As Dr Harsh Vardhan, India's representative and Health Minister, takes on responsibilities as the Chairman of the Executive Board of the World Health Organization this can be said about this critical juncture.The WHO is going through soul searching and a reevaluation of how it has implemented the vision of its founders during the closing years of a world torn apart  by war in 1945, and actual founding in 1946.  At the UN conference in San Francisco in 1945, Dr. Szeming Sze of China, Dr. Geraldo da Paula Souza of Brazil and Karl Ewang of Norway, were keen to set up an international organization for cooperation on health. The Indian representative was Arcot Ramaswamy Mudaliar, a member of Winston Churchill's War Cabinet, who chaired the committee on economic and social problems. He was the president of the Economic and Social Council which called for an international health conference in Feb. 1946. It was at that conference on 19 June 1946 that the World Health Conference came into being. A native of Madras he then returned to Mysore as chief minister. The vision at the time for international cooperation on problems such as smallpox which killed 2 million people each year were quite different from the fast moving epidemics with international travel in 2020. Today 4.4 billion passengers traveled by air in 2018, according to International Travel Association, 100 times compared to less than 40 million in 1950, and about 10 times the 400 million when Nixon's visit opened up China in 1972. The world we live in is different and the World Health Organization needs to be redesigned for the 21st century. The entire process of how the WHO operates has to be rethought. Immediate steps include- 1. The appointment by Executive Board should be reinstated as this is more representative of the world population and the major centres of advanced public health, including the major countries. Throughout its life the WHO made appointments through the Executive Board not by election. The election in 2019 by 200 countries was actually not representative of the world population as it gave India, Brazil and other early founders at the 1945-46 conferences only 1 vote each with population of 1.2 billion and 210 million, the same as tiny countries Barbados population 385,000 and Laos 7 million. 2. Reassessment of the entire process in which nations are requested to give permission of teams from major countries in Europe, North America and Asia, major population centres, so that the 6-7 week delay between the U.S. request to China for a special team to go to Wuhan on January 6, 2020 and the permission granted Feb 16, a costly delay of 7 weeks which added millions of cases and hundreds of thousands of deaths. In a super fast moving pandemic with international sports stadiums and 4 billion air passengers spreading it like wild fire around the world, there is little room for error, every day counts. Never should this happen again, as Dr. Sze Szeming China's representative said once in 1945 we must learn from mistakes, as mistakes were made in the years before World War II that were costly for China, India, North and South America, Other Asia, Africa, large population centres. 3. As was the effort then in 1946 and the early years, the effort to keep the staffing positions of leadership in the World Health Organization should be kept far from politics. Very experienced and capable people are needed from major countries with long records of public health experience and committed to the huge task, as was the vision of the founders.     ...

Sheila Bair leaves behind a much stronger FDIC. William Isaac, a former FDIC chairman says she has had a positive effect on the agency by raising its profile and energy level. Under the Dodd-Frank financial reform legislation FDIC now has the powers to take over large complex financial institutions that are in serious trouble. Mr Gruenberg, who succeeds Ms Bair, has worked closely with her at FDIC. Jim Wigand, heads the new Office of Complex Financial institutions at the FDIC. One of the powers given by the legislation is for FDIC to have its examiners at financial firms to go over operations, providing backup supervision to the Federal Reserve and the Office of the Comptroller of the Currency. According to FDIC officials analysts should be in place at financial firms the FDIC oversees during the next 12 months. The FDIC is also working on a rule requiring banks with assets larger than $50 billion and other large nonbank financial firms to provide a "living will," a plan that would help regulators speedily and cleanly wind down a firm in a future financial crisis. In March 2011 the FDIC approved a draft rule requiring firms to submit these plans, along with regular updates and reports on the firms' current credit exposure. Ms. Bair says the proposal will be completed in August. Bair has also put in place FDIC managers with considerable experience who can continue the work of strengthening the regulatory system she started in 2006. In her work at FDIC Bair has performed a remarkable public service at a difficult time in the nation's history....

Samuelson shows why the Ryan Plan needs serious consideration because it brings in competition from the private sector to control medical costs. The Obama plan does not reduce Medicare costs he says because it merely transfers the costs to mandated Affordable Care Act spending. And the Independent Advisory Board of 15 experts given the job of reducing Medicare spending if it exceeds a certain amount is ineffective- it cannot increase patient cost-sharing, restrict benefits or modify eligibility or cut spending by more than 1.5% in any particular year according to the Kaiser Family Foundation. Important points to remember about health care are: 1) sustainable Medicare for current and future generations can only be on the basis of sound finances 2) sound finances mean first and foremost controlling health care costs 3) private sector competition is the better way to control health care costs in todays environment where cost reduction needs to be large enough to make Medicare sustainable especially when competition shifts health care delivery away from the cost increasing fee-for-service system 5) Obama Affordable Health Care Act does little to change the costly fee for service system and the basic mechanism of cost escalation in U.S. health care. The Ryan plan's voucher option injects this dose of competition into the system and only for those who choose this option, it was also drafted with the help of Democrat Ron Wyden, and is cautious because it does'nt start this till 2023- giving time for discussion and improvement, and therefore a constructive effort to look at serious ways to control uncontrolled fee-for-service spending....

