In fall 2020 India and South Africa submitted a resolution to WTO to suspend IP protection for Covid vaccines, therapeutics and tests, From the Indian perspective this decision comes a bit late when India has already vaccinated over 1 billion people using the Astra Zeneca Oxford vaccine. The Oxford vaccine was made available to Indian manufacturing companies to make locally in a way the could be done at low cost to meet needs of over 1 billion people in India. From the perspective of pharmaceutical companies this is giving away technology even if this was a public health emergency, as shown in this editorial from WSJ.

This NYT editorial says the Pfizer bid to acquire Astra Zeneca is an inversion, a tax maneouvre using openings in U.S. and British tax laws that allows an American company to incorporate in London by acquiring a British company. In effect Pfizer beomes a subsidiary of the British company. This reduces taxes owed even though business takes place at Pfizer as before. NYT says 25 companies have adopted inversions since 2008. The top corporate tax rate in Britain at 20% is lower thant the top rate of 35% in the U.S. Additional tax maoeuvres could make it easier for income shifting to tax havens, and make it possible to free up cash held in foreign countries without the need to first repatriate it to the U.S. The problem is that such maneouvres are taking place at a time of large U.S. deficits, and deep cuts in public services and government investment in R&D, infrastructure, education, that would lead to future economic growth.

Kate Bingham, head of the Vaccine Task Force in Britain is commended for her intelligent approach in placing bets on vaccines in different technologies, "four buckets" as she calls them. This includes the Moderna and Pfizer in the mRNA technology and the viral vector Astra Zeneca, J&J vaccines. This approach made the British vaccination drive effective by being supported by a resilient supply system.

The Indian government has supported the effort to get several companies to make the Sputnik Russian vaccine in India in an effort to diversify supplies. Reddy Labs is one of the major manufacturers working on the Sputnik vaccine in 2021.

It is provided at cost, and storage is in ordinary refrigerators for long periods, with production in India of large quantities of the vaccine, making it a vaccine that could reach large parts of the world's population.

The U.S. government has allocated massive investments with different manufacturers to ensure that a vaccine is available. Worldwide demand is about 2 billion doses over 2 years. China faces risk of a surge inthe coronavirus in June 2021. The area in and around Guangzhou appears to be seriously affected. The city tested almost its entire population of 18.7 million between June 6 Sunday and June 8 Tuesday. This report shows pictures of a deserted Beijing airport, strict restrictions on foreign travel. The SinoPharm vaccine effectiveness against the Delta variant in India and UK is unknown. The government is locking down entire neighborhoods rather than entire cities or provinces.  As the risks of the Delta variant and other new variants increases most of the population even in the US and Europe have either no dose or one dose. Researchers at the University of New South Wales in Australia show the Astra Zeneca vaccine effectiveness with one dose at only 30%, only after two weeks following the second dose does the vaccine effectiveness reach about 70%. The population of China and India are so large that much larger parts of the population remain unvaccinated. In China with 1.3 billion people and even if the figure of 800 million doses stated by the government is accepted- it could be an overestimate as the US has only managed 300 million doses with many vaccines- most of the population is unprotected. Vaccine skepticism is high in China making vaccination an uphill task. SinoPharmvaccine is not as effective as Pfizer, Moderna, Astra Zeneca, or Covaxin vaccines, making the task even more of an uphill kind. ...

The Oxford vaccine is shown to be effective for older adults over 70 years in age. One of the key features of the Oxford vaccine is that it is designed to be accessible in cost for not just high income countries but across all parts of the world including the countries in Asia, Africa and Latin America where some of the poorest people live. Because India has strong already established manufacturing capabilities for vaccines and is collaborating with Oxford and Astra Zeneca for a low cost vaccine it is in a position to drive the campaign for an effective plus low cost 100% accessible vaccine for people around the world. Jenny Strasbourg of the WSJ provides this much needed report from London about the courageous decision by AstraZeneca and Oxford University to give vaccines away at no profit to the whole world, to billions of people in Africa, Asia, and Latin America. Without this brave decision by a British company and a British University the world would be a lot poorer, more variants could have happened, making us realize the great contribution Britain has made and how indispensable it is to the planet. Add to this the effort of Indian companies including Serum Institute that provided the manufacturing facilities and capabilities for making most of the British vaccine. AstraZeneca delivered 2.3 billion doses of the vaccine globally as of mid-December, according to the company. The International Monetary Fund estimates that low and middle income countries received 3.25 billion vaccines as of Dec. 11, About half of this or 1.6 billion doses were Astra Zeneca shots. This is a bigger share than any other vaccine by far and a life saver to the world. AstraZeneca stepped up early in a true to the best ideals in Britain to meet the needs of the world-  aiming to deliver 3 billion doses in 2022 and sell them at no profit as long as the pandemic continues. As the shot does not need cold storage it is ideal for India and other Asia, Africa and Latin America. "We are all very proud throughout the company of the impact we have had," says AstrZeneca CEO Pascal Soriot. By far the biggest manufacturing was done at Serum Institute of India which supplied 1.3 billion doses of the AstraZeneca vaccine to 70 countries. Serum Institute can produce as much as 250 million doses of vaccine a month making it possible for India to tackle the vaccination population of 1.3 billion people.   None of this could have happened without Oxford University and AstraZeneca and Indian companies with Mr. Modi's active support living up to the best ideals of Britain and India for the world. European Union appears to be lagging behind in arranging purchases of vaccine supplies, with UK and US ahead. The shortages in Europe of vaccine, and limited supplies of the Astra Zeneca vaccine to the EU, is resulting in a nasty argument with the UK. At one point the EU planned to limit vaccine exports from the Pfizer Belgian plant to the UK, including closing off the Northern Ireland border.  That move came under criticism from EU's Michael Barnier.

