The situation in California on December 12 is dire as its daily cases for coronavirus increase to over 25,000. For Los Angeles county the figure is over 10,000. California coped well with the first wave. As the summer arrived fatigue with lockdown restrictions had increased, social distancing and mask wearing were not followed. When bars and restaurants opened in Los Angeles county huge numbers of people gathered in close proximity. Beaches were crowded.  A further spike in cases is expected from the Thanksgiving period. LA officials are saying one person is dying every 20 minutes. ICU capacity is shrinking. In southern California it has dwindled to less than 10%.  In Santa Clara county in Bay area only 31 ICU beds remain for 2 million residents. San Francisco is expected to run out of ICU beds on December 27, 2020. A patchwork of restrictions varying by county means there is no uniform effort. Fatigue with restrictions means compliance is patchy with social distancing and mask wearing even at the height of this pandemic. Public officials have failed to convince the public to do their part including the governor of the state and the mayor of San Francisco, because of inconsistent messages. ...

2G+ concept in Bundesliga means that football stadiums in Germany are filling up to capacity with fans who are double vaccinated or tested not required to wear masks. Small groups of fans not fully vaccinated are separated in separate locations in the stadium requiring masks and social distancing. The B.1.1.7 coronavirus variant from UK has changed the level of vaccination needed to where life returns to normal, says the Pasteur Institute in France. The level of vaccination has to be higher than the 70% or 80% earlier thought  to be needed for people over age of 18, in the range of 90% now that more infectious variant spread is happening. A look at countries in Europe shows different strategies for tackling coronavirus Delta variant as schools reopen in September 2021. Italy requires all teachers to carry a covid digital certificate that shows vaccination or PCR test results. France, Germany and Spain do not require vaccination for teachers to go to schools, and rely mostly on social distancing and mask requirements in schools. Britain not only does not require vaccination digital certificate, but also has not made masks and social distancing mandatory. Health experts say there are serious risks in this approach with mass return to schools and offices after August 2021, and the fall weather with more time indoors. This could lead to a surge in coronavirus as in the US where the reopening of schools and Delta variant has led to surge particularly in states such as Texas, Florida, California. In UK 65% of total population is now fully vaccinated, in Italy 62%. Teachers in Italy fully support the new rules. In Lombardy region with Milan as the capital, only 300 out of 300,000 workers returned to schools without the green pass, according to regional officials. Britain remains an outlier says this report in the WSJ, taking more risks than is proper at this time, and simply hoping for the best. Not the best strategy in this situation.   ...

So far Italy has resisted the second wave of the coronavirus by adhering to the strict social distancing and mask guidelines better than other parts of Europe. After the traumatic experience in March and April Italians especially in the north near Milan are strictly following the guidelines. The daily number of new infections is at 1851 on October 1, and fatality rate is low.

Teachers and parents see the return to normalcy not lasting long  with the relaxed rules. WSJ was first to point out the dangers of holiday hotspots in Spain, Greece and Croatia in spreading the coronavirus in Europe. One story in the WSJ showed how 21 million tourists visiting Croatia in the summer were not following the basic social distancing and mask guidelines. These tourists returning to Germany, Austria and other countries were spreading the virus after becoming infected in bars and restaurants, shops in Croatia which had not followed the basic guidelines that had been followed earlier in Croatia in April. Over the summer after months of lockdown people simply used fatigue with restrictions as a reason for going the other way and ignoring them.

Mask use is one of the best ways along with social distancing as it reduces the load factor of the virus, reduces the deadly power of the virus. So that the illness even if it happens would not end up being severe, giving patients a chance to recover quickly. Study after study in Germany, in the U.S., in Britain is confirming this. It is also common sense considering that there is a load factor in the virus infection. The bigger the load of virus intake through the nose while breathing in other people's droplets that are then entering our body through receptors in the nose, the bigger the intake, the more severe the infection.

Behaviours of young people going to beaches and bars, ignoring social distancing guidelines and not wearing masks is leading to a rise in the number of daily cases in Europe. This includes an illegal rave in a Berlin park. The incidence of coronavirus is now 19.6 per 100,000 people for persons ages 20 to 29 compared to 9.7 per 100,000 for the French population as a whole. Germany has recorded 900 new cases daily, France 1000, and Spain 2000 as a result of these youth behaviours during the last week.

This report in NYT suggests that coronavirus in New York is becoming endemic. Endemic means chronic instead of the acute crisis situation in 2020. This is because hospitalizations are not filling up ICU's the way they did in 2020. The burden of the coronavirus pandemic is now on people who are unvaccinated and people under 45 years.  New York has a higher rate of vaccination than the rest of the country. About 76% of people ages 12 and over in New York are vaccinated. Yet the 24% of younger people who remain unvaccinated is a part of the population that has fears about getting vaccinated which are harder to overcome. As many of these younger people also spend more time outdoors, and are part of the community, more likely to be in contact with families, relatives and friends indoors in winter, and less likely to wear masks and follow social distancing the pandemic, one could see another surge in the winter of 2021.  Another problem could be that some portion of the population of vaccinated has not taken booster shots by the middle of winter which would create a population of a third of the people even in cities such as New York that remain unprotected. The emergence of another difficult variant could also be a complicating factor. In this way chronic or endemic may still be elusive where masking, social distancing and other preventive actions are not taken. For the community as a whole there will be the risk to upward mobility from the people who risk losing jobs because of fears of the vaccine.     ...