German chancellor Angela Merkel arrived for a meeting of eurozone leaders in Brussels on October 23, 2011. She said: "I believe that now we have reached a more realistic view of the situation in Greece and that we will provide the necessary means to be able to protect the euro." Germany has insisted that bondholders take writeoffs of between 50-60% of Greek debt so that Greece would have sustainable debt. A review of Greece's debt by the European Commission in coordination with the ECB and the IMF shows that Greece's debt situation is totally unsustainable and will require a bondholder writeoff of around 60%. according to that report a 60% writeoff for bondholders would be required to bring Greece's debt below 110 percent of GDP by 2020. This has supported the German "realistic" view and Jean-Claude Juncker of Luxembourg, who heads the euro group of finance ministers stated that "we agreed yesterday (Friday, Oct. 21) that we have to have a significant increase in the banks' contribution." France also backed away from the plan it was supporting for the European Financial Stability Facility (the fund established to lend to troubled countries) to borrow from the European Central Bank, something Germany opposes. French finance minister Francois Baroin, said the issue was "not a definitive point of discussion for us,... what matters is what works." The Dutch support the Germans on these issues and Dutch finance minister, Jan Kees de Jager, said the use of the European central bank was "no longer an option." Options being considered are for the European Financial Stability Facility to offer insurance against a portion of losses on Italian and Spanish bonds....

This WSJ editorial makes an excellent argument of how the wrong conclusions can be drawn from Hamas, as an offshoot of the Egyptian Muslim Brotherhood gaining participation and winning of the elections in Gaza. It calls this a mistake in 2006, which does not affect the liberal democratic openings of the Bush administration in the Arab world. Hamas had an armed militia and rejected the 1993 Oslo records, so the necessary committments which are required for democratic processes to work were not put in place, primarily on the advice of Secretary of State Condoleeza Rice who made an exception in this case. The Journal says the mistake was not about free elections, but elections before the proper groundwork had been prepared, which requires that anti-democratic parties cannot be part of a democratic system and elections- a lesson that goes back to 1933. If the Brotherhood in Egypt wants to participate in elections says the Journal, it has to promise to play by democratic rules , and work to establish religious and social pluralism, and honor treaty commitments. And the constitutional system has to setup a system of strong checks and balances that prevent an elected party from subverting the democratic process for future generations whatever its support at any particular time. This is significant as it puts things in the proper context and also clearly establishes a well established point- democracy can only work for democrats. And at the same time preserves what is best about America's heritage and core values in America's stance with the rest of the world, and in this case with the Arab world....

The Federal Trade Commission says food companies in the U.S. spent $2.3 billion in 2006 for advertising to children. With the epidemic in childhood obesity in the U.S., this raises serious questions about how product packaging, images and themes affect the eating behaviour of children. New guidelines have now been written at the request of Congress. They were written by the F.T.C., the Food and Drug Administration, the Agriculture Department, and the Centers for Disease Control. The regulatory agencies say they will take comments and consider changes before submitting a report to Congress. The guidelines call for foods advertised to children to include healthy ingredients such as whole grains, fresh fruits and vegetables, or low fat milk. The foods cannot contain unhealthy amounts of sugar, saturated fat, trans fat and salt. The sugar requirement would have cereals contain no more than 8 grams of added sugar per serving. Fruit Loops for example contains 12 grams of sugar per serving. The guidelines apply to both children and teenagers. However these guidelines are voluntary. At this time an industry led effort has not produced results. The Children's Food and Beverage Advertising Initiative, which is operated by the industry, lets each company set its own nutritional criteria. The regulatory agencies see the need for the food industry to follow a uniform set of standards. Without serious action on this issue the U.S. healthcare system will continue to be burdened with high rates of obesity related illnesses in the general population, and out of control costs. And the U.S. will continue to face the urgent problem of a lack of healthy eating habits of children teenagers, and adults....

