  keeps some some of the more popular parts of the Affordable Care Act such as not taking pre-existing conditions to deny insurance, keeping children covered on parents insurance till age 26, and increase contributions to Health Savings accounts. It is different in that the expansion of Medicaid at 138% of poverty rate threshold is rolled back. Age is used for tax credits instead of income, hitting those approaching Medicare age harder. The maximum charged to older people is now set at  5 times compared to 3 times what young have to pay. In general the Republican bill is seen as targeting the elderly to keep premiums down. The elderly on low incomes are hit hardest. Fox News O'Reilly Show showed the host questioning House Republicans, citing the CBO estimates that elderly on low incomes may have to pay as much as $14,000 a year for insurance making it basically unaffordable, and 52 million Americans would be affected adversely. Large companies are no longer required to offer mandatory health insurance under the new bill. Conservatives from Freedom Caucus wanted to see the essential areas of benefits covered by the law limited to fewer than the ten areas in the Affordable Care Act. The ACHA Republican bill leaves to states to determine what are required essential benefits. At one point maternity benefits were to be dropped but this was changed to let states decide. As a result the bill is 33 votes short of the number needed for passing the house in March 2017.   Neither the Democrats Affordable Care Act or the Republican House bill of Speaker Ryan do much to tackle the real problem- the absolute amount and increases for health care dollars for treating each disease in the U.S. compared to European and other countries. As a result health care has become more of a partisan struggle between the two parties than a real effort by all to overcome the problems that have to be tackled. Republicans want to see premiums drop and keep the burden on the deficit down- but with the level of U.S. health care costs disproportionately high compared to Europe and the rest of the world the arithmetic is tough and ends up leaving out vulnerable groups such as the elderly on low incomes, thus making the whole proposition prone to fail. For the same reason the Democrats failed to keep premiums down with a wider safety net leading to calls for repeal of their version.   ...

Arizona, Utah and South Carolina are 3 states that are cited by the US Labor Department for not adopting any portion of the Occupational and Health Safety Emergency Standard for health care workers. The Labor Department says that by not adopting these standards for social distancing, mask use and paid time off for vaccination, these states are risking the health and safety of health care workers. OSHA sent letters to these states revoking the states abilities to run their own occupational health safety programs.