Astra Zeneca faces over 9000 lawsuits from people who developed diabetes after taking it. Its a drug for bipolar disorder and schizophrenia that has sales of $20 billion over the years, with $4 billion in 2008. Now it is revealed that Astra Zeneca conducted studies on the effectiveness of Seroquel from 2000, and these studies show that Seroquel was less effective than a generic medicine haloperidol, that has been around for 50 years. Seroquel was only more effective than a placebo,the analysis says from hundreds of pages of unsealed documents.

India plans to vaccinate 300 million people in the first drive of the vaccination campaign. The two vaccines approved are the Covishield from Astra Zeneca partnership with Serum Institute of India, and the Bharat Biotech vaccine. India is the largest vaccine manufacturer in the world, and its vaccination production and campaign is being watched closely.  The largest maker of vaccines is Serum Institute of India based in Pune. Countries that are seeking India's help in vaccines are Brazil, South Africa, Morocco, Saudi Arabia, Bangladesh. Brazil has ordered 1.5 million vaccine doses. The Indian government has approved sending vaccines to other governments. Price of 100 million doses to be supplied by Serum Institute to the Indian government is 200 rupees a dose or about $2.73 a dose. Vaccine sold in private markets will be priced at 2000 rupees a dose. A series of voicemails and emails in court documents now disclosed, show that AstraZeneca tried to suppress findings about diabetes after effects in taking its psychiatric drug Seroquel. In an August 15, 2005, voicemail sent to company salespeople an employee Christine Ney, followed up on a"weight and diabetes sell sheet" they had recently sent. It said that the salespeople should assuage doctors' fears about their patients' weight gains, telling them that the data did not show any causal link between diabetes and the drug. "Our objective is to neutralize customer objections to Seroquel's weight and diabetes profile", Ms Ney said in the voice mail message. She instructed representatives to "refocus the call" away from diabetes to the drug's tolerability. While all this was going on and years before this, Astra Zeneca concealed a drug safety expert's own assessment of Seroquel's relation to diabetes. In a 2000 position paper about the safety of Seroquel sent to Dutch regulatory authorites, an AstraZeneca doctor named Wayne Geller wote that there was a relationship between the drug and diabetes. He wrote " there is reasonable evidence to suggest that Seroquel therapy can cause impaired glucose regulation including diabetes melliutus in certain individuals."...

The third or booster dose of vaccine is needed to take out the Omicron variant. The UK Health Security Agency analyzed data from 581 Omicron cases and thousands of Delta cases to calculate how effective vaccines such as Pfizer and Astra Zeneca were against the Omicron variant. This early analysis shows a third booster dose prevents around 75% of people from getting any coronavirus symptoms. The double dose of the vaccine however provides good protection against severe coronavirus that needed hospital treatment says the UK Health Security Agency. Of equal concern is the rate of spread of the Omicron variant. Here this BBC report shows graphs of UK National Health Security Agency which show the rate of spread is rapid with cases doubling every 2-3 days. For the UK which on December 10 had about 1265 cases this means says this BBC report that the number of Omicron cases could be well above 100,000 in the UK by the end of December. The BBC graph shows the curve for Omicron cases moving in a close to vertical direction upwards. There are 100 vaccines being developed all over the world, of these 30 are in India. One is a collaboration with Britain for the Oxford Astra Zeneca vaccine to be produced largely in India. The other is one by ICMR. Other vaccines are the Johnson and Johnson  one dose onl vaccine. These vaccines have the advantage of not requiring ultra low temperature storage. 

Even though the Pfizer BioNTech vaccine is off to a start in Britain this does not mean that the other vaccines are not going to be out soon as early as by the end of December. There is also no clarity on how long the protection lasts. More than the one billion doses of the Pfizer vaccine will be required making the other vaccines just as or even more important. Asia and Latin America, Africa are still not in the picture and will have to be for the protection to be effective.

Germany's chancellor Angela Merkel says she will not support the moves by the EU commission to impose export bans on export of Astra Zeneca vaccines made in EU countries to Britain. A Dutch factory has 4 million doses of this vaccine ready for shipment.  Merkel says, "there are a huge range of international interdependencies when it comes to vaccine production. You have to be very careful when it comes to imposing export bans. France and Germany see the need for the principles of "reciprocity" and "equivalence" to be covered in settling the differences on vaccine supplies. The rhesus macaques given the vaccine at a Montana lab did not get coronavirus.The surface protein of coronavirus is simply added to the ChAdox1 so that it is able to attach to host cells and infect them in the human body, and let antibodies develop from this. Oxford Biomedica is the company that is part of the consortium making the coronavirus vaccine being developed by Oxford University's Jenner Institute.  Her Mr. Dawson describes the challenges he faced and cash crunches 4 times in 12 years, the last 4 years ago. The turning point he says was in 2012 when the cell and gene therapy was validated with a new drug developed for a form of cancer using this method. Oxford Biomedica is setting up a facility for manufacturing the vaccine in England at a 84,000 square foot former Royal Mail sorting facility in the city's business park called Oxpark. Dawson says cell and gene therapy is going to be big in health care. He did not see it coming till 2012. The Oxford vaccine can be stored in a fridge, the others use mRNA technology of messenger RNA which requires ultra low temperature storage. Astra Zeneca could have handled the trials and methodology for results in a better way. As the two trials one that produced results of 62% and the other results of 90% cannot be combined to give results of 70% but are two distinct and separate trials. However too much emphasis has been placed on the vaccine, as other prevention measures remain important for 2021. Other vaccines are being developed in Britain with new technology and in India by ICMR which are in trials stage and about which not enough is known.