China faces three main challenges and how well it handles them will determine if China does well in the future because the things that helped China in the last 30 years of development are now gradually coming to a close. The three main challenges are a changing work force and the gradual phasing out of the demographic dividend thats responsible according to some experts for a third of the progress this far, the gap between the rich and the poor, and severely constrained resources and supplies of energy and environmental resources. On the first its not something China can do to much about, on the second its going to have to have a more balanced development and repair the network of social services and redirect resources to the poorer sections (see the link to the conference at Lindau, Germany and Nobel Prize Winning economists opinions on this issue). This will bring more discussion and challenges about how to proceed as a lot of actions to build new infrastructure and new construction has been done by taking over land where needed. And on the third challenge has not been done so well so far as the amount of energy required to each yuan of economic output has not changed much, seeing a 3.7% improvement over 2006 in 2007 and only a 2.9% improvement in the first half of 2008 over 2007. All this is why Secretary Paulson cautions that many American might be worrying about the wrong thing, China overtaking the USA, what really is the worry he says is whether serious troubles in China will affect the stability of the USA and global economies....

Missteps by the Detroit automakers include fighting fuel efficiency legislation in 2005, even when the USA faced higher gas prices, and diluting the fuel efficiency legislation with a target of 35mpg for 2020 at a time when Europeans were taking up more aggressive challenges as public opinion there moved in that direction. They also spent heavily in lobbying spending, about $175 million for GM and Ford in the last 10 years, and some would say lobbying against the national interest and the national security interest of the USA, because failure to reduce consumption of oil through fuel efficient cars weakens the economy by sending hundreds of billions of dollars overseas to mideast countries. The closing of plants in states like Alabama, Louisiana, Georgia and Delaware and consolidating their operations closer to home weakened Congressional support, And the foreign auto makers built plants in places in the south like Alabama resulting in Senator Shelby of Alabama becoming allied with them. Rick Wagoner failed to show the vision and leadership needed, and Detroit failed to realize that vision and leadership were required to run these companies. not coming up through the large bureaucracies of these companies. And people associate him with declining market share and a company in decline and asky why. The whole mood of the country is reflected in newspaper columns across the country, in reader comments that run into the hundreds for each article overwhelmingly negative for taxpayer money going to Detroit automakers. This is the situation today and catches the Detroit automakers management, union, dealers, suppliers, by surprise as they have become so used to the status quo and know nothing different....

This report in the WSJ points to president Biden's speech to a joint session of the US Congress that providing two years of free community college would "change the dynamic" for education in America taking the first step to correct a dangerous drop in college enrollment for young men in America and ensuring working class families have access to college education. The last thirty years of skewed wealth distribution, loss of manufacturing in America, have created alarming distortions in  the access to college education for working class families. Mrs. Biden is a fervent advocate for community college access in today's America, as a community college teacher for 30 years. Biden's $45.5 billion 5 year plan would waive tution for 2 years of public community college. States would have to opt-in to participate, and federal government would provide 100% funding in the first year, decreasing contribution by 5% each subsequent year, with states picking up rest of the cost. It is quite shocking that this is being dropped from the Biden $3.6 trillion Families and Workers Plan that is now being whittled down to $2 trillion. Not because it is not badly needed for American economic competitiveness, and helping workers and families. But because following narrow parochial interests the National Association of Independent Colleges and Universities opposes it. And because the US Congress is evenly split between Democrats and Republicans 50-50 in the Senate. The Association of Independent Colleges sees a shift to community colleges and a sharp drop in its enrollment. Community colleges saw a dangerous drop in enrollment of 12% to 4.5 million students in 2020 from the spring of 2019, according to National Student Research Center. Never was a program more badly needed, as American men are alarmingly falling behind in enrollment. Here are some responses to the failure to take even the first steps to broaden college access so that America can return to economic competitiveness. "What kind of world do we want to live in?" Martha Kanter, College Promise. "That's kind of a devil's choice, isn't it? The whole system has to work from infant care all the way through." Senator Tina Smith, Democrat of Minnesota. This is because child care and children's education will be funded yet a struggling generation of college students will be left out. US Chamber of Commerce opposes a $45 billion program that is critical to American competitiveness with China and other countries. US Congress drops a program that at $45 billion is only about 2% of the $2 trillion package and which is critical to economic competitiveness. Former Republican Governor Bill Haslam of Tennessee supports community college access as a pillar of economic development and it passed the supermajority in 2014. Mike Krause, Republican former director of the state higher education commission says- "I have been surprised by the lack of enthusiasm for what is really a massive workforce development concept that also provides a path to the middle class. You'd think that would hold some appeal for Republicans and Democrats." The lack of clarity and concentration, lack of unity of purpose to get all vaccinated,  is visible in America's vaccination drive. That same lack of clarity and concentration, lack of unity of purpose, is visible in America's faltering efforts at correcting serious and alarming problems for access to college and American competitiveness in the world. Shoppers are showing resistance to price increases in retail stores. The Fed under Jay Powell has taken a resolute stand against inflation slowing inflation in house sales and rental, in automobile pricing and other sectors of the US economy. New investments under the climate change bill passed in Congress and the CHIPS and Science Act, Inflation Reduction Bill, mean more factory openings and jobs in America. A milder winter in Europe is helping it tackle an energy shortage and bringing oil prices under control.

India achieves an initial vaccination target of 3 million vaccinations in a single day on March 14, 2021. Much of recent increase is from people over 60 getting vaccinated. For healthcare workers the first priority group about 50% still need to be vaccinated. This sets the stage for taking the vaccination drive to the next level in India. Increasing number of people are now getting their second dose.

Health experts see not vaccinating the vulnerable population fast and wide enough as a missed opportunity. The need has never been greater to vaccinate with better organization, vaccinating staff, and supply of vaccines at locations, so that this beats any second surge.

Johnson & Johnson begins a 60,000 person clinical trial on 3 continents in the final stage of testing. It could learn vital results in early next year which if positive would lead to government authorization for emergency use soon after. J&J will test whether a single dose of vaccine will work. It is also doing work separately with the UK government for a separate phase three study to test whether a 2 dose vaccine will work.

Other companies in final stage of testing are Moderna and Pfizer with studies results expected in October and Astra Zeneca working with Oxford expecting results by the end of 2020.

The Consumer Price Index CPI rose by 8.3% in August, US Labor Department reports from same month a year ago. This is down 8.5% in July and 9.1% in June, even though oil prices are coming down. The average household is spending $460 a month more to pay for the same basket of goods and services than last year according to Moody's. Core CPI excluding volatile food and energy prices was higher in August at 6.3% compared to 5.9% in July and June 2022. The US price of gasoline was average of $3.71 at the pump in August down 26% from its high in June, according to OPIS/DowJones.

Powell thinks that rates should have been higher- what is called the neutral rate for stable inflation and employment should have been higher some years back. This rate is closer to 4 percent. It also means fewer or no interest rate cuts in 2025. The Fed funds rate is now 4.3%.

Obama told the crowd he understood the public's mood for change and that he himself had benefitted from it. Yet he said that it did not add up. Obama said: "This guy is suddenly going to be your champion? I mean, he spent most of his life trying to stay as far away from working people as he could, and now this guy is going to be the champion of the working people. Huh." "I mean he wasn't going to let you in his golf course. He wasn't going to let you buy in his condo. And now suddenly this guy is going to be your champion."

Her response she said would be to "put the government at the service of those who found themselves poorer as a result of monetary policy." Following the fifth North Korean nuclear test DW.com talks with Rudiger Frank about the test and what this means. Frank says the negotiations with North Korea and efforts to limit the program so far have been a colossal failure. Tests have continued and without a change in strategy more tests will be followed by the usual condemnations by world leaders and further sanctions. Frank says this change in strategy would include recognizing North Korea as a nuclear state, and getting North Korea to ensure the weapons are safe and secure in the country, getting a clear idea of what and where the weapons are. The International Atomic Agency would be asked to go in and make inspections. The next step would be to freeze the program at some level agreed to. This is a tough step to take but it only recognizes the reality of the situation, and continued development by North Korea of nuclear weapons if no steps for change are taken.

The mismanagement of the economy under president Macri is leading to an economic crisis in the country. By embracing economic orthodoxy and slashing subsidies for fuel, electricity and transportation Mr. Macri who won the election 3 years ago has cause the prices of these basic goods to skyrocket. This has hurt the middle class and poor in Argentina. For most of this century Argentina has pursued populist policies, and in the last five decades periods of free market principle based economics were followed by severe crises, and subsequent restoration of populist policies to improve the economic conditions that had deteriorated.  The peso lost half of its value in 2018, leading to a IMF bailout of $57 billion. Inflation is at 50%, unemployment above 9%. To stem the fall in the peso the central bank increased interest rates to 60% stifling the economy and business. Under his predecessor Christine Kirchner the peso's value suffered and its currency reserves were low after fall in soyabean prices, yet the currency had not suffered the kind of decline that it has seen under Mr. Macri. The cutting of subsidies and the economic crisis has increased the number of poor to about a third of the population. Argentina now faces another of the repeated cycles of going from a populist Peronist administration to a free market orthodoxy supporting government, followed by an economic crisis and a shift back to Peronist populist administration policies. Part of the problem is that Argentina, and Brazil, and most of Latin America is still dependent on commodity exports, and the economy dependent on commodity prices. The manufacturing sector has not taken off as it has in Asian countries. This has led to repeated crises in times when the currency reserves declined and affected the currency, also leading to bouts of severe inflation.   ...
